A 67-year-old female suffering from diabetes mellitus type I revealed brownish-yellow discolouration, subungual hyperkeratosis, and onychodystrophy of several finger nails. Upon culturing of nail specimens, Trichophyton gallinae could be repeatedly identified on Sabouraud glucose agar. To our knowledge, this is the first report of a human infection caused by this species in Germany. Milker's nodules (pseudocowpox) are harmless skin lesions most commonly seen in persons whose occupation regularly brings them into close contact with cattle. They are caused by the paravaccinia virus, a member of the poxvirus family. Awareness of their clinical and histopathologic features is important, especially with today's heightened awareness of the threat of biologic warfare, to distinguish them from more worrisome entities such as tularemia and anthrax.

We report the case of a 35-year-old man with acquired palmar spiny keratoderma (SK) in association with type IV hyperlipoproteinemia. Tinea nigra usually presents as a brown to black macule on the palmar or plantar skin and is sometimes misdiagnosed as a malignant melanoma or as a junctional nevus, prompting unnecessary surgical procedures and anguish for the patient. Superficial scraping of the skin for microscopic inspection with potassium hydroxide reveals pigmented hyphae, easily confirming the diagnosis of tinea nigra.

Mycosis fungoides (MF) is a cutaneous lymphoma of presumed T-helper cell origin. It usually presents with patches and infiltrated plaques, but a wide range of atypical forms have also been described. We report a 48-year-old man who had a 2-year history of a pruritic, warty plaque on the right palm with surrounding poikilodermatous changes. He subsequently developed additional macules of poikiloderma atrophicans vasculare (PAV) on the right upper limb and abdomen.
