[The guideline 'Hearing Impairment' of the Dutch College of General Practitioners; a comment from the Netherlands Society of Otolaryngology and Cervico-Facial Surgery].

This guideline has been well thought out, and is a useful basis for the general practitioner to diagnose and manage hearing loss. However, it advises watchful waiting in children with otitis media, a questionable view. In evaluating simple diagnostic tools, the speech recognition is overvalued at the expense of audiometry and the tuning-fork test. Posttraumatic perforation of the eardrum, in combination with hearing loss is incorrectly presented as a self-limiting problem in the majority of cases.

(AI)lephant in the Room: Utility of Generative AI to Develop Discussion Guides for Cardiac Surgery Morbidity and Mortality Conferences.

OBJECTIVE: Morbidity & Mortality Conferences (M&Ms) are forums to assess adverse outcomes and to analyze their root causes. We sought to utilize a large language model (LLM) to generate discussion materials for cardiac surgery M&Ms, based upon data available in the electronic medical record (EMR). DESIGN: EMR data, including information from notes, bloodwork, vital sign monitoring, and the medication administration record, were input into a private instance of GPT-5. A prompt was iterated to accomplish three tasks, resulting in the generation of (1) a case outline, (2) a discussion table to i

Mathematical modeling of flow-mediated thrombin generation after treatment of postoperative bleeding in cardiac surgery with cardiopulmonary bypass.

BACKGROUND: Postcardiopulmonary bypass (CPB) bleeding is partly driven by impaired thrombin generation. Recent trials suggest differing clinical effects of fresh frozen plasma (FFP) and prothrombin complex concentrate (PCC), but conventional and static assays do not resolve their hemostatic mechanisms. OBJECTIVES: This study aimed to compare FFP and PCC effects on simulated post-CPB thrombin generation and identify mechanisms underlying varied treatment response. METHODS: We used a mathematical model of flow-mediated coagulation to simulate thrombin generation in virtual patients with physio

Development and validation of the airway surgery enclosure for high-risk aerosol-generating airway procedures: a bench and clinical study.

Procedures on the upper airway in patients with respiratory viruses are considered to carry the greatest risk of infection spread to operating room personnel through aerosolization. Appropriate personal protective equipment must be worn, but availability varies worldwide and resources may be limited. We describe the development, validation, and safe implementation of a reusable enclosure with an inexpensive, acrylic design, for use in high-risk airway procedures. Examples of common yet high-risk, aerosol-generating procedures performed with the Airway Surgery Enclosure (ASE) include laryngo-br

First-generation bypass surgery for a giant fusiform aneurysm of the middle cerebral artery: an illustrative case and surgical video.

Giant fusiform aneurysms of the middle cerebral artery (MCA) are complex and rare vascular lesions with a poor natural history and challenging treatment decision-making. We report the case of a 46-year-old male with a history of chronic hypertension and a transient ischemic attack who presented with left-sided hemiparesis. A cerebral angiotomography revealed an unruptured giant fusiform aneurysm in the M2 segment of the right MCA. After carefully evaluating the procedure's risks and benefits with the patient, he underwent a low-flow bypass surgery. An anastomosis between the superficial tempor

Novel approach to intraoperative peritoneal lavage with an extracorporeal stirring method in laparoscopic surgery for generalized peritonitis: Preliminary results.

INTRODUCTION: The primary concern with laparoscopic intraoperative peritoneal lavage (IOPL) for generalized peritonitis relates to the difficulty and uncertainty in ensuring adequate washout of contaminated fluid. Here, we describe a new method of laparoscopy-assisted IOPL. METHODS: We performed emergency surgery in 10 patients with generalized peritonitis necessitating IOPL. A small wound retractor was inserted into the abdominal cavity through an incision and elevated to raise the abdominal wall. More than 3-L saline was injected via the retractor at one time. The abdomen was manually shake

[Risk factors generally neglected in oral surgery and implantology: the high LDL-cholesterol and the insufficient level of vitamin D].

INTRODUCTION: Vitamin D deficiency is widespread throughout the world. Vitamin D has an important role in the regulation of phosphocalcic metabolism as well as in a large number of biological and metabolic processes. According to some studies, there is a correlation between vitamin D and LDL-cholesterol levels. A deficiency of vitamin D and / or a high level of LDL-cholesterol could represent risk factors for bone healing and osteointegration of dental implants. The purpose of our study is to demonstrate the reality of the problem of deficiency or deprivation in vitamin D in a population of pa

Approaches to Peripheral Nerve Repair: Generations of Biomaterial Conduits Yielding to Replacing Autologous Nerve Grafts in Craniomaxillofacial Surgery.

Peripheral nerve injury is a common clinical entity, which may arise due to traumatic, tumorous, or even iatrogenic injury in craniomaxillofacial surgery. Despite advances in biomaterials and techniques over the past several decades, reconstruction of nerve gaps remains a challenge. Autografts are the gold standard for nerve reconstruction. Using autografts, there is donor site morbidity, subsequent sensory deficit, and potential for neuroma development and infection. Moreover, the need for a second surgical site and limited availability of donor nerves remain a challenge. Thus, increasing eff

Multilevel measurement of acetabular version using 3-D CT-generated models: implications for hip preservation surgery.

BACKGROUND: Numerous structural deformities of the acetabulum are associated with hip osteoarthritis (OA). Acetabular retroversion has been implicated in the development of femoroacetabular impingement. However, it is unknown if retroversion occurs from isolated excessive bone in the superior acetabulum or a torsional phenomenon of the entire pelvic segment. QUESTIONS/PURPOSES: A method was developed to measure acetabular version (AV) using 3-D models thus eliminating positional factors. This method was used to determine if acetabular retroversion is an isolated entity or an effect involving

An In Situ-Generated Near-Infrared Chemiluminescent Nanoprobe for High-Contrast Lymph Node Mapping and Tumor-Guided Surgery.

Chemiluminescent nanoprobes have attracted increasing attention in bioimaging owing to their excitation-free emission and high signal-to-background ratio. However, most reported self-sustaining chemiluminescent nanosystems rely on strong photosensitizers to generate large amounts of reactive oxygen species to initiate the emission process. Although this strategy may result in deep tissue penetration, the excessive ROS accumulation could raise potential phototoxicity concerns. In this work, we designed a bioimaging probe in which the ROS generation, ROS trapping, and near-infrared (760 nm) chem

A Clinically Compatible Method for Generating Preoperative Finite Element Models to Simulate Facial Appearance and Movements in Orthognathic Surgery.

This study proposes an innovative method for generating patient-specific 3D models of the facial soft tissue, which can be integrated into clinical routine to predict the consequences of orthognathic surgery. This surgery, which impacts both facial aesthetics and functionality, requires precise tools tailored to each patient. To this end, a 3D face model has been developed, integrating anatomically precise bone structures (mandible and maxilla) and soft tissues (skin, fat, and muscles). This reference model is then fitted to each patient's anatomy using a nearly automatic method. This process

Comparing Regional Anesthesia and General Anesthesia for Postoperative Pain Control in Abdominal Surgeries: A Systematic Review and Meta-Analysis.

Good postoperative pain management is really significant for patient recovery and satisfaction, and reduction of complications. Regional anesthesia (RA) has become popular and is used increasingly during abdominal surgeries instead of general anesthesia (GA) due to the benefits such as better pain control, lower opioid consumption, and quicker recovery time. This systematic review and meta-analysis aimed at contrasting the security and efficiency of RA against GA in relieving postoperative pain during abdominal surgeries. A systematic search of databases such as PubMed, Embase, and Cochrane wa
