Suspected Cauda Equina Syndrome: Can the Presence of an Orthopaedic Doctor in the Emergency Department Reduce Waiting Times for MRI Scans and Inpatient Stays?

Introduction Cauda equina syndrome (CES) is a very rare but devastating surgical emergency that can lead to permanent bowel, bladder or sexual dysfunction and lower limb paralysis. Although it is a clinical syndrome, an MRI scan is a critical diagnostic investigation for these patients and should be done as soon as possible. Our hospital is a district general hospital with no spinal services on site. There is a protocol in place for the management of these patients locally with MRI scanning during daytime hours. However, if the patient presents after 8 pm, they are discussed with the tertiary

Two False Negative Test Results in a Symptomatic Patient with a Confirmed Case of Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) and Suspected Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis (SJS/TEN).

The recent outbreak of COVID-19 has put significant strain on the current health system and has exposed dangers previously overlooked. The pathogen known as severe acute respiratory syndrome coronavirus 2 (SARS-COV-2), is notable for attacking the pulmonary system causing acute respiratory distress, but it can also severely affect other systems in at-risk individuals including cardiovascular compromise, gastrointestinal distress, acute kidney injury, coagulopathies, cutaneous manifestations, and ultimately death from multi-organ failure. Unfortunately, the reliability of negative test results

Risk Stratification for Complications in Patients Hospitalized With Community-Acquired Pneumonia.

AIM: Community-acquired pneumonia (CAP) remains a major cause of morbidity in children. Although severity assessment tools are available, they are not specifically designed to predict the development of a broad spectrum of pulmonary and systemic complications. This study aimed to identify admission factors associated with complicated pneumonia (CP) in hospitalized children with CAP and to develop a practical risk scoring model for early risk stratification. METHODS: Patients hospitalized with CAP were classified into two categories: uncomplicated CAP and complicated pneumonia. Independent pre

Domestically Acquired Legionnaires' Disease: Two Case Reports and a Review of the Pertinent Literature.

BACKGROUND: Legionella species may colonize in home water systems and cause Legionnaires' disease (LD). We herein report two cases of sporadic LD associated with the solar energy-heated hot water systems of the patients' houses. CASE REPORT: A 60-year-old woman with chronic bronchitis and diabetes mellitus presented with a high fever, abdominal pain, and diarrhea. Physical examination revealed rales, and her chest radiograph showed a homogeneous density in the left lung. The Legionella urinary antigen test was positive, and an indirect fluorescent antibody test revealed a serum antibody titer

An inexpensive, interdisciplinary, methodology to conduct an impact study of homeless persons on hospital based services.

Homelessness is a primary concern for community health. Scientific literature on homelessness is wide ranging and diverse. One opportunity to add to existing literature is the development and testing of affordable, easily implemented methods for measuring the impact of homeless on the healthcare system. Such methodological approaches rely on the strengths in a multidisciplinary approach, including providers, both healthcare and homeless services and applied clinical researchers. This paper is a proof of concept for a methodology which is easily adaptable nationwide, given the mandated implemen

Routine physics work efficacy and efficiency improvement through a shared-duty physicist-of-the-day model in a multi-hospital radiation oncology network.

BACKGROUND: Large multi-hospital radiation oncology networks often rely on site-exclusive physicist coverage models that can lead to imbalanced workload distribution, inconsistent task execution, communication fragmentation, and vulnerability to delayed or missed quality checks. Increasing clinical complexity and distributed staffing models necessitate operational strategies that improve efficiency, standardization, and safety across geographically separated sites. PURPOSE: To describe the implementation of a shared-duty physicist-of-the-day (POD) model supported by a real-time operational da

Integrating Neurology, Palliative Care and Emergency Services in ALS: A Community-Integrated Neuropalliative Pathway in Modena, Italy.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease that causes severe motor, respiratory and communication impairment and imposes a high psychosocial burden on patients and families. Recent evidence shows that integrated neuropalliative care-early collaboration between neurology and palliative services with community support-improves quality of life and reduces avoidable hospitalisations. Yet there are few descriptions of how such integration is operationalised. OBJECTIVE: This study examines a Community-Integrated Neuropalliative Pathway (CINP) impleme

Rural-Urban Parity and Socioeconomic Gaps in Traumatic Brain Injury Outcomes.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of death and disability in the United States. Rural residence and low community income are independently associated with poorer health outcomes, mediated by variations in injury mechanism and healthcare access. We evaluated the impact of geographic location on injury patterns and acute-care metrics in adults with TBI treated at Maryland's statewide Level I trauma center. METHODS: Adults (≥18 years of age) with radiographically confirmed TBI admitted between 2017 and 2021 were identified retrospectively from the institutional trauma r

Design and Implementation of a Perioperative Surgical Home at a Veterans Affairs Hospital.

The innovative Perioperative Surgical Home model aims to optimize the outcomes of surgical patients by leveraging the expertise and leadership of physician anesthesiologists, but there is a paucity of practical examples to follow. Veterans Affairs health care, the largest integrated system in the United States, may be the ideal environment in which to explore this model. We present our experience implementing Perioperative Surgical Home at one tertiary care university-affiliated Veterans Affairs hospital. This process involved initiating consistent postoperative patient follow-up beyond the po

[Psychiatric disorders in hospitalized patients with hematologic neoplasms].

We report the results of a year of regular psychiatric support in the Hematology Department (UTI-DH) at Santa Maria Hospital. During a six-month period, the total group of hospital inpatients suffering from leukemia, Hodgkin's disease or non-Hodgkin's lymphoma, were assessed, using a semi-structured interview. A prevalence of 30% of adjustment disorders (depression and/or anxiety) and 2% of organic mental syndromes was found employing the DSM-III-R diagnostic system. In the second six-month period only patients referred by their doctor and/or nurse were observed. The two rates are discussed. A

Design and Application of Comprehensive Management System For Patients with Chronic Aortic Valve Disease-A Perspective From West China Hospital.

BACKGROUND: Chronic aortic valve disease (AVD) is a prevalent age-related health issue. Current guidelines recommend transthoracic echocardiography as the method of detection. Early screening is crucial to decrease mortality caused by aortic valve disease. However, a lack of consistent and effective guidelines for screening populations with asymptomatic AVD persists. Based on the comprehensive management system (CMS) for AVD developed by West China Hospital of Sichuan University, the objective of this study is to enhance the survival rate and overall quality of life for patients suffering from
