[Effectiveness of total hip arthroplasty for severe developmental dysplasia of hip in adults].

OBJECTIVE: To evaluate the effectiveness of the total hip arthroplasty (THA) for severe development dysplasia of the hip (DDH) in adults. METHODS: Between July 2007 and January 2013, 25 patients (27 hips) with severe DDH underwent THA with cementless prosthesis. There were 10 males (11 hips) and 15 females (16 hips) with an average age of 38.6 years (range, 21-57 years). The left hip was involved in 10 cases, the right hip in 13 cases, and bilateral hips in 2 cases. There were 8 cases (9 hips) of Crowe type III and 17 cases (18 hips) of Crowe type IV. The main clinical manifestations were uni

Precision of the PRECICE internal bone lengthening nail.

BACKGROUND: Previous designs of internal bone lengthening devices have been fraught with imprecise distraction, resulting in nerve injuries, joint contractures, nonunions, and other complications. Recently, a magnet-operated PRECICE nail (Ellipse Technologies, Inc, Irvine, CA, USA) was approved by the FDA; however, its clinical efficacy is unknown. QUESTIONS/PURPOSES: We evaluated this nail in terms of (1) accuracy and precision of distraction, (2) effects on bone alignment, (3) effects on adjacent-joint ROM, and (4) frequency of implant-related and non-implant-related complications. METHODS

Navigation in femoral-shaft fractures--from lab tests to clinical routine.

INTRODUCTION: This study evaluates the use of a navigation system (BrainLAB, Feldkirchen, Germany) to intra-operatively check for correct length, axis and rotation in intramedullary nailing of femoral-shaft fractures in an experimental setting and in clinical routine. MATERIALS AND METHODS: We tested the navigation system in two experimental settings before introducing it into clinical routine. In the first experiment, 10 osteotomised model femora were fixed with intramedullary nails by using a navigation system. The goal was a locking fixation in predefined values for length and rotation. In

[Do paediatric and adult surgeons follow different approaches to physeal fractures?].

INTRODUCTION: The treatment of paediatric fractures is the concern of several different surgical specialties. There has been no scientific investigation on the different concepts of paediatric (PS) and adult surgeons (AS). METHODS: 62 paediatric traumatologists were asked concerning their experience with physeal fractures of the leg, including ten cases. RESULTS: Growth disturbances was estimated to be more rare by PS. On evaluation of the examples there were no significant differences in the judgement of degree and direction of the displacement. For displaced fractures, PS rather preferred

An alternative treatment method to restore limb-length discrepancy in osteoarthritis with high congenital hip dislocation.

AIM: The purpose of this study was to describe the rationale and to analyse the results of uncemented total hip arthroplasty (THA) in patients with osteoarthritis and congenital dislocation of the hip (CDH) prior to iliofemoral distraction. METHOD: Twenty hips were followed-up for 5.8 years (1-12 years). First the THA components were implanted after soft tissue release. In the interval, iliofemoral distraction with an external distraction apparatus was conducted. The second surgery consisted of an open reduction. An iliofemoral distraction of 0.8 mm/day was monitored. The treatment time was 8

Outcome in unilateral or bilateral DDH treated with one-stage combined procedure.

UNLABELLED: The surgical treatment of patients with neglected developmental dysplasia of the hip (DDH) has been the subject of controversy. We asked if age affected outcome in patients with neglected DDH with unilateral or bilateral dislocation who underwent one-stage combined procedures. We retrospectively reviewed the results of 40 patients (51 hips) treated with a one-stage combined procedure consisting of open reduction, pelvic osteotomy, and femoral shortening. The average age at the time of surgery was 5.4 years for Group I (bilateral dislocation, 22 hips) and 6.7 years for Group II (uni

Coxa vara with proximal femoral growth arrest in patients who had neonatal extracorporeal membrane oxygenation.

This is a retrospective review of four patients in whom a pattern of coxa vara with proximal femoral growth arrest and metaphyseal irregularities developed. These patients were all treated with neonatal extracorporeal membrane oxygenation and presented with a progressive gait disturbance and pain, leg-length discrepancy, and limited abduction. Imaging revealed coxa vara with proximal femoral growth arrest. Two patients (three hips) underwent proximal femoral valgus osteotomy, one patient underwent fixation of a femoral neck fracture with subsequent greater trochanter transfer, and one patient

[Growth prognosis after para-epiphyseal tangential epiphysial injuries of the lower extremity exemplified by the distal femur: outcome after injuries of epiphyses of the distal femur].

Fractures of the distal end of the femur are rare. Premature partial closure of the physis may occur after metaphyseal fractures. After epiphyseal fractures it has a high incidence of occurrence. The cause of physeal arrest is not known. Stimulation of the physes ends in leg length discrepancy and was seen in 50% of all cases. Spontaneous correction of an axis deviation is possible after antecurvation up to an age of 5 years. But this should be only accepted during therapy of metaphyseal fractures of the distal femur. Side-to-Side deviations remodel in all cases. Aim of therapy in distal epiph

Unstable pelvic fractures in children.

A total of 3561 freshly injured children received treatment at the paediatric trauma department of the National Institute of Traumatology between 1984 and 1994. Out of these 38 (approx. 1%) had pelvic fractures. Based on the classification of Tile and Laer, 15 pelvic fractures were diagnosed to be unstable, from which 8 were polytraumatised, 4 shocked and 1 was a casualty. Run downs could be considered as the main cause of these fractures. Conservative treatment was provided for 13 unstable cases (bed rest: 5, band suspension: 1, femur skeletal traction: 4, femur traction and band suspension:

Combined capillary-venous-lymphatic malformations without overgrowth in patients with Klippel-Trénaunay syndrome.

OBJECTIVE: Klippel-Trénaunay syndrome (KTS) is described in the literature as a complex syndrome characterized by various combinations of capillary, venous, and lymphatic malformations associated with limb overgrowth. In the first description by Maurice Klippel and Paul Trénaunay, tridimensional bone hypertrophy was believed to be the cause of limb enlargement. The purpose of this study was primarily to assess the presence of real bone hypertrophy as a cause of enlargement of the limb and to underline the rare presence of undergrowth of the affected limb in patients with KTS. METHODS: A two-c

Physeal bar equivalent.

Premature partial physeal arrest without the formation of an osseous bar - physeal bar equivalent (PBE) - is uncommon. Four children with a PBE had an infection near the distal femoral physis before the age of 11 months. Some growth was achieved after resection of the PBE in each case. Of two cases diagnosed and treated early, one required only contralateral physeal arrests to achieve limb-length equality at maturity. The other, currently 8 years and 4 months old, has a 1.1-cm limb-length discrepancy 6 years after PBE resection and will require observation until maturity. Of two cases diagnose

Management of ununited intracapsular femoral neck fractures by using quadratus femoris muscle pedicle bone grafting in young patients.

OBJECTIVES: Intracapsular femoral neck fracture with delayed presentation in young patients can pose surgical challenge. Such scenarios are rare in developed countries, but common in developing countries. METHODS: We retrospectively reviewed the records and radiographs of 42 patients (28 males and 14 females) with ununited intracapsular femoral neck fracture who presented late to our unit. These subjects were managed by open reduction and internal fixation that was supplemented with cortico-cancellous bone graft from posterior iliac crest as well as quadratus femoris muscle pedicle bone graft

Greater trochanteric transfer for the treatment of coxa brevis.

We reviewed the outcomes of 28 patients for whom we did greater trochanteric transfer. Twenty-two (79%) patients treated for coxa brevis by greater trochanteric transfer had good relief of pain and limp. Four (1%) outcomes were fair and two (0.7%) were poor. Our outcomes compared favorably with those already reported in the literature. Patients who had avascular necrosis caused by Perthes disease tended to have a better outcome than those caused as a complication of treatment for hip dysplasia. A computer model based on radiographic measurements was useful in planning the placement of the tran

Insufficient stem antetorsion and lower cup abduction is a combined risk factor for posterior hip dislocation in patients undergoing THA for femoral neck fractures: a retrospective analysis.

BACKGROUND: The role of acetabular and femoral component positions with respect to the risk of post-operative instability and dislocation remains debated. In this study, we aimed to identify potential risk factors for early dislocation following primary total hip arthroplasty (THA) for displaced intracapsular femoral neck fractures (FNF) using radiological measurements. METHODS: We retrospectively analyzed data for patients who underwent cementless primary THA for FNF using a posterolateral approach between January 2018 and December 2021. Follow-up duration, age, sex, affected side, and mean

Double-tapered conical taper in primary and revision surgery: rationale and short-term follow-up.

BACKGROUND:: The new double-conical tapered stem is a novel cementless modular system, which is indicated for both primary and revision surgeries. It has been designed with different angles at the proximal (5°) and distal sections (1° 36') in order to obtain optimal fixation by proximal and distal fit and fill of the femoral canal. AIM:: The aim of this prospective study was to evaluate the short-term clinical and radiographic outcomes of primary and revision hip surgery with the new double-conical stem. METHODS:: 61 stems were implanted (December 2013 to September 2016) in 42 cases of prima
