Primary Ovarian Pregnancy: 43 Years Experience in a Single Institute and still a Medical Challenge.

BACKGROUND: Despite awareness regarding tubal pregnancy, ovarian pregnancy still remains a diagnostic challenge. The correct diagnosis is most frequently made intraoperatively and requires histopathologic confirmation. Therefore, additional diagnostic measurements are needed for earlier and more accurate detection of ovarian pregnancies which will allow more rapid and efficient treatment. OBJECTIVES: To assess the time trends, clinical manifestations, surgical management and post-procedure outcome of 46 primary ovarian pregnancies in a single institution during three time periods. METHODS: I

Complete "in situ" avulsion of the radial artery complicating transradial coronary rotational atherectomy.

BACKGROUND: Transradial percutaneous access (TR) is promoted because of increased patient comfort and convenience as well as a lower risk of access site and cardiac complications in the literature. Increased use of the TR purports a new set of possible complications for which the vascular surgeon must be capable to recognize and manage. METHODS: A 48-year-old, devout Jehovah's Witness, woman with a history of coronary artery bypass surgery presented with a non-ST-segment elevation acute myocardial infarction. Pretransfer catheterization demonstrated a heavily calcified, 90% distal left main s

Intraoperative and perioperative complications during revision arthroplasty for salvage of a failed total ankle arthroplasty.

BACKGROUND: Revision of a failed total ankle arthroplasty (TAA) remains a challenge. Advances in total ankle implant design have renewed interest in revision TAA as an alternative to ankle arthrodesis or amputation in the management of a failed TAA. The purpose of our study was to review a series of failed Agility TAA revised to INBONE II TAA and identify reasons for revision as well as perioperative complications. METHODS: A retrospective review of 35 cases of failed Agility TAA revised to an INBONE II TAA was performed at 1 institution. Patient demographics, indications for revision, radiog

Congenital vaginal obstruction in neonates and infants: recognition and management.

BACKGROUND: Congenital vaginal obstruction commonly presents after puberty and presentation in infancy is uncommon. Due to lack of awareness, diagnosis may be missed in infants, sometimes with disastrous consequences. METHOD: A retrospective review of 9 neonates and infants treated for congenital vaginal obstruction in 16 years. RESULTS: The patients were aged 2 days to 4 months (median 28 days) at presentation. The main presenting feature was a lower abdominal mass, producing intestinal and urinary obstruction in 7 patients. In 2 patients, other conditions were suspected and diagnosis made

Aberrant right subclavian artery encountered during debridement of T2 osteomyelitis and associated phlegmon.

BACKGROUND CONTEXT: An aberrant right subclavian artery is a rare congenital abnormality of the aortic arch. The anomalous origin for the right subclavian artery arises as the last branch of the thoracic aorta. In the most common anomalous form, the right subclavian artery passes posterior to both the esophagus and trachea as it crosses midline to supply the right upper extremity. The aberrant right subclavian artery is often not symptomatic, but it can cause dysphagia. PURPOSE: To describe a case of an aberrant right subclavian artery discovered during debridement of T2 osteomyelitis. STUDY

Congenital pouch colon syndrome: a report of 17 cases.

BACKGROUND: Congenital pouch syndrome (CPC) is a rare condition seen in association with anorectal malformation that occurs almost exclusively in northern India. We reviewed cases seen in our institution to study aspects of clinical presentation, diagnosis, embryogenesis and management and raise awareness of this relatively infrequent entity. PATIENTS AND METHODS: From March 2002 to September 2004, 17 neonates/infants (11 males and 6 females) treated for CPC associated with anorectal malformations included 13 with type IV and 4 with type I CPC. Diagnosis was made by a single large air-fluid l

Nerve injury after laparoscopic varicocelectomy.

PURPOSE: Laparoscopic varicocelectomy is a minimally invasive option for varicoceles in children. Occasional reports of nerve injury after inguinal laparoscopic procedures have been published. There is anatomical variation in the sensory innervation of the anterior thigh and variable branching patterns of the nerves involved. We report a retrospective analysis of our patients, focusing on the incidence of sensory changes on the ipsilateral anterior thigh after laparoscopic varicocelectomy. MATERIALS AND METHODS: The medical records of all patients who underwent laparoscopic varicocelectomy at

Sentinel lymph node mapping and biopsy for breast cancer: a review of the literature relative to 4791 procedures.

The status of axillary nodes is the most important prognostic factor in breast cancer to select patient subgroups for adjuvant chemotherapy; the current standard of care for surgical management of invasive breast cancer is complete removal of the tumor by either mastectomy or lumpectomy followed by axillary lymph node dissection (ALND). The recent introduction of intraoperative lymphatic mapping and sentinel lymph node biopsy (SLND) represents a major new opportunity for appropriate and less invasive surgical management of many tumors. There is an almost uniformly enthusiasm concerning the pot

Recognition and management of accessory mitral tissue causing severe subaortic stenosis.

Failure to recognize the presence of accessory mitral tissue causing subaortic stenosis can lead to not only the performance of inappropriate operations, but the persistence and recurrence of obstruction or even death. Over a 12-month period, we treated 2 children with severe subaortic stenosis caused by accessory mitral tissue. In 1 patient, who was 4 years old, the echocardiogram showed the accessory mitral tissue to be attached to the anterior mitral leaflet and ballooning into the subaortic area. The other patient, as a newborn, underwent simultaneous repair of a complete canal defect and

Prosol-chlorhexidine irrigation reduces the incidence of bacteremia during ultrasonic scaling with the Cavi-Med: a pilot investigation.

The purpose of this pilot investigation was to determine whether the incidence of bacteremia following subgingival ultrasonic scaling and root planing could be reduced by the use, pre- and intraoperatively, of an irrigant containing 0.12 per cent chlorhexidine (CHX); Prosol. Individuals having evidence of significant periodontal disease (minimum of seven sites per quadrant 4.0 mm and bleeding on probing) were entered into this study. By use of a random number table, patients were assigned to either the experimental or control groups. The procedures, as described below, were carried out in a do

Pheochromocytoma--recognition and management.

Pheochromocytoma is an infrequent cause of hypertension. In spite of its rarity, pheochromocytoma has assumed notorious importance because or a wide variety of clinical features associated with this syndrome. Hypertension remains the most important clinical lead. The presence of other features, such as severe headache, perspiration, palpitations or orthostatic hypotension, makes the diagnosis of pheochromocytoma likely. The diagnosis of this condition can be made with greater certainty than that of any other form of secondary hypertension. Pharmacologic tests are no longer used for evaluation

Transcranial Motor-Evoked Potentials Provide More Reliable Intraoperative Neuromonitoring Than Somatosensory-Evoked Potentials in Charcot-Marie-Tooth Patients Undergoing Periacetabular Osteotomy for Hip Dysplasia.

BACKGROUND: Charcot-Marie-Tooth (CMT) disease is a common inherited progressive sensorimotor neuropathy which can complicate intraoperative monitoring (IOM). Transcranial motor-evoked potentials (TcMEPs) have been found to be effective in obtaining IOM in CMT patients undergoing spine surgery. The aim of this study was to examine the effectiveness of TcMEPs and somatosensory-evoked potentials (SSEPs) in IOM in CMT patients undergoing periacetabular osteotomies (PAOs) for hip dysplasia. METHODS: A retrospective case series was conducted in all CMT patients aged <19 years at a single institutio

Management of Incidental Hernia Discovered During Abdominal Contouring in Post-Bariatric Surgery Patients.

PURPOSE: An increase in bariatric surgery has led to a rise in postbariatric contouring procedures. Despite a comprehensive preoperative assessment, body habitus in these patients may significantly limit the abdominal exam. Abdominal contouring procedures typically elevate large portions of the skin and fat off the abdominal wall, and unexpected hernia may be discovered intraoperatively. No study to date has characterized such hernia discovery at the time of body contouring surgery. We reviewed our experience of management of incidental hernia found during abdominoplasty or panniculectomy afte

Diagnostic Challenge of a Tumor-mimicking Spinal Pseudogout Lesion: Case Report and Literature Review.

BACKGROUND/AIM: Retro-odontoid pseudotumor (ROP) is a non-neoplastic soft-tissue mass occurring around the C2 odontoid process and is most often associated with rheumatoid atlantoaxial instability. In rare instances, calcium pyrophosphate dihydrate (CPPD) crystal deposition (pseudogout) in the C1-C2 ligaments - a presentation known as "crowned dens syndrome" - can produce a periodontoid mass. Such CPPD-induced lesions usually present with acute neck pain and stiffness, with calcifications visible around the odontoid process on imaging. However, these masses can occasionally enlarge and compres

Different complication patterns between anterior and lateral approaches in total ankle arthroplasty: A systematic review and meta-analysis.

PURPOSE: The optimal surgical approach for total ankle arthroplasty (TAA) remains debated. The anterior and lateral transfibular approaches differ in surgical exposure and may influence complication patterns. This systematic review and meta-analysis aimed to compare revision rates, overall complication rates and specific complication profiles between anterior and lateral approaches in TAA. METHODS: A PRISMA-compliant systematic review of PubMed, Embase and Scopus databases was performed. Studies reporting complications following primary TAA performed through either approach were included if t

Prevalence of Fusion Between Adjacent Thoracic Spinous Processes in Adult Cadavers.

INTRODUCTION: Thoracic vertebrae play a crucial role in spinal stability and the protection of the spinal cord. Thoracic spinous process fusion, whether congenital or acquired, can affect spinal biomechanics and complicate surgical procedures. Thoracic spinous process fusion is an anatomical variation with potential clinical significance; however, its prevalence and distribution remain underreported. This study aimed to determine the prevalence and vertebral-level distribution of thoracic spinous process fusion in adult cadavers. METHODS: A cross-sectional observational cadaveric study was co
