Fractured and delayed: A qualitative analysis of disruptions in care for gynecologic malignancies during incarceration.

OBJECTIVE: Women are experiencing growing rates of incarceration at twice the pace of that for men. Additionally, one-third will be older than 55 years of age by the end of the decade. Women who are incarcerated experience a higher prevalence of gynecologic malignancies and present with higher stage disease, which may be contributing to the greater mortality from cancer than the age-adjusted US population. Limited access to guideline-recommended screening and prevention and resource limitations across correctional facilities may result in gynecologic cancer disparities. Reasons for delayed gyn…

Challenging Health Inequities in Incarceration: a Call for Equitable Care for Kidney Disease and Hypertension.

PURPOSE OF REVIEW: To review the current literature on care of hypertension and chronic kidney disease for people who are currently and formerly incarcerated, and to make recommendations for improving outcomes. RECENT FINDINGS: There is a growing body of literature describing care for kidney disease and hypertension for incarcerated and formerly incarcerated individuals that documents the provision of care itself, notably that many jails contract with private companies; the system is not designed to provide sustained, chronic disease care; and the transition from incarceration to community is…

Transgender Incarceration and Law Enforcement as a Source of Harm: Upstream and Primordial Prevention Perspectives.

In the United States (US), transgender individuals are more likely to experience violence and sexual assault in jails and prisons compared with cisgender peers. Harms of incarceration on transgender individuals include limited access to medical care and hormone therapy, as well as being housed in facilities based on biological sex instead of gender identity. However, there has been insufficient research on addressing factors that lead to transgender individuals being incarcerated in the first place. In this article, we argue the need to focus on law enforcement interactions with transgender in…

Trauma and Incarceration: A Latent Class Analysis of Lifetime Trauma Exposures for Individuals in Prison.

This paper aims to contribute to the knowledge base about the lives of individuals who experience incarceration in the U.S. in order to advance post-release intervention services. Research has shown that among the millions of Americans who cycle through prisons and jails each year, the majority are poor, in poor health, living in contexts of chronic violence, often with mental illness, and more than half are people of color. Of particular concern for this population are high rates of trauma exposure and PTSD, though the research in this area is underdeveloped, particularly for men. Using surve…

Anterior uterine incarceration complicated by placenta previa and placenta accreta spectrum disorder: a case report.

BACKGROUND: Uterine incarceration is a rare obstetric complication that is always associated with retroversion and prone to misdiagnosis. Pelvic examination and imaging methods including ultrasound and magnetic resonance imaging (MRI) are used as the primary diagnostic tool. We present an asymptomatic anterior uterine incarceration complicated by placenta previa and placenta accreta spectrum (PAS) disorder, which could be diagnosed during the pregnancy periods, but was first diagnosed during the cesarean section (CS) and got the surgeons into trouble. CASE DESCRIPTION: A 28-year-old woman, gr…

First case report of uterine perforation and bowel incarceration following a clandestine abortion in Morocco.

Uterine perforation is a rare complication of abortion. It becomes much rarer when associated with a small bowel incarceration at the uterine breach. Its diagnosis can be suspected clinically, but radiology remains more sensitive for diagnosis. Surgery is the cornerstone in treating this entity as it provides both diagnostic and therapeutic management. A multidisciplinary therapeutic approach should be immediately performed to ensure a good prognosis. In this report, we describe a case of small intestine incarceration in the breach of a uterine perforation that occurred in the weeks following …

Coping with incarceration: examining the longitudinal relationship between individual coping styles and mental health outcomes.

BACKGROUND: Experiencing incarceration leads to increased rates of stress that result in a variety of negative physical, mental, and emotional outcomes. However, little research focuses on how individuals vary in their coping responses to stressful life events, like imprisonment. AIMS: This study extends prior research by examining whether changes in coping styles throughout the first year of incarceration influence mental health symptomology at 6- and 12-months post placement. METHODS: Using longitudinal data collected via semi-structured interviews with incarcerated men, this study measure…

Mortality after release from incarceration in New Zealand by gender: A national record linkage study.

BACKGROUND: People who enter and leave places of incarceration experience considerable health inequities and are at increased risk of premature death compared to the general population. Causes of premature death in this population vary markedly between countries and so country-specific information is needed. Additionally, there is a lack of large population-based studies which can disaggregate mortality risk based on person and incarceration factors. This study is the first examination of mortality in the period following release from incarceration in New Zealand. METHODS: We linked deidentif…

Incarceration exposure during pregnancy and maternal disability: findings from the Pregnancy Risk Assessment Monitoring System.

BACKGROUND: Extant research reveals that currently and formerly incarcerated individuals exhibit higher rates of disability. Moreover, recent research highlights that women exposed to incarceration during pregnancy -either personally or vicariously through a partner- face poorer health. However, prior research has not detailed the connection between incarceration exposure and risk for maternal disability. METHODS: The aim of this study is to evaluate the association between a women's exposure to incarceration during pregnancy and disability including difficulty with: communication, hearing, r…

Internal Hernia With Incarceration of the Cecum: A Case Report After Simultaneous Kidney-Pancreas Transplant.

Simultaneous pancreas and kidney (SPK) transplant is the most effective form of managing difficult-to-treat diabetes; however, this does not come without risk. The surgical complications after SPK transplant can be seen in up to 30% of recipients but do not always correlate with a decrease in patient or graft survival. One of the known complications is an internal hernia. Herein, we describe the first case of cecal incarceration in an internal hernia through the defect created by the transplanted ureter and the retroperitoneum in a SPK transplant recipient. Our management entailed the reductio…

Unusual Richter's Hernia: Impacted foreign body leading to incarceration and perforation - A rare clinical entity.

INTRODUCTION & IMPORTANCE: Richter's hernia is a clinically deceiving entity as is particularly associated with high morbidity and mortality which can be can be abated by timely diagnosis and surgery. Direct inguinal hernias having a wide neck have lesser chances of incarceration and strangulation when compared to indirect inguinal hernias. The knowledge about this type of hernia is especially important in the context of laparoscopy as majority of the laparoscopic port site hernias are characteristically Richter's type in configuration. The objective of this report is to highlight the unusual …

Linking Women Experiencing Incarceration to Community-Based HIV Pre-Exposure Prophylaxis Care: A Qualitative Study.

Women experiencing incarceration (WEI) in the United States are disproportionately impacted by HIV, yet HIV pre-exposure prophylaxis (PrEP) is underutilized by women in the United States. In order to inform an intervention to promote PrEP initiation during incarceration and facilitate linkage to PrEP care following release from incarceration, we conducted individual, semistructured qualitative interviews with WEI (N = 21) and key stakeholders (N = 14). While WEI had little or no previous knowledge about PrEP, they viewed it as something that would benefit women involved in the criminal justice…

Voice, Counterspaces, and Ethical Care in Research With Black Women With Trauma and Incarceration Histories.

Formerly incarcerated women face diverse challenges to re-entry, which include recovering from health illnesses and trauma to navigating various systems of stigma and surveillance. It is these multilevel challenges to reintegration that also make formerly incarcerated women vulnerable participants in research. As such, this qualitative study explores how 28 formerly incarcerated Black women experience the research interview process. Findings revealed that women participated in research because these contexts were viewed as spaces for "truth telling" and increasing awareness that can effect cha

Omental Incarceration over Twenty Years Presenting as a Hyperechoic Endometrial Mass in a Postmenopausal Woman.

Uterine perforation related with dilatation and curettage (D&C) is an uncommon event. Combined complications such as hemorrhage, adjacent organ injury, and omental incarceration may require an emergent surgical treatment. These are usually evident immediately or several days after the D&C, and a delayed presentation of uterine perforation are extremely rare. Herein, we report a rare case of omental incarceration presenting as a hyperechoic endometrial mass in a postmenopausal woman, diagnosed twenty-three years after the D&C. According to this case, when we encounter a hyperechoic endometrial …
