Dilated cardiomyopathy with cardiogenic shock in a child with Kearns-Sayre syndrome.

Kearns-Sayre syndrome (KSS) is a mitochondrial myopathy resulting from mitochondrial DNA deletion. This syndrome primarily involves the central nervous system, eyes, skeletal muscles and the heart. The most well-known cardiac complications involve the conduction system; however, there have been case reports describing cardiomyopathy. We describe a case of a child with KSS who presented with decompensated cardiac failure from dilated cardiomyopathy representing cardiomyocyte involvement of KSS. Our patient had a rapidly progressing course, despite maximal medical management, requiring emergent

Exercise electrocardiography in the evaluation of cardiac dysrhythmias in children.

Exercise electrocardiography is a useful means to evaluate dysrhythmias in children and young adults. Exercise may suppress a dysrhythmia present at rest and thus suggest a relatively benign problem. It may provoke a latent dysrhythmia which is absent at rest but suggested by a history of exercise intolerance or palpitation with exertion. Exercise may also modify a dysrhythmia. Maximum exercise testing has been used to evaluate each of these situations in children. The results of testing will be presented. Since no dysrhythmias were provoked in 170 healthy children, we think that any dysrhythm

Atypical Progeria Primarily Manifesting as Premature Cardiac Valvular Disease Segregates with LMNA-Gene Variants.

Mutations in the LMNA-gene can cause a variety of 'laminopathies'. These laminopathies are associated with a range of phenotypes, including disorders affecting the adipose tissue, peripheral nerves, the heart, such as dilated cardiomyopathy and conduction system abnormalities, and less commonly, progeroid disorders. This case series describes two families in which two novel LMNA-gene variants were identified, and who presented with an atypical progeroid phenotype with primarily premature aortic and mitral valve stenosis. Interestingly, these families exhibited no clear evidence of multisystem

Complete Heart Block in Systemic Sclerosis with Characterization on Cardiac MRI.

Cardiac involvement of infiltrative disease, such as systemic sclerosis, carries significant morbidity and mortality. All parts of the heart may be affected, although the conduction system is less commonly involved. We report a rare case of systemic sclerosis causing third-degree atrioventricular block and the first known reported case to use cardiac magnetic resonance imaging (CMR) as a diagnostic and prognostic instrument. It is not known whether using cardiac CMR in systemic sclerosis with cardiac involvement could lead to earlier intervention with escalation of medical therapy or earlier r

Step-by-Step Cardioneuroablation Approach in Two Patients with Functional Atrioventricular Block.

Parasympathetic overactivity may cause functional atrioventricular block episodes and necessitate pacemaker implantation in symptomatic cases and those refractory to conventional therapies. In these patients, if it can be clearly demonstrated that there is no structural damage in the conduction system, elimination of the vagal activity based on radiofrequency catheter ablation of main ganglionated plexi around the heart, which is called as cardioneuroablation, might be a rational approach. In this review article, we try to discuss patient selection and procedural steps suitable for cardioneuro

Is steroid therapy enough to reverse complete atrioventricular block after percutaneous perimembranous ventricular septal defect closure?

Due to the close proximity of the perimembranous ventricular septal defect to the conduction system, complete atrioventricular block may occur after transcatheter closure in around 1-5% of cases. Some authors reported on successful use of steroids and aspirin in reversing complete atrioventricular heart block early after transcatheter closure of perimembranous ventricular septal defect using the Amplatzer membranous ventricular septal defect occluder. In this paper, we report a case of reappearance of complete atrioventricular block requiring a definitive pacemaker implantation after an initia

Sudden cardiac death in a 5-year-old girl associated with parvovirus B19 infection.

We report on a 5-year-old girl who suddenly collapsed and died while dancing at a family party. Histological examination of the heart including the cardiac conduction system revealed lymphocytic infiltrations of the sinu-atrial node and perivascular infiltration in the atrio-ventricular region. Additionally, foci of mononuclear infiltrates were observed in the myocardium. Consequently, myocarditis was diagnosed as cause of death. The child also had lymphocytic conjunctivis, parotitis and tracheitis. Evaluation of infections by means of nested polymerase chain reaction revealed parvovirus B19 D

Left Bundle Branch Area Pacing as a Rescue Strategy After Failed Cardiac Resynchronization Therapy in a Patient With Persistent Left Superior Vena Cava, Absent Coronary Sinus Branches, After Bilateral Maze Procedure: A Case Report.

BACKGROUND Cardiac resynchronization therapy can fail in the presence of anatomical variations, such as persistent left superior vena cava, which can preclude coronary sinus cannulation and left ventricular lead placement. Conduction system pacing, particularly left bundle branch area pacing, has emerged as a physiological alternative for achieving ventricular resynchronization in such anatomically challenging situations and may provide an effective solution when standard cardiac resynchronization therapy (CRT) is not feasible. CASE REPORT We report the case of a 78-year-old man with chronic h

Permanent pacemaker implantation after On-X surgical aortic valve replacement: SWEDEHEART observational study.

OBJECTIVE: Bioprosthetic aortic valves with an extended subannular component, such as transcatheter valves, exert increased compression on the cardiac conduction system and increase the risk for permanent pacemaker implantation. It is unknown if the On-X mechanical prosthetic valve, which has an elongated subannular valve housing, increases the risk of permanent pacemaker implantation following aortic valve replacement. DESIGN: Observational nationwide cohort study. SETTING: Swedish population-based study. PARTICIPANTS: All patients aged 18-65 years who underwent primary mechanical aortic v

The electrocardiogram manifestations of atrial and ventricular septal defects before and after repair.

Atrial septal defects (ASDs) and ventricular septal defects (VSDs) account for the majority of congenital heart disease and may remain clinically relevant from infancy into adulthood. Echocardiography is the gold standard for diagnosis and detailed anatomical assessment of these defects; however, the electrocardiogram (ECG) may provide additional pathophysiological and prognostic insights. This review summarizes ECG findings in ASD and VSD before and after repair, with respect to the effects of defect type, timing of intervention, and postoperative evolution of electrical disturbances. A rare phenomenon involving ventricular separation: a case report.

BACKGROUND: Ventricular separation is a multipart, extensive disease of the heart that hinders the electrical conduction of the cardiac system ventricular muscle, causing a bidirectional conduction block. The occurrence of ventricular separation suggests that the myocardium is in a state of severe ischemia, and the prognosis is generally poor. Herein, we present arescue case in which the extremely rare phenomenon of ventricular separation developed and was documented in realtime. CASE PRESENTATION: An 82-year-old man with syncope presented to the emergency departmentin a critical condition wi

Conduction System Versus Biventricular Pacing in Pacing-Induced Cardiomyopathy: Systematic Review and Meta-Analysis.

BACKGROUND: Biventricular pacing (BVP) and conduction system pacing (CSP) can enhance pacing-induced cardiomyopathy (PICM) patients' clinical outcomes. Although BVP is a class I recommendation in guidelines, CSP may provide a superior physiological alternative to BVP. This meta-analysis compares the efficacy and safety of CSP and BVP in PICM patients. METHODS: Databases including PubMed, Cochrane Library, Web of Science, and Embase were searched from their establishment to June 2025. Data analysis was performed using Stata 17. RESULTS: Twenty observational studies involving 821 patients with

Atrioventricular nodal reentrant tachycardia.

Atrioventricular nodal reentrant tachycardia represents the most frequent, regular tachyarrhythmia in humans, but its exact circuit has so far remained elusive. This review presents new evidence that allows the proposal of a model for the arrhythmia circuit as well as a reclassification of the types of atrioventricular nodal reentrant tachycardia and discusses the implications for effective and safe catheter ablation of the arrhythmia.

Double Ventricular Response with Aberrant Conduction Leading to Ventricular Dysfunction.

Double ventricular response (DVR), where a single P wave results in two QRS complexes, is a rare presentation of dual AV node physiology. It has been associated with ventricular dysfunction in the setting of incessant tachycardia. We present the case of an otherwise healthy adolescent who had frequent DVR without tachycardia leading to left ventricular dysfunction. Slow pathway modification led to a significant reduction in ectopy and normalization of ventricular function. This highlights that DVR without tachycardia might lead to ventricular dysfunction in pediatric patients. Slow pathway mod

Recurrence quantification analysis for fine-scale characterisation of arrhythmic patterns in cardiac tissue.

This paper uses recurrence quantification analysis (RQA) combined with entropy measures and organization indices to characterize arrhythmic patterns and dynamics in computer simulations of cardiac tissue. We performed different simulations of cardiac tissues of sizes comparable to the human heart atrium. In these simulations, we observed four classic arrhythmic patterns: a spiral wave anchored to a highly fibrotic region resulting in sustained re-entry, a meandering spiral wave, fibrillation, and a spiral wave anchored to a scar region that breaks up into wavelets away from the main rotor. Optimization of Arrhythmia-based ECG-lead Selection for Computer-interpreted Heart Rhythm Classification.

The 12-lead ECG only has 8 independent ECG leads, which leads to diagnostic redundancy when using all 12 leads for heart arrhythmias classification. We have previously developed a deep learning (DL)-based computer-interpreted ECG (CIE) approach to identify an optimal 4-lead ECG subset for classifying heart arrhythmias. However, the clinical diagnostic criteria of cardiac arrhythmia types are often lead-specific, so this study is going to explore the selection of arrhythmia-based ECG-lead subsets rather than one general optimal ECG-lead subset, which could improve the classification performance
