Risk Factors Associated With Failure to Reach Minimal Clinically Important Difference in Patient-reported Outcomes Following Minimally Invasive Transforaminal Lumbar Interbody Fusion for Spondylolisthesis.

STUDY DESIGN: Retrospective cohort. OBJECTIVE: To determine risk factors associated with failure to reach the minimal clinically important difference (MCID) in patient-reported outcomes (PROs) for patients undergoing minimally invasive transforaminal lumbar interbody fusion (MIS TLIF) for spondylolisthesis. SUMMARY OF BACKGROUND DATA: The MCID of PROs are often utilized to determine the benefit of spinal procedures. However, negative predictive factors for reaching MCID in patients surgically treated for lumbar spondylolisthesis have been difficult to elucidate. MATERIALS AND METHODS: A pro

Predictors of cancellation and rescheduling of elective shoulder arthroscopy: Experiences at an urban academic medical center.

BACKGROUND: Cancellation of elective shoulder arthroscopy negatively affects the surgical scheduling process and results in significant financial loss for a health care institution. Despite this, the patient-related and surgical-related risk factors of cancellation remain undefined. METHODS: A retrospective review at a single inner-city urban tertiary-care medical center was undertaken, including consecutive non-instability shoulder arthroscopy procedures from January 2016 through March 2024. Patient demographics and surgical data, including age at booking, marital status, diagnosed depressio

Racial disparities in patients with proliferative diabetic retinopathy treated with pars plana vitrectomy in an underserved population.

BACKGROUND: Proliferative diabetic retinopathy (PDR) is a serious microvascular complication of diabetes that can lead to irreversible vision loss. Prior research studies have documented how racial minorities with PDR experience less routine screening, less frequent treatment, and more complications. Although pars plana vitrectomy (PPV) is a widely used treatment modality for PDR, minimal research is currently available addressing how race impacts surgical presentation and outcomes. METHODS: This study is a retrospective case series involving a sample of patients who underwent PPV for PDR at

Trauma Center Outcomes After Transition From Level 2 to Level 1: A National Trauma Data Bank Analysis.

BACKGROUND: Previous US-based studies have shown that a trauma center designation of level 1 is associated with improved patient outcomes. However, most studies are cross-sectional, focus on volume-related issues and are direct comparisons between levels. This study investigates the change in patient characteristics when individual trauma centers transition from level 2 to level 1 and whether the patients have similar outcomes during the initial period of the transition. STUDY DESIGN: We performed a retrospective cohort study that analyzed hospital and patient records included in the National

Is Patient Satisfaction Associated with Objective Measures of Geriatric Hip Fracture Care?

BACKGROUND: Patient satisfaction is publicly reported and used as a subjective measure of quality of care in alternative payment reimbursement models. In this study, we evaluated the association between patient satisfaction scores and objective measures of geriatric hip fracture care. Therefore, according to patient satisfaction score, we investigated the differences for geriatric hip fracture admissions in regard to 1. surgical delay during the index admission and 2. mortality within 1 year. METHODS: We identified 65,974 patients between the ages of 60 and 99 years with a primary diagnosis o

Disparities in Surgical Care of Patients with Colorectal Cancer Liver Metastases.

BACKGROUND: Up to 25% of patients with colorectal cancer present with liver metastases (CRLM), and 50% develop metastases over time. Surgical and ablative management of CRLM can be curative, but certain demographic and socioeconomic factors disproportionately hinder vulnerable patient populations from receiving advanced local therapies. METHODS: We queried the 2011-2021 National Cancer Database for cases of CRLM. We explored patient and facility characteristics associated with receipt of local intervention versus no intervention for metastatic liver lesions. RESULTS: Of 72,273 cases, 18.0% u

Perils and progress in epilepsy surgery utilization: Twenty-five years later.

More than 25 years have passed since the first randomized controlled trial (RCT) established that surgery is superior to continued anti-seizure medication (ASM) for drug-resistant temporal lobe epilepsy, and nearly as long since a joint practice parameter urged that appropriate surgical candidates be referred to a specialized center for evaluation-guidance that subsequent consensus has since broadened to all patients with drug-resistant epilepsy. Despite Class I evidence and durable guidelines, epilepsy surgery remains one of the most underutilized effective interventions in medicine: fewer th

Impact of perioperative COVID-19 infection on postoperative complication in cesarean section using Korean National Health insurance data.

The vulnerability during pregnancy has raised concerns about the potential impact of COVID-19 on obstetric anesthesia, an essential aspect of maternal care during cesarean section procedures. To evaluate the influence of COVID-19 infection on obstetric anesthesia during cesarean section, we analyzed the data from Korean National Health Insurance System (NHIS). This retrospective study utilized data from Korean NHIS. We included patients admitted with operation codes specific to cesarean section between January 1, 2020, and December 31, 2021. We classified patients into a COVID (+) group with a

Incidence and treatment trends of infectious spondylodiscitis in South Korea: A nationwide population-based study.

The incidence of infectious spondylodiscitis (IS) has increased in recent years due to an increase in the numbers of older patients with chronic diseases, as well as patients with immunocompromise, steroid use, drug abuse, invasive spinal procedures, and spinal surgeries. However, research focusing on IS in the general population is lacking. This study investigated the incidence and treatment trends of IS in South Korea using data obtained from the Health Insurance Review and Assessment Service. A total of 169,244 patients (mean age: 58.0 years) diagnosed from 2010 to 2019 were included in the

A Single Institution Multi-disciplinary Approach to Power-assisted Liposuction for the Management of Lymphedema.

OBJECTIVE: To implement and evaluate outcomes from a comprehensive, multi-disciplinary debulking program in the United States. SUMMARY OF BACKGROUND DATA: Interest in and access to surgical treatment for chronic lymphedema (LE) in the United States have increased in recent years, yet there remains little attention on liposuction, or debulking, as an effective treatment option. In some other countries, debulking is a common procedure for the surgical treatment of LE, is covered by insurance, and has demonstrated excellent, reproducible outcomes. In this study we describe our experience impleme

Reoperation Rates After Surgery for Degenerative Cervical Spine Disease According to Different Surgical Procedures: National Population-based Cohort Study.

STUDY DESIGN: National population-based cohort study. OBJECTIVE: To evaluate reoperation rates of cervical spine surgery for cervical degenerative conditions utilizing a national population database. SUMMARY OF BACKGROUND DATA: There is an inherently low incidence of reoperation after surgery for cervical degenerative disease. Therefore, it is difficult to sufficiently power studies to detect differences between reoperation rates of different cervical surgical procedures. National population-based databases provide large, longitudinally followed cohorts that may help overcome this challenge.
