Foot drop following closed reduction of a total hip replacement.

Sciatic nerve palsy has been well documented and reported, following primary and revision hip replacement. It can also occur following dislocation of hip replacement. The incidence of sciatic nerve palsy in dislocated hip replacement is <0.1%. However, new onset sciatic nerve palsy following a closed reduction of dislocated hip replacement is even rarer. We describe a case of sciatic nerve palsy from traction injury following closed reduction of a total hip replacement.

Imaging of Sports-related Injuries of the Lower Extremity in Pediatric Patients.

With increasing participation and intensity of training in youth sports in the United States, the incidence of sports-related injuries is increasing, and the types of injuries are shifting. In this article, the authors review sports injuries of the lower extremity, including both acute and overuse injuries, that are common in or specific to the pediatric population. Common traumatic injuries that occur in individuals of all ages (eg, tears of the acetabular labrum and anterior cruciate ligament) are not addressed, although these occur routinely in pediatric sports. However, some injuries that

A Review on Management of Insufficiency Fractures of the Pelvis and Acetabulum.

"Insufficiency fractures of the pelvis and acetabulum are occurring at increasing rates. Osteoporosis is the most prevalent risk fracture. Diagnosis begins with plain radiographs followed by advanced imaging with computed tomography and/or MRI. Pelvic ring fragility injuries are classified by the Fragility fractures of the pelvis system. Elderly acetabular fractures may be classified by the Letournel system. Management of these injuries is primarily nonoperative with early immobilization when allowed by fracture characteristics. When warranted, percutaneous fixation and open reduction internal

Risk factors for injury after a fall.

A major portion of the morbidity, mortality, and social cost due to fall injuries is associated with age-related fractures, especially hip fractures. The risk of such injuries rises as the intensity of trauma increases and as bone resistance to fracture decreases. Both osteoporosis and trauma appear to be necessary but not sufficient causes of most age-related fractures. Future work may identify additional factors that help determine who, among the elderly who fall, is most likely to suffer a fracture. In the interim, however, prophylactic efforts must focus on avoiding falls and delaying the

Bilateral Hip Abductor Tendon Repair and Gluteus Maximus Transfers: A Case Report.

CASE: A 76-year-old woman presented with bilateral hip pain and hip abductor weakness following years of trochanteric bursa steroid injections. MRI showed injury of the hip abductor tendons bilaterally with muscle atrophy, and she underwent staged bilateral hip abductor tendon repair with gluteus maximus transfer, resulting in improvement in pain and function. CONCLUSION: This is among the first reported cases of bilateral hip abductor tendon repairs with gluteus maximus transfers in a patient with native hips. Patients presenting with hip abductor insufficiency and muscle atrophy may benefit

[Increased risk of knee and hip arthrosis among elite athletes. Lower level exercise and sports seem to be "harmless"].

Both joint injury and joint stress contribute to the development of arthrosis. The article consists in a review of literature in this field which sheds light on the risk of developing hip or knee arthrosis as a result of sports participation. Soccer is a sport associated with manifest joint overload and risk of injury to the knee. Owing to the increased incidence of cruciate ligament and meniscus injury among soccer players, there is a markedly increased risk of arthrosis (or post-traumatic arthrosis), radiological signs of arthrosis being present in 50 per cent of cases after 15 years. Arthroscopic-Assisted Removal of Incarcerated Fragment following Manual Reduction of Posterior Hip Dislocation in Acute Polytrauma Patient: A Case Report.

INTRODUCTION: Posterior hip dislocations combined with acetabular posterior wall fractures are usually managed with open reduction and internal fixation. However, select cases may benefit from closed reduction followed by minimally invasive arthroscopic evaluation. CASE REPORT: We report a case of a 27-year-old male who sustained left side complex posterior hip dislocation with ipsilateral compound Grade 2 fracture of the shaft of the tibia with facial and chest injury following a high-speed motorcycle accident. The injury was successfully managed with closed manual reduction of the hip joint

Traumatic Hip Dislocation with Proximal Femoral Epiphyseal Fracture in 12-Year-Old Boy: A Case-Based Review.

INTRODUCTION: Pediatric hip fractures comprised <1% of all pediatric fractures. These fractures are usually associated with high-energy trauma, such as motor vehicle accidents or fall from height. The Delbet classification of pediatric hip fractures is frequently used to describe these injuries and to relate the fracture type to the development of avascular necrosis. Treatment should be given as an emergency basis and monitored according to the type of fracture and the patient's age. Historically, casting was used for such fractures; however, due to the high complication rate, the treatment mo

Combined acetabular and femoral neck fractures with intrapelvic femoral head dislocation: Successful staged management of a rare injury in two cases.

Combined central acetabular and femoral neck fractures with intrapelvic femoral head dislocation is an infrequent situation that provides a problematic condition for surgeons attempting to reconstruct the hip joint. Herein, we report two cases involving central acetabular fracture-dislocation combined with intrapelvic dislocation of a fractured femoral neck. Each case involved associated injuries that made primary total hip arthroplasty (THA) impossible and necessitated using the fewest skin incisions possible. As a result, we first attempted a posterior acetabular fixation of both the anterio

Total hip arthroplasty for treatment of fused hip with 90 degrees flexion deformity.

Conversion of a fused hip to total hip arthroplasty is a challenging procedure. The results depend on the degree of the change that occurs in the muscles and soft tissues at the hip from long immobilization, and considerable postoperative rehabilitation is necessary. Complications such as nerve injury, dislocation, infection and heterotopic ossification are common. Most of the authors recommended trochanteric osteotomy for better exposure. We are reporting a case of conversion total hip arthroplasty performed for a hip fused in 90 degrees of flexion. Surgical technique is described in detail a

Conversion total hip arthroplasty following extracapsular hip fracture fixation with a cephalomedullary device: a comprehensive review.

With the annual incidence of hip fractures and hip fracture fixation rising, the need for conversion total hip arthroplasty has also risen. About half of the 280,000 hip fractures that occur annually in the United States are extracapsular. Commonly extracapsular hip fractures are treated with either cephalomedullary nails (CMNs) or sliding hip screws (SHS). More recently, there has been a shift toward increased CMN use due to increased training with this fixation method as well as perioperative and biomechanical benefits. Given this shift, orthopedic surgeons need to understand the factors tha

Characteristics of Femoral Torsion Angles and Relationships Between Femoral Torsion Angles and Hip Rotational Motion Variables in Adult Baseball Players.

BACKGROUND: Abnormalities in hip rotational motion (HRM) are risk factors for throwing injuries. To evaluate hip rotational motion, it is necessary to assess the torsion angle. However, no studies have investigated the femoral torsion angle (FTA) in baseball players. PURPOSE: To investigate differences in hip FTA of adult baseball players through use of ultrasonography to evaluate the relationship between the FTA and HRM. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: A total of 149 elite male baseball players (mean age, 20.0 ± 1.8 years; 64 pitchers, 85 position player

Hot topics in biomechanically directed fracture fixation.

The evolution of locking plates and modern nail constructs provides the orthopaedic trauma surgeon with a myriad of options with regard to implant selection for common fractures. There is a significant amount of biomechanical literature comparing modern constructs with those conventionally used. A basic understanding of this literature is required to make informed decisions with regard to implant selection in the management of these injuries. This article reviews the most recent biomechanical literature regarding implant selection and application for a variety of commonly treated injuries, inc

Central Fracture Dislocation of Hip With Ipsilateral Femoral Neck Fracture: A Case Report.

UNLABELLED: Central fracture dislocation of the hip with associated ipsilateral femoral neck fracture is exceedingly rare and presents a formidable management challenge. CASE: A 31-year-old woman sustained this fracture with medial displacement of the femoral head into the pelvic cavity after a high-energy road traffic accident. Within 6 hours, surgical reduction of the femoral head and internal fixation of both femoral neck and acetabulum were performed using a dynamic hip screw and a plate, respectively. At 1 year, the patient demonstrated good functional recovery without avascular necrosis

CASE: We report a very rare case of simultaneous ipsilateral floating hip and floating knee (SIFHFK) injury with the neck of the femur and patella fracture and hip dislocation associated with sciatic nerve injury in a 45-year-old man after a road traffic accident. We adopted a staged approach for surgical fixation of the fractures after stabilization. At the latest 2 years follow-up, the patient was asymptomatic with satisfactory clinical outcome. CONCLUSION: SIFHFK injuries present a challenging and emergent clinical scenario and require extensive clinical experience, careful planning, and m

Osteopathic diagnosis of an acetabular injury.

Physical findings demarking pathologic somatovisceral reflex activity and fascial strain patterns may lead the osteopathic physician to diagnoses that are masked within the initial presentation of a patient. The authors present a case report that demonstrates the use of osteopathic principles in the diagnosis of a chronic acetabular fracture and acetabular labral tear in a 19-year-old man. The injuries resulted from a posterior hip dislocation sustained during a basketball game more than 1 year before presentation. Osteopathic manipulative treatment and diagnostic techniques also relieved the

Abductor Tendon Tears in the Native and Prosthetic Hip.

Gluteal tendon tears are an increasingly recognized source of laterally based hip pain. These tendon tears are typically degenerative in nature and require careful history, physical examination, and imaging interpretation for accurate diagnosis. After a trial of nonoperative management, open or endoscopic repair can predictably improve pain, though expected functional improvement is correlated to the degree of preoperative muscle atrophy. The authors advocate for an open double-row suture technique to reliably visualize the full extent of the tear, restore the anatomic footprint of the avulsed

Hip arthroscopy: evolution, current practice and future developments.

Arthroscopic examination and treatment is an ever-increasing part of modern orthopaedic practice in this age of minimally invasive surgery. Arthroscopic procedures have been widespread in surgery of the knee and the shoulder for many years; however, the hip until relatively recently, has been largely neglected. Even now hip arthroscopy is not widely available; this may be due to the complexity of the procedure, the requirement of specialist equipment and a reportedly long learning curve. On the other hand, it has gone through a period of rapid growth over the last decade and is being performed
