Propensity for Intra-abdominal and Hepatic Adiposity Varies Among Ethnic Groups.

BACKGROUND & AIMS: We compared fat storage in the abdominal region among individuals from 5 different ethnic-racial groups to determine whether fat storage is associated with disparities observed in metabolic syndrome and other obesity-associated diseases. METHODS: We collected data from 1794 participants in the Multiethnic Cohort Study (60-77 years old; of African, European [white], Japanese, Latino, or Native Hawaiian ancestry) with body mass index values of 17.1-46.2 kg/m2. From May 2013 through April 2016, participants visited the study clinic to undergo body measurements, an interview, a…

A severe case of rat lungworm disease in Hawa'i.

A 23-year-old man living on the island of Hawa'i developed a life threatening case of eosinophilic meningitis caused by infection with Angiostrongylus cantonensis (rat lungworm disease: RLWD). He was comatose for 3 months, incurring brain and nerve damage sufficiently extensive that he was not expected to recover. The case was complicated by secondary infections of methicillin-resistant Staphylococcus aureus, Clostridium difficile, and pneumonia, which resulted in an empyema requiring a thoracoscopy and decortication. He was treated with prednisone, mebendozal, and pain medication for RLWD, an

Ankle-to-brachial index and dementia: the Honolulu-Asia Aging Study.

BACKGROUND: Measurement of the ankle-to-brachial index (ABI) is a noninvasive test to assess peripheral arterial disease. A low ABI is a strong correlate of cardiovascular disease and subsequent mortality. Evidence indicates the existence of vascular components in the pathogenesis of dementia. Here, we examine the association of ABI with dementia and subtypes. METHODS AND RESULTS: Data are from the Honolulu-Asia Aging Study (HAAS), a prospective community-based study of 3734 Japanese American men 71 to 93 years of age at baseline in 1991 to 1993. The analysis included 2588 men who were free o

Are There Modifiable Risk Factors for Hospital Readmission After Total Hip Arthroplasty in a US Healthcare System?

BACKGROUND: Although total hip arthroplasty (THA) is a successful procedure, 4% to 11% of patients who undergo THA are readmitted to the hospital. Prior studies have reported rates and risk factors of THA readmission but have been limited to single-center samples, administrative claims data, or Medicare patients. As a result, hospital readmission risk factors for a large proportion of patients undergoing THA are not fully understood. QUESTIONS/PURPOSES: (1) What is the incidence of hospital readmissions after primary THA and the reasons for readmission? (2) What are the risk factors for hospi

Body mass index and thromboembolic stroke in nonsmoking men in older middle age. The Honolulu Heart Program.

BACKGROUND AND PURPOSE: While evidence suggests that obesity has an independent relation to coronary artery disease, similar findings for stroke have not been established. The purpose of this study was to examine the relation between body mass index and the risk of thromboembolic stroke independently of other risk factors. METHODS: Since 1965, the Honolulu Heart Program has followed a cohort of men in a prospective study of cardiovascular disease. This article examines the relationship between the baseline measurement of body mass index and the risk of thromboembolic stroke in 1163 nonsmoking

Maternal quality and safety outcomes for Asians and Pacific Islanders in Hawai'i: an observational study from five years of statewide data.

BACKGROUND: Empirical evidence regarding maternal quality and safety outcomes across heterogeneous Asian and Pacific Islanders subgroups in the United States is limited, despite the importance of this topic to health disparities research and quality improvement efforts. METHODS: Detailed discharge data from all Hawai'i childbirth hospitalizations (n = 75,725) from 2008 to 2012 were considered. Validated measures of maternal quality and safety were compared in descriptive and multivariable models across seven racial/ethnic groups: Filipino, Native Hawaiian, other Pacific Islander (e.g., Samoan…

Comorbid substance use disorders with other Axis I and II mental disorders among treatment-seeking Asian Americans, Native Hawaiians/Pacific Islanders, and mixed-race people.

Little is known about behavioral healthcare needs of Asian Americans (AAs), Native Hawaiians/Pacific Islanders (NHs/PIs), and mixed-race people (MRs)-the fastest growing segments of the U.S. population. We examined substance use disorder (SUD) prevalences and comorbidities among AAs, NHs/PIs, and MRs (N = 4572) in a behavioral health electronic health record database. DSM-IV diagnoses among patients aged 1-90 years who accessed behavioral healthcare from 11 sites were systematically captured: SUD, anxiety, mood, personality, adjustment, childhood-onset, cognitive/dementia, dissociative, eating…

The prevalence of gestational diabetes among Micronesians in Honolulu.

OBJECTIVES: There is a high prevalence of type 2 diabetes among Micronesians leading to the assumption that they are at an increased risk for gestational diabetes. The objective of this study was to determine the prevalence of gestational diabetes among the Micronesian population in Honolulu, Hawai'i. Secondary objectives were to determine factors associated with gestational diabetes and compare maternal and fetal outcomes between the Micronesian gestational diabetic and non-diabetic populations. METHODS: A retrospective chart review was performed of all delivery records from January 1997 to

Finding the best fit: a grounded theory of contraceptive decision making in women.

PROBLEM: Half of the pregnancies in the United States that occur annually are unintended, and of these, half occur among women using contraceptives. METHODS: Interviews with 16 women, field notes, and memos were analyzed using constant comparative analysis and the methods of open, axial, and selective coding were used until saturation occurred. FINDINGS: The core category that describes the process of contraceptive decision making in women is "finding the best fit." Women choose a method or methods to prevent pregnancy based on their knowledge, experience, and evaluation of what would be the

High-density lipoproteins in the prevention of atherosclerotic heart disease. Part I. Epidemiological and family studies.

It has recently been proposed that the concentration of the high-density lipoprotein (HDL) fraction in plasma bears a negative relationship to the incidence of atherosclerotic heart disease. The biological and environmental factors affecting plasma HDL levels and the evidence pertaining to the proposed 'negative risk potential' of this lipoprotein are reviewed. HDL concentrations are low at birth, but rise rapidly in early infancy to adult or above-normal adult levels. This trend is influenced by biological factors such as sex and ethnicity and by a host of environmental variables. Despite met

The impact of Kaua'i Care Transition Intervention on hospital readmission rates.

OBJECTIVES: To evaluate the effects of Kaua'i Care Transition Intervention (KCTI), a patient-centered intervention program, on reducing hospital readmission rates among patients 60 years or older. STUDY DESIGN: A prospective quasi-experimental prepost design. METHODS: Hospital admissions data for the year 2010 (January 1 to December 31) served as the baseline data and were used to identify patients at risk of hospital readmission. KCTI was implemented over a 12-month period from April 1, 2012, to March 31, 2013, and 30-day, 60-day, and 1-year readmission rates were assessed for both the inte

Particle numbers of lipoprotein subclasses and arterial stiffness among middle-aged men from the ERA JUMP study.

We examined the association between serum lipoprotein subclasses and the three measures of arterial stiffness, that is, (i) carotid-femoral pulse wave velocity (cfPWV), which is a gold standard measure of central arterial stiffness, (ii) brachial-ankle PWV (baPWV), which is emerging as a combined measure of central and peripheral arterial stiffness and (iii) femoral-ankle PWV (faPWV), which is a measure of peripheral arterial stiffness. Among a population-based sample of 701 apparently healthy Caucasian, Japanese American and Korean men aged 40-49 years, concentrations of lipoprotein particles
