Evaluation of speech and language assessment approaches with bilingual children.

BACKGROUND: British society is multicultural and multilingual, thus for many children English is not their main or only language. Speech and language therapists are required to assess accurately the speech and language skills of bilingual children if they are suspected of having a disorder. Cultural and linguistic diversity means that a more complex assessment procedure is needed and research suggests that bilingual children are at risk of misdiagnosis. Clinicians have identified a lack of suitable assessment instruments for use with this client group. AIMS: This paper highlights the challeng…

Genetic and environmental influences on writing and their relations to language and reading.

Identical and fraternal twins (N=540, age 8 to 18 years) were tested on three different measures of writing (Woodcock-Johnson III Tests of Achievement-Writing Samples and Writing Fluency; Handwriting Copy from the Group Diagnostic Reading and Aptitude Achievement Tests), three different language skills (phonological awareness, rapid naming, and vocabulary), and three different reading skills (word recognition, spelling, and reading comprehension). Substantial genetic influence was found on two of the writing measures, writing samples and handwriting copy, and all of the language and reading me…

Language lateralization in individuals with callosal agenesis: an fMRI study.

Since the seminal work of Broca in 1861, it is well established that language is essentially processed in the left hemisphere. However, the origin of hemispheric specialization remains controversial. Some authors posit that language lateralization is genetically determined, while others have suggested that hemispheric specialization develops with age. Tenants of the latter view have further suggested that the adult pattern of left hemispheric specialization is achieved by means of callosal inhibition of homologous speech areas in the right hemisphere during ontogeny. According to this hypothes…

Language performance in children with cochlear implants and additional disabilities.

OBJECTIVES/HYPOTHESIS: Quantify post-cochlear implant (CI) language among children with disabilities and determine the role of nonverbal cognitive quotients (NVCQ) in predicting language. STUDY DESIGN: Small cohort study in pediatric tertiary care center. METHODS: Children (n = 20) with CIs and developmental disabilities were enrolled. Receptive and expressive language was reported as language quotients (LQs). Pre- and post-CI LQs were compared using the signed-rank test. Multiple regression models analyzed language while controlling for possible confounders. RESULTS: Five subjects had symp…

Providing written language services in the schools: the time is now.

PURPOSE: The current study was conducted to investigate the provision of written language services by school-based speech-language pathologists (SLPs). Specifically, the study examined SLPs' knowledge, attitudes, and collaborative practices in the area of written language services as well as the variables that impact provision of these services. METHOD: Public school-based SLPs from across the country were solicited for participation in an online, Web-based survey. Data from 645 full-time SLPs from 49 states were evaluated using descriptive statistics and logistic regression. RESULTS: Many s…

Language assessment in Wada test: comparison of methohexital and amobarbital.

INTRODUCTION: Methohexital has replaced amobarbital during Wada testing at many centers. The objective of our study was to compare the use of methohexital and amobarbital during Wada testing regarding language and memory lateralization quotients as well as speech arrest times. METHODS: A chart review of 582 consecutive patients undergoing 1041 Wada-procedures was performed (left=60, right=63, bilateral=459). Language lateralization was calculated based on duration of speech arrest using a laterality index, defined as (L-R)/(L+R). Memory lateralization was expressed as percentage of retained o…

Grounded theory as a method for research in speech and language therapy.

BACKGROUND: The use of qualitative methodologies in speech and language therapy has grown over the past two decades, and there is now a body of literature, both generally describing qualitative research, and detailing its applicability to health practice(s). However, there has been only limited profession-specific discussion of qualitative methodologies and their potential application to speech and language therapy. AIM: To describe the methodology of grounded theory, and to explain how it might usefully be applied to areas of speech and language research where theoretical frameworks or model…

Awareness of breathing: the structure of language descriptors of respiratory sensations.

OBJECTIVE: Recent research suggests that dyspnea is not a single sensation but a multidimensional construct reflected in different verbal descriptors that can provide useful diagnostic information. In this study superordinated clusters of dyspnea were investigated in combination with a dimensional approach. DESIGN: We examined the use of 20 respiratory symptom descriptors by healthy volunteers who completed a protocol of seven experimental conditions: Quiet breathing, breath holding, paced breathing, climbing stairs, resistive load breathing, voluntary hyperinflation, and voluntary hyperventi

FMRI lateralization of expressive language in children with cerebral lesions.

PURPOSE: Lateralization of language function is crucial to the planning of surgery in children with frontal or temporal lobe lesions. We examined the utility of functional magnetic resonance imaging (fMRI) as a determinant of lateralization of expressive language in children with cerebral lesions. METHODS: fMRI language lateralization was attempted in 35 children (29 with epilepsy) aged 8-18 years with frontal or temporal lobe lesions (28 left hemisphere, five right hemisphere, two bilateral). Axial and coronal fMRI scans through the frontal and temporal lobes were acquired at 1.5 Tesla by us…

Language and arithmetic--a study using the intracarotid amobarbital procedure.

The intracarotid amobarbital procedure is used as a standard procedure in presurgical evaluation to assess hemispheric lateralization of language and memory, but has not been applied to investigate numerical processing. Patients with medically intractable epilepsy (n=20) were consecutively recruited during a presurgical evaluation programme. All 14 patients with left-lateralized language showed better arithmetic performance with the left hemisphere (intracarotid amobarbital procedure right), while five out of six patients with bilateral or right-hemispheric language representation showed bette…

The effects of motor neurone disease on language: further evidence.

It might sound surprising that Motor Neurone Disease (MND), regarded still by many as the very example of a neurodegenerative disease affecting selectively the motor system and sparing the sensory functions as well as cognition, can have a significant influence on language. In this article we hope to demonstrate that language dysfunction is not only a pronounced and well documented symptom in some MND patients but also that the study of language in MND can address interesting theoretical questions about the representation of language and conceptual knowledge in the brain. After a brief introdu…

The early identification of dyslexia: children with English as an additional language.

It is generally accepted that dyslexia should be identified early for interventions to have maximum effect. However, when children speak English as an additional language (EAL), diagnosis is more complex and there is concern that these children tend to be under-identified. This paper reports a longitudinal study following the development of phonological awareness skills in relation to progress in learning to read with a cohort of British Asian children learning EAL and their monolingual peers. It also sought to determine the usefulness of a measure of phonological skills for the identification…

Early language development in children with a genetic risk of dyslexia.

We report on a prospective longitudinal research programme exploring the connection between language acquisition deficits and dyslexia. The language development profile of children at-risk for dyslexia is compared to that of age-matched controls as well as of children who have been diagnosed with specific language impairment (SLI). The experiments described concern the perception and production of grammatical morphology, categorical perception of speech sounds, phonological processing (non-word repetition), mispronunciation detection, and rhyme detection. The results of each of these indicate …

Walter Moxon and his thoughts about language and the brain.

Walter Moxon wrote a well-timed paper in 1866 on aphemia with accompanying right sided hemiplegia. Like many other investigators of this apparent lopsidedness of the articulatory system for human speech. Moxon had to reconcile left hemisphere specialization for this function with the overriding law of symmetry, which for a large sector of the scientific community was a sine qua non of the anatomy and function of high level animal cognition. His reasoning was essentially that since the right dominant hand (and hence the left hemisphere) in some sense led overall bilateral limb movement patterns

Provision of speech-language services in the schools: working with the law.

Decisions regarding eligibility for speech-language services in the schools can be the most difficult decisions facing school-based speech-language pathologists. Clinicians and parents must be informed about the federal mandates for services in the schools for children with special needs in order to make appropriate decisions and to ensure that children with disabilities are receiving the services to which they are entitled. This article discusses the Individuals with Disabilities Education Act (IDEA), the federal law guaranteeing school-based services to children with disabilities, as an educ…

Maternal neuronal antibodies associated with autism and a language disorder.

Neurodevelopmental disorders could be caused by maternal antibodies or other serum factors. We detected serum antibodies binding to rodent Purkinje cells and other neurons in a mother of three children: the first normal, the second with autism, and the third with a severe specific language disorder. We injected the serum (0.5-1.0 ml/day) into pregnant mice during gestation and found altered exploration and motor coordination and changes in cerebellar magnetic resonance spectroscopy in the mouse offspring, comparing with offspring of mice injected with sera from mothers of healthy children. Thi…
