Impingement and childhood hip disease.

Hip impingement is now recognized as a relatively common cause for hip pain in young adults. The early evolution of impingement begins in childhood in several common hip disorders (slipped capital femoral epiphysis, Perthes, early avascular necrosis due to other causes). Recognition and treatment of childhood impingement may prevent the evolution to early hip arthritis.

[Acetabular retroversion in hip dysplasia].

OBJECTIVE: To study the occurrence and radiograph features of the acetabular retroversion in hip dysplasia, and to evaluate the effects on the retroversion by the traditional maneuvers in the periacetabular osteotomy. METHODS: Bernese periacetabular osteotomy was performed to 45 continuous dysplastic hips (43 patients) from December 2001 to November 2003. There were 37 female (39 hips) and 6 male (6 hips), average age was 28.9 (range, 15-45). The diagnosis of acetabular retroversion was based on the "cross-over" and "posterior wall" signs on the standard radiographs of the patients pre- or po…

Survival following total hip replacement.

BACKGROUND: Several studies have shown that patients who undergo total hip replacement live longer than control subjects, but the reason for this apparent protective effect is not clear. The purpose of our investigation was to assess whether the association had the characteristics of a causal relationship or whether it might appear to be due to comorbidity or other factors. METHODS: We compared survival over a six-year period for 28,469 Medicare enrollees who had elective primary total hip replacement in 1996 and a control group from the general Medicare population, matched 5:1 on year of bir…

[Emergencies in total hip replacement].

Over the last three decades total hip replacement became one of the most common surgical procedures in orthopaedic surgery. According to the number of large joint endoprosthesis, hip replacement is on the first place. Lately, the number of revisional and special tumoral endoprosthesis is increasing, with more severe complications. Dislocation is a leading early complication of total hip arthroplasty. Dislocations after primary total hip arthroplasties occur at an overall incidence of 1-3%, and at 15-20% in revision and tumoral procedures. Closed reduction and eventually immobilization is the m…

[Hip sonography for newborn screening].

In the past, congenital dysplasia and dislocation of the hip joint have, despite treatment, led frequently to late sequals (deformations and incongruous articular surfaces), requiring repeated corrective interventions. Nevertheless, life-long handicaps could not always be prevented. It is known for a long time that sequels, such as articular deformations and desorders of growth of the femoral head, can be prevented the better, the earlier the treatment is initiated. However, a reliable method for early diagnosis has been lacking to date. Even systematic clinical examinations of all newborn bab…

[Candida infection in hip alloarthroplasty].

A 46-year-old polytoxicomanic patient underwent a total hip replacement for necrosis of the femoral head. The patient suffered additionally from chronic pancreatitis with insulin-dependent diabetes and polyneuropathy. Three weeks later he developed a deep wound infection followed by surgical revision. The infection persisted despite further revision operations and systemically and locally applied antibiotics. After removal of the prosthesis, microbiology revealed Staphylococcus aureus, enterococcus, and Candida parapsilosis. Five additional revision operations, application of suction-irrigation …

Imaging of pediatric hip disorders.

Developmental and acquired abnormalities of the hips are common in childhood. Radiographs, MR imaging, CT, and nuclear medicine play an important role in the diagnosis and management of these disorders. Knowledge of the surgical and clinical treatments of these disorders is important to interpret accurately the radiology studies and impact treatment of the children.

Psychiatric illness in hip fracture.

OBJECTIVE: to review the literature on the prevalence and effect on outcome of psychiatric illness in older people with hip fracture. METHODS: searching of medical databases and bibliographies to identify relevant studies. Application of predetermined quality criteria for prevalence and outcome studies. RESULTS: 19 studies met criteria for a prevalence study. Rates of psychiatric illness varied, with depression in 9-47%, delirium in 43-61% and unspecified cognitive impairment in 31-88%. Four studies met criteria for an outcome study. Psychiatric illness resulted in increased mortality and de…

Vitamin D and Hip Fracture.

Hip fractures are recognized as a major public health problem worldwide. Demographic changes will lead to enormous increases in the number of hip fractures and projections indicate that the number of hip fractures occurring worldwide each year will rise from 1.26 million in 1990 to 4.5 million by 2050. However, preventive strategies are available. Supplementation with Ca2+ and vitamin D restores bone quality through suppression of secondary hyperparathyroidism and decreases the risk of falling through improvement of neuromuscular coordination and body sway. This leads to a reduction of hip fra…

Traumatic hip dislocation in childhood.

Eighteen cases of traumatic dislocations of the hip in children under 15 years of age presenting between 1985 and 1995 were reviewed. Fifteen of the dislocations were posterior. There were two groups: a younger group dislocating with minimal trauma and an older group whose injuries involved significantly more trauma. All patients were treated by closed reduction, but two required open reduction because of intraarticular fragments preventing a full reduction. On long-term follow-up of the 16 available patients (average length of follow-up, 5 years 10 months; range, 17-132), there were no cases …

Hip Dislocation: Current Treatment Regimens.

Dislocation of the hip occurs only with high-energy trauma, and concomitant injuries are common. Early diagnosis and institution of treatment are necessary to obtain the best possible results. Treatment protocols include emergent reduction of the femoral head to reestablish perfusion, postreduction radiography and computed tomography to look for associated fractures and to judge the concentricity of the reduction, stability testing, and early mobilization. Open reduction may be required if a concentric reduction cannot be obtained in a closed manner. Despite appropriate management, posttraumat…

[Hip dislocation without bone injuries].

Dislocation of the hip is rare and so we regard every hip dislocation as an emergency until it has been reduced. From 1977 to 1996 a total of 82 patients with hip dislocation were treated at the Traumatic Surgery Clinic, University of Vienna, Austria. The average age at injury was 29 years. Some 77 patients had unilateral dislocation of the hip, but only in 5 cases was bilateral dislocation found. Iliac dislocation was most common (54 patients). Our treatment consisted in reduction within 6 h of the injury under general anaesthesia (Böhler) and 14 days bedrest. Then the patients were mobilized…

Recurrent irritable hip in childhood.

Between 1989 and 1992 we admitted 426 children with an irritable hip, 363 (85.2%) once and 63 (14.8%) on 143 occasions. We assessed the records retrospectively to determine whether the groups differed and in particular whether recurrence was followed by pathological sequelae. We identified no feature which distinguished between them at either presentation. The use of bone isotope scans was greatly increased in recurrent cases, without clinical benefit. No relationship between recurrence and subsequent abnormality was identified, with 22 (42%) of the recurrences taking place in the opposite hip…

Infant hip sonography: current concepts.

Sonography of the infant hip has gained wide acceptance in the decade since its introduction. The two principle techniques of Graf and Harcke have been combined with the proposal of a Dynamic Standard Minimum Examination. Whereas sonography is used increasingly to manage developmental dislocation and/or displasia of the hip, there is no agreement on the use of sonography for universal newborn screening. This article describes in detail the Dynamic Standard Minimum Sonographic Examination of the infant hip. In addition, this article reviews the classification and management of infant hip disord…
