Acrylic frontal cranioplasty.

The correction of contour defects of the frontal bone has been accomplished with a variety of autogenous and alloplastic materials. We report our experience in reconstructing 11 cases of congenital, traumatic, and postsurgical frontal defects with acrylic. The acrylic was cured intraoperatively within the defect in some patients, or prefabricated into a prosthesis and wired into position in others. The operative techniques for both methods of repair are detailed. The reconstruction was successful in all cases and there were no complications. The acrylic implant has been found to be well tolera…

Subarachnoid Osteoma of the Right Frontal Lobe.

BACKGROUND: Intracranial osteoma arising from nonosseous tissue and surrounded by brain parenchyma is extremely rare. We report an intracranial osteoma surgical case with no heterotopic ossification. CASE DESCRIPTION: A 32-year-old woman presented with headache, vertigo, and weakness. Preoperative neuroimaging revealed a spherelike, calcified intracranial lesion in the right frontal region. The bone-hard mass was completely removed by right frontal craniotomy; adhesion and invasion of the skull inner plate and dura were not found. The subarachnoid lesion was surrounded by right middle frontal

Growth in hemifacial microsomia studied with the aid of roentgen stereophotogrammetry and metallic implants.

Displacement of the mandible and of the maxillary bones with growth was studied in 11 children with hemifacial microsomia. The subjects were from three to 14 years of age at the initial examination and were observed over a period of 350 to 1218 days. Roentgen stereophotogrammetry with the aid of metallic implants proved to be an efficient method for highly accurate recordings of growth relative to the frontal bone. Complicated movements of the jaws occurred and either increased or decreased the degree of facial asymmetry. No correlation could be found between the extent of the mandibular defor…

Primary ectopic meningioma.

Primary ectopic meningiomas are rare. A case of a 16-year-old male who presented with a large mass in temporofrontal region is reported. X-ray skull showed soft tissue shadow with hyperostosis of frontal and temporal bone. CT scan of brain demonstrated a markedly enhancing lesion and bony hyperostosis with no intracranial component. Total excision of tumour with hyperostotic bone was done. Patient is well without any evidence of recurrence two years after surgery. Relevant literature is reviewed.

Cranial reconstruction in osteogenesis imperfecta.

Although osteogenesis imperfecta (OI) is recognized as a cause of craniosynostosis, therapeutic endeavors have not been reported. We present an 8-month-old girl with type 3 OI, in whom bilateral occipital flattening, biparietal widening, and frontal narrowing were effectively managed with a combined surgical release and reconstruction and molding cap therapy. The quality of the calvarial bone in OI requires a modified approach to the conventional bone techniques commonly used in the correction of craniosynostosis deformities.

A proton magnetic resonance spectroscopy study of the chronic lead effect on the Basal ganglion and frontal and occipital lobes in middle-age adults.

BACKGROUND: Lead is known to be a health hazard to the human brain and nervous system based on data from epidemiologic studies. However, few studies have examined the mechanism or biochemical changes caused by lead in the human brain, although recently some have used magnetic resonance spectroscopy (MRS) to test brain metabolism in vivo. OBJECTIVES: In this study, we used 3-T MRS to investigate brain metabolism in workers chronically exposed to lead and matched nonexposed controls. METHODS: Twenty-two workers at a lead paint factory served as chronically exposed subjects of this study. These

[Endoscopic feasibility study and nasal septum median path of frontal sinus surgery].

OBJECTIVE: To discuss the feasibility of endoscopic frontal sinus surgery in the nasal septum median path. METHOD: (1) Sixty adult cadaveric heads fixed with formalin were CT scanned, and were three dimensional reconstruction. (2) Thirty adult cadaveric heads were sawn along the sagittal line close to the side of the nasal septum, then the important anatomic marks were observed and measured. (3) Combined with CT and anatomical data, thirty adult cadaveric heads were operated in different degree, and the damage of nasal septum and fila olfactoria were detected in the same time. RESULT: (1) The

Diagnosis and Management of Suture-Related Concerns of the Infant Skull.

The cranial fontanelles and sutures have several benign variations, including most cases of "early" or "late" closure of the anterior fontanelle, bathrocephaly, overriding sutures, and benign metopic ridging. However, recognizing true craniosynostosis and referring the patient to a craniofacial specialist in a timely fashion are imperative, as minimally invasive options can be offered to most patients younger than 6 months of age. Gaining comfort with the physical examination of an infant with an abnormal head shape is best achieved through experience and pattern recognition and will frequentl

Subsegmental Osteotomy Technique With Distraction Osteogenesis for Natural Frontal Contouring in Metopic Synostosis.

Vectoring issues occur due to the dynamics of unidirectional procedure that is selected. To circumvent such difficulties, we have devised subsegmental osteotomy with distraction. In our technique, frontal bone flap was divided into 4 segments that were linked to each other by suturing wires in an 8-year-old patient with trigonocephaly. Subsegmental osteotomy and wiring of bone flap enabled us to slide bone in a more curvilinear manner and achieve natural contour. Distraction was initiated on postoperative day 7 at the rate of 1 mm/d for 20-mm gain. At the 9-year follow-up, the patient showed i…

Frontal sinus cementifying ossifying fibroma.

Cementifying ossifying fibroma is a fibro-osseous lesion which produces cementum. Cementifying ossifying fibroma is a mesodermal, non-odontogenic tumor of ectopic multipotential periodontal membrane blast cells. Although, it is a benign bone tumor, it has an aggressive destructive behavior in patients under 20 years of age with high tendency for recurrence. Cementifying ossifying fibroma can be recognized microscopically by the presence of cementicles. This is a case of frontal sinus cementifying ossifying fibroma presenting with proptosis in a 39-year-old male.

Hybrid of distraction osteogenesis unilateral frontal distraction and supraorbital reshaping in correction of unilateral coronal synostosis.

Unicoronal synostotic plagiocephaly is routinely treated by intracranial wide frontal and bilateral supraorbital reshaping. Recent advancement of distraction osteogenesis in craniofacial surgery has extended to patients with craniosynostosis. Although a controversy remains between conventional osteotomy and reshaping and application of the distraction technique in surgical treatment of craniosynostosis, there have been several positive clinical reports on distraction techniques for nonsyndromic and syndromic craniosynostosis. Unicoronal distraction applied successfully to a case of frontal pla

[Facio-craniostenosis].

Facio-craniostenosis. Facio-craniostenosis is associated with premature stenosis of one or several cranial sutures. The clinical variety is defined by the affected suture. In the complex forms, major disturbances of the anterior fossa of the cranial base are observed. Intracranial hypertension sometimes observed may lead to impairment of the function of the brain or the eyes. The authors have observed 73 children with craniostenosis and have operated upon 49 of them. Three types of methods have been used. The stress is made on the latest one combining advancement of the orbito-frontal "bandeau

Fronto-orbito nasal dislocations. Initial total reconstruction.

The craniofacial trauma can produce compound fractures with bone displacement in the central part of the upper face, i.e. the bones constituting the forehead, orbit, and nose. such dislocations are called fronto-orbito-nasal dislocations. A total and definite surgical reconstruction in one stage offers advantages such as good aesthetic and functional results. An injured person can enter professional and social life without further delay. A major advantage is also the minimizing of the risk of meningeal fistula with infectious mortal risk. A prerequisite for this surgery is accurate clinical an

[Fronto-orbital osteotomies for craniostenoses. 21 cases (author's transl)].

Three types of craniostenoses, plagiocephalies, trigonocephalies, and oxy-brachycephalies provoke modifications in the fronto-orbital region of the face, for which fronto-orbital remodelling osteostomies are proposed. Results obtained in 21 cases treated in this manner were very encouraging, as they were of good quality.
