Consecutive cutaneous and visceral leishmaniasis manifestations involving a novel Leishmania species in two HIV patients in Thailand.

BACKGROUND: Satisfactory internal fixation of comminuted radial head fractures is often difficult to achieve, and radial head resection has been the accepted treatment. In this study, we compared the results of radial head resection with those of open reduction and internal fixation in patients with a comminuted radial head fracture. METHODS: Twenty-eight patients with a Mason type-III radial head fracture (some with associated injuries) were enrolled in the study. Fifteen patients underwent radial head resection as the initial treatment (Group I), and thirteen patients underwent open reducti

Physeal Bar Formation After Salter-Harris III and IV Medial Malleolus Fractures in Children Aged ≤10 Years.

INTRODUCTION: Salter-Harris (SH) III and IV medial malleolar fractures in young children pose a risk of physeal bar formation and subsequent growth disturbance. The aim of this study was to compare surgical versus nonsurgical treatment and to identify factors predictive of physeal injury. METHODS: A retrospective cohort analysis examined patients aged ≤10 years treated for SH III/IV medial malleolar fractures between 2012 and 2024 at a single institution. Patients received either surgical (open reduction and internal fixation (ORIF) or percutaneous reduction and screw fixation (PRSF)) or nons

[Fractures of the tibial tuberosity associated with avulsion of the patellar ligament in adolescents].

PURPOSE OF THE STUDY: Avulsion fracture of the tibial tuberosity is considered as a relatively uncommon adolescent injury. The avulsion of the patellar ligament may occur as part of the injury. Two cases of sixteen year old boys with this kind of injury have been reported. Fractures are described using Ogden et al. classification : type I is an avulsion fracture of the most distal portion of the ossification center of the tubercle ; type II, an upward angulation of the lip fragment ; type III, a fracture of the lip fragment with propagation of the fracture-line into the knee joint. Each type i

Radiological findings of orbital blowout fractures: a review.

Purpose: To summarize the radiological findings in patients with orbital blowout fractures. Methods: We reviewed the published literature on radiological findings of orbital blowout fractures that were searched on PubMed and included our own radiologic findings on patients with orbital blowout fractures that were seen at our hospital. Results: Radiologic examination reveals a variety of findings in each case. However, common radiological findings of orbital blowout fractures include comminuted/unhinged, hinged, and linear fractures. These fractures are usually located in the orbital floor medi

The arthroscopic management of intra-articular distal radius fractures.

Intra-operative arthroscopy and fluoroscopy provide improved visualisation and guide the restoration of intra-articular distal radial fractures while minimising the operative dissection required for their stabilisation. Radial styloid fractures, distal radial fractures with dorsal, palmar or combined ulnar-sided "die punch" fragments, palmar and dorsal Barton's fractures, and various three- and four-part intra-articular fractures without significant bone loss or defect are especially suited for this technique. The experienced arthroscopist may wish to apply the technique to more severely commi

Controversies in the Management of Bicolumnar Fractures of the Distal Humerus.

Bicolumnar fractures of the distal humerus pose numerous treatment challenges for upper-extremity surgeons. Although open reduction and internal fixation demonstrates advantages compared with nonsurgical treatment, restoration of osseous anatomy can be difficult, particularly for comminuted, intra-articular fractures. Despite well-recognized complications, total elbow arthroplasty remains an option for elderly patients with fractures not amenable to fixation. Although indications remain controversial, distal humerus hemiarthroplasty has emerged as a potential alternative to total elbow arthrop

A prospective study of 924 digital fractures of the hand.

A prospective study of 1129 patients with 1358 digital fractures of the hand was carried out between March 1984 and March 1994; of these, 924 fulfilled the criteria for inclusion. Functionally stable fractures with acceptable alignment were managed with immediate active mobilization. Unstable fractures, or fractures with unacceptable alignment, were openly reduced and fixed internally so that active mobilization could begin immediately after operation. The function results using various systems of evaluation were excellent or good in over 90 per cent of the fractured thumbs, and excellent or g

[Open fractures of the elbow joint].

Both in open and in closed elbow fractures, anatomical joint reconstruction and rigid internal fixation, which permit early initiation of motion, are essential for good joint function. However, in open distal intraarticular humerus fractures this is more difficult to achieve, owing to the high rate of comminuted types C 2 and C 3 fractures. In addition, the extensive soft tissue lesions often mean that physiotherapy cannot be started until later than would otherwise be desirable. This retrospective study shows that the functional results in 60 patients operated on for correction of open distal

Proximal median neuropathy secondary to humeral neck fracture.

Median neuropathies proximal to the wrist are uncommon and usually result from penetrating injuries, fracture dislocation of the distal humerus, or compression by fibrous bands. A 66-year-old man suffered a comminuted fracture of the proximal humerus after a fall. Electrodiagnostic studies revealed a severe proximal median neuropathy and a mild distal radial mononeuropathy. Proximal median neuropathy rarely occurs in humeral neck fracture, mostly because the median nerve is not in close contact with the humerus proximally.

Autopsy Features of Fatal Donkey Attack.

Lethal donkey attacks have very rarely been described. The case of a 65-year-old man who was found deceased on a country road with 2 domestic donkeys nearby is, therefore, reported. Examination of the body revealed contusions and lacerations of the face and scalp, a comminuted fracture of the left maxilla, comminuted fracturing of the right radius and ulna and of the left anterior superior iliac spine, a flail chest, and pulmonary contusions. In addition, there were bite marks on the left thigh, right buttock, right axilla/upper arm, and left cheek which corresponded to the dental arcades of t

Three Year Review of Dorsal Plating for Complex Intra-Articular Fractures of the Distal Radius.

BACKGROUND: The volar approach is commonly used for plating intra-articular fractures of the distal radius. Despite this, certain fracture configurations are more suitable for dorsal plate fixation. This technique has not gained favour due to the reported high incidence of extensor tendon irritation and attrition ruptures. With the advent of lower profile plates this risk has decreased. METHODS: We report on forty-six cases performed in a tertiary hand centre between January 2011 and May 2014. Patients were identified from a database of distal radius fractures treated with open reduction and

Skull fracture and haemorrhage pattern among fatal and nonfatal head injury assault victims - a critical analysis.

BACKGROUND: The global incidence of fatal head injuries as the result of assault is greater than the number of non-fatal cases. The important factors that determine the outcome in terms of survival of such head injury cases include the type of weapon used, type and site of skull fracture, intra cranial haemorrhage and the brain injury. The present study aims to highlight the role of skull fractures as an indirect indicator of force of impact and the intra cranial haemorrhage by a comparative study of assault victims with fatal and nonfatal head injuries. METHODS: 91 head injury cases resultin

Dorsal plating for distal radius fractures.

A dorsal approach to the distal radius for comminuted intra-articular fractures affords excellent exposure of the joint surface. Dorsal plating can reliably buttress the joint, leading to low rates of arthrosis, but at the expense of wrist stiffness and high rates of extensor tendon problems. New, smaller implants may prove capable of supporting the joint with lower rates of extensor irritation.

Traumatic fracture of the patella in 11 cats.

Traumatic patellar fracture in cats mainly occurred in male neutered cats with an average age of four years, six months and a mean weight of 4.8 kg. Patellar fractures were concurrent with long bone fractures and cruciate ligament rupture or occurred after gunshot. The patella luxated medially or laterally in four cats. Fracture fragments were usually small, either from the proximal pole (base) or distal pole (apex) or longitudinal fragments. Treatment was by fragment excision when the fragment was small or by reconstruction with pins and wires when fractures were comminuted. The outcome in si

[Injuries of the lumbosacral segments of the spine].

Having studied 470 case histories of patients with injuries of the spine the author found traumas of the 5th lumbar vertebra in 23 cases (4.9%). The characteristic injuries were compression wedge-shaped fractures, multiple fractures of the vertebrae, comminuted and traumatic spondylolistheses and dislocation fractures. A typical mechanism of the injury was a blow in the lumbar region or in the back with subsequent bending of the trunk. The tactics of surgical treatment of such lesions is presented, the main tasks of which are the reconstruction of the shape and the volume of the vertebral cana

Medial subtalar dislocation associated with fracture of the posterior process of the talus.

A 17-year-old male sustained a medial subtalar fracture dislocation while playing basketball. The dislocation of the subtalar joint was reduced by closed means, but a large, comminuted, displaced posterior process fracture remained. Open reduction and internal fixation through a posteromedial approach was used to restore congruity to the subtalar joint. MRI at 12 months after injury showed no signs of avascular necrosis in the talus.

Iatrogenic subtrochanteric fracture: a complication of Zickel nails.

We studied the cases of four patients who sustained a fresh femoral subtrochanteric fracture during late removal of a Zickel nail that had been inserted for treatment of a fracture. The configuration of the intramedullary device appeared to be a significant factor in the production of the complication. Comminution of the proximal part of the femur (sufficient to require supplementary fixation with interfragmentary screws) and relative youth of the patient may increase the incidence of the complication.

Risk Factors of Nonunion After Anterior Screw Fixation of Anderson and D'Alonzo Type II Odontoid Fractures.

BACKGROUND/AIM: Anderson-D'Alonzo Type II odontoid fractures are common in the elderly and often treated surgically due to high nonunion rates and increased morbidity and mortality associated with conservative treatment. However, high rates of nonunion still occur with surgical treatment and specific fracture characteristics predicting nonunion remain unclear. This study aimed to identify morphological fracture features associated with nonunion in patients undergoing anterior screw osteosynthesis for Type II odontoid fractures. PATIENTS AND METHODS: In this retrospective study, 60 adult patie
