Overcoming resistance in chronic myelogenous leukemia.

Chronic myeloid leukemia is defined by the acquired genetic mutation, t(9;22), which leads to the fusion-protein BCR-ABL. Prior to the development of imatinib mesylate (Gleevec), treatment was limited and provided only limited survival benefit. Imatinib has dramatically changed the course of the disease and has led to the significantly prolonged survival in the majority of patients. However, there is growing concern for resistance to imatinib and to subsequent second generation tyrosine kinase inhibitors (dasatinib and nilotinib) due to the T315I mutation. With no currently approved effective …

The chronic myeloid leukemia stem cell.

Chronic myeloid leukemia (CML) is a clonal stem cell disorder that is characterized by the acquired chromosomal translocation BCR-ABL. This gives rise to a constitutively active tyrosine kinase deregulation of the normal mechanisms of cell cycle control. In the normal hematopoietic system, hematopoietic stem cells (HSC) self-renew to form identical daughter cells but also differentiate to mature blood cells. Leukemic stem cells (LSC) share these properties of self-renewal and also differentiate to mature leukemic cells. LSC have been isolated from patients with CML: these cells give rise to le…

Targeted therapy for chronic lymphocytic leukemia.

The introduction of targeted agents such as the monoclonal antibodies rituximab (anti-CD20) and alemtuzumab (anti-CD52) has brought about a remarkable change in the therapy of chronic lymphocytic leukemia (CLL). Although it is unclear whether overall survival has been improved, the incorporation of these monoclonal antibodies into chemoimmunotherapy regimens has dramatically improved complete response rates and progression-free survival in patients with both newly-diagnosed and relapsed CLL. The success of rituximab and alemtuzumab has spurred the development of other monoclonal antibodies tar…

High hyperdiploid childhood acute lymphoblastic leukemia.

High hyperdiploidy (51-67 chromosomes) is the most common cytogenetic abnormality pattern in childhood B-cell precursor acute lymphoblastic leukemia (ALL), occurring in 25-30% of such cases. High hyperdiploid ALL is characterized cytogenetically by a nonrandom gain of chromosomes X, 4, 6, 10, 14, 17, 18, and 21 and clinically by a favorable prognosis. Despite the high frequency of this karyotypic subgroup, many questions remain regarding the epidemiology, etiology, presence of other genetic changes, the time and cell of origin, and the formation and pathogenetic consequences of high hyperdiplo…

Bendamustine therapy in chronic lymphocytic leukemia.

BACKGROUND: Bendamustine is now approved in the US for the treatment of chronic lymphocytic leukemia and low grade non-Hodgkin's lymphoma, and is currently being explored in the treatment of several solid tumor types. OBJECTIVE: The bi-functionality of bendamustine was used to provide a therapeutic understanding of both its benefit as well as adverse effects. METHODS: Pertinent biochemistry and molecular biology pathways are reviewed with regards to bendamustine activity. In view of these pathways bendamustine was reviewed in human clinical trials. RESULTS/CONCLUSION: Bendamustine combines …

Adipocytes impair leukemia treatment in mice.

Obesity is associated with increased cancer incidence and mortality. We have previously found that obesity in children is associated with a 50% increased recurrence of acute lymphoblastic leukemia (ALL) in high-risk patients. We have therefore developed novel in vivo and in vitro preclinical models to study the mechanism(s) of this association. Obesity increased relapse after monotherapy with vincristine (P = 0.03) in obese mice injected with syngeneic ALL cells. This occurred although the drug was dosed proportionally to body weight, equalizing blood and tissue drug levels. In coculture, 3T3-…

High-risk childhood acute lymphoblastic leukemia.

Although most children with acute lymphoblastic leukemia (ALL) are cured, certain subsets have a high risk of relapse. Relapse risk can be predicted by early response to therapy, clinical and pharmacogenetic features of the host, and genetic characteristics of leukemic cells. Though early treatment response can be assessed by the peripheral blast cell count after 1 week of single-agent glucocorticoid treatment or percent of bone marrow blasts by morphology after 1 or 2 weeks of multiagent induction treatment, determination of minimal residual disease by polymerase chain reaction (PCR) or flow …

Cellular therapies in acute lymphoblastic leukemia.

The majority of adult patients with acute lymphoblastic leukemia (ALL) will die from the disease. Although the prognosis for pediatric patients is significantly better than for adult patients with ALL, the prognosis for patients with relapsed or refractory disease is poor in all cases. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) from a related donor offers a significant therapeutic benefit for pediatric patients, although the benefit of this therapy to adults with ALL is less established. Because most patients lack a suitable related donor, alternative approaches to allo-HSC…

[New insights into acute lymphoblastic leukemia].

Acute lymphoblastic leukemia is a malignant disorder of lymphoid progenitor cells. Advances in our understanding of lymphoblastic leukemia have mainly come from new molecular technologies and genomics. This article describes recent advances in our understanding of maturation arrest of leukemic cells, initial and subsequent gene defects and rearrangements, the role of chemokines, and lymphoid cell homing. These advances point to new ways of targeting leukemic cells.

PRAME expression in hairy cell leukemia.

PRAME has been proposed as a useful marker for solid tumors and acute B-cell malignancies. Several studies demonstrate expression in CLL. To further examine its B-cell tumor distribution, we studied PRAME in both CLL and hairy cell leukemia (HCL). While by conventional PCR only 8% of 37 HCL and 27% of 22 CLL patients were positive, nearly all patients and normal donors expressed PRAME by real-time quantitative (TaqMan) PCR. We conclude that HCL and CLL differ in PRAME overexpression, and that basal normal expression of PRAME may limit its usefulness for following patients with minimal residual…

[Multidrug resistance in chronic lymphocytic leukemia].

INTRODUCTION: New prognostic factors discovered in chronic lymphocytic leukemia have recently got into the center of clinical interest. While the predictive value of cytogenetical abnormalities, immunoglobulin heavy chain gene mutation status, CD38 and ZAP70 expression is already well known, the significance of multi-drug resistance in chronic lymphocytic leukemia is not well characterized. AIMS: The goal of this study was to characterize the multidrug resistance features in 82 patients with chronic lymphocytic leukemia at the genetical, expression- and functional level and to compare it with…

Pulmonary alveolar proteinosis in pediatric leukemia.

BACKGROUND: Pulmonary alveolar proteinosis (PAP) is a rare disorder characterized by intra-alveolar accumulation of periodic acid-Schiff (PAS)-positive surfactant components. Leukemia is the cancer most often associated with PAP; prolonged neutropenia and reduction of alveolar macrophages by myeloablative chemotherapy or leukemic infiltration are implicated. Only isolated cases of PAP have been reported, and pediatric experience is limited. PROCEDURE: We reviewed all pathology records (1962-2007) of St. Jude Children's Research Hospital to identify patients with PAP. RESULTS: Five patients h…

Curative strategies in acute promyelocytic leukemia.

Acute promyelocytic leukemia (APL) was initially described as a distinct clinical entity in 1957, one year before the first meeting of the American Society of Hematology. With routine administration of all-trans retinoic acid (ATRA) combined with chemotherapy in the early 1990s and arsenic trioxide (ATO) in the late 1990s, cure can now be expected in the majority of both newly diagnosed and relapsed patients. ATRA with anthracycline-based chemotherapy for induction and consolidation followed by ATRA plus low-dose chemotherapy maintenance is currently the standard of care for newly diagnosed pa…

Monoclonal antibody-based therapeutics for leukemia.

Monoclonal antibodies (mAbs) represent a unique class of therapeutics because of their exquisite antigen specificity and effector mechanisms. Clinically, antibody-based therapy has increasingly become an established modality for the treatment of cancer. Among the nine mAbs and immunoconjugates approved by the FDA for therapeutic indications in oncological diseases, five are for use in hematological malignancies. Alemtuzumab is approved for B cell chronic lymphocytic leukemia; rituximab, an anti-CD20 antibody approved for the treatment of B cell non-Hodgkin's lymphoma, is being tested in patien…

Leptomeningeal disease in chronic lymphocytic leukemia.

Chronic lymphocytic leukemia (CLL) is the most common lymphoproliferative disorder in the western hemisphere, with an annual incidence of 3:100000. Commonly patients are asymptomatic but not rarely disease progression occurs in the setting of lymphadenopathy and extensive leukemic burden. Leptomeningeal involvement in patients with CLL is infrequent, with presenting symptoms of headache (23%), acute or chronic changes in mental status (28%), cranial nerve abnormalities (54%) including optic neuropathy (28%), weakness of lower extremities (23%) and cerebellar signs (18%). In this report, we dis…

BCR-ABL-negative chronic myeloid leukemia.

Constitutive activation of protein tyrosine kinases plays a central role in the pathogenesis of myeloproliferative disorders, including BCR-ABL-negative chronic myeloid leukemia. Current research is focused on elucidating the full spectrum of causative mutations in this rare, heterogeneous disease. Activated tyrosine kinases are excellent targets for signal transduction therapy, and an accurate diagnosis including morphology, karyotyping, and molecular genetics will become increasingly important to direct individualized treatment. In addition, new molecular findings need to be incorporated int…
