Hippocampal hypometabolism in older adults with memory complaints and increased amyloid burden.

OBJECTIVE: To identify the functional and pathologic correlates underlying subjective memory complaints (SMCs) in cognitively normal older adults. METHODS: Two hundred fifty-one older adults underwent resting-state fluorodeoxyglucose (FDG)-PET and Pittsburg compound B-PET β-amyloid (Aβ) imaging and filled out a questionnaire regarding SMCs. Participants were classified into 2 groups based on their Aβ burden. Age-adjusted voxel-wise correlations were used to examine SMCs, amyloid status (Aβ+ vs Aβ-), and the interaction between SMCs and Aβ status as predictors of metabolism. Region-of-interest

Optic disc edema in raised intracranial pressure. VI. Associated visual disturbances and their pathogenesis.

The pattern and pathogenesis of nonlocalizing visual disturbances, associated with optic disc edema (ODE). raised cerebrospinal fluid pressure, and intracranial space-taking lesions were investigated experimentally in rhesus monkeys with simulated progressive brain tumor and clinically in patients with benign intracranial hypertension. The visual disturbances occurring in one of both eyes were of three types: recurrent attacks of transient obscuration, permanent blindness, and various types of visual field defects. The studies indicate that the visual disturbances are usually due to two mechan

Regional brain iron deposition and functional alterations in thalassemia: a multimodal human study with murine validation.

BACKGROUND AND OBJECTIVES: Chronic anemia and systemic iron overload in thalassemia major (TM) patients may result in neurological deficits, although researchers have not completely elucidated their specific impacts on the brain. This study employed a multimodal research methodology to investigate the characteristics and intrinsic connections of cerebral iron deposition, cerebral dysfunction, and cognitive impairment in TM patients, providing theoretical support for brain monitoring and neuroprotective interventions in this condition. METHODS: We prospectively enrolled 88 patients with TM, in

Altered EEG microstate dynamics reflect depressive symptoms in temporal lobe epilepsy.

BACKGROUND: Depressive symptoms are a common and disabling comorbidity in temporal lobe epilepsy (TLE), yet the neural mechanisms linking seizure networks to affective symptoms remain unclear. Although limbic network dysfunction has been implicated in both epilepsy and depressive disorders, it is unknown whether the time-varying dynamics of large-scale electrophysiological brain states reflect depressive symptom severity in TLE. In this study, we examined whether EEG microstate dynamics capture network alterations associated with depressive symptoms in individuals with unilateral TLE. METHODS

Widespread distribution of α-synuclein oligomers in LRRK2-related Parkinson's disease.

Mutations in leucine-rich repeat kinase 2 (LRRK2) are the most common cause of familial and sporadic Parkinson's disease (PD). While the clinical features of patients with LRRK2-PD resemble those of typical PD, there are significant differences in the pathological findings. The pathological hallmark of definite PD is the presence of α-synuclein (αSYN)-positive Lewy-related pathology; however, approximately half of patients with LRRK2-PD do not have Lewy-related pathology. Lewy-related pathology is a late-stage αSYN aggregation that can be visualized with hematoxylin and eosin stains or convent

Structural-functional coupling abnormalities in temporal lobe epilepsy.

BACKGROUND: Nowadays, researchers are using advanced multimodal neuroimaging techniques to construct the brain network connectome to elucidate the complex relationship among the networks of brain functions and structure. The objective of this study was to evaluate the coupling of structural connectivity (SC) and functional connectivity (FC) in the entire brain of healthy controls (HCs), and to investigate modifications in SC-FC coupling in individuals suffering from temporal lobe epilepsy (TLE). METHODS: We evaluated 65 patients with TLE matched for age and gender with 48 healthy controls. Th

A diagnostic index based on quantitative susceptibility mapping and voxel-based morphometry may improve early diagnosis of Alzheimer's disease.

OBJECTIVES: Voxel-based morphometry (VBM) is widely used to quantify the progression of Alzheimer's disease (AD), but improvement is still needed for accurate early diagnosis. We evaluated the feasibility of a novel diagnosis index for early diagnosis of AD based on quantitative susceptibility mapping (QSM) and VBM. METHODS: Thirty-seven patients with AD, 24 patients with mild cognitive impairment (MCI) due to AD, and 36 cognitively normal (NC) subjects from four centers were included. A hybrid sequence was performed by using 3-T MRI with a 3D multi-echo GRE sequence to obtain both a T1-weigh

Common and Distinct Disruptions of Cortical Surface Morphology Between Autism Spectrum Disorder Children With and Without SHANK3 Deficiency.

SH3 and Multiple Ankyrin Repeat Domains 3 (SHANK3)-caused autism spectrum disorder (ASD) may present a unique opportunity to clarify the heterogeneous neuropathological mechanisms of ASD. However, the specificity and commonality of disrupted large-scale brain organization in SHANK3-deficient children remain largely unknown. The present study combined genetic tests, neurobehavioral evaluations, and magnetic resonance imaging, aiming to explore the disruptions of both local and networked cortical structural organization in ASD children with and without SHANK3 deficiency. Spatiotemporal distribution and age of seizure onset in a pediatric epilepsy surgery cohort with cortical dysplasia.

OBJECTIVE: Focal Cortical Dysplasias (CD) are a common etiology of refractory pediatric epilepsy and are amenable to epilepsy surgery. We investigated the association of lesion volume and location to age of seizure onset among children with CD who underwent epilepsy surgery. METHODS: A retrospective study of epilepsy surgery patients with pathologically-confirmed CD. Regions of interest (ROI) determined preoperative lesion volumes on 1.5 T and 3 T T2 and SPGR MRIs, and location in 7 distributed neural networks. Descriptive and inferential statistics were used. RESULTS: Fifty-five patients we

Long-term reactions to pulsatile tinnitus are marked by weakened short-range functional connectivity within a brain network in the right temporal lobe.

BACKGROUND: There have been recent efforts to characterize brain functional activity features in patients with pulsatile tinnitus (PT). These efforts have revealed evidence of aberrant functional connectivity (FC) of the right middle temporal gyrus (MTG) in PT patients with prolonged disease duration. PURPOSE: To assess the possible predictive effect of aberrant FC of MTG in PT patients with prolonged disease duration. STUDY TYPE: Retrospective. POPULATION: Thirty-four patients with recent-onset PT (RPTIN), 24 patients with long-term PT (LPTIN), and 35 age-, gender-, and education-matched h

A study of the brain functional network of Deqi via acupuncturing stimulation at BL40 by rs-fMRI.

OBJECTIVE: Acupuncture is a therapeutic treatment defined as the insertion of needles into the body at specific points (i.e., acupoints). The acupuncture sensation of Deqi is an important component of acupuncture, but the functional brain responses of Deqi have not been entirely supported by the results of functional magnetic resonance imaging (fMRI) studies. The aims of this study were to test the conditions that would generate a Deqi sensation and to investigate the effect of Deqi and the response of acupuncture at different depths and intensities on brain fMRI blood oxygen level-dependent (

Association of White Matter Hyperintensities, Regional Brain Glucose Metabolism, and Cognitive Impairment in Aβ-Negative Patients.

BACKGROUND AND OBJECTIVES: Periventricular white matter hyperintensities (PWMHs) and regional brain glucose hypometabolism have each been linked to cognitive impairment, but their interplay independent of β-amyloid (Aβ) is unclear. This study examines whether PWMHs relate to region-specific cortical hypometabolism and metabolism mediates domain-specific cognition in Aβ-negative individuals. METHODS: This retrospective cross-sectional study included 141 Aβ-negative patients with mild cognitive impairment (MCI) older than 50 years and 83 normal controls (NCs). All participants underwent 18F-flu

Brain-wide magnetic resonance imaging analysis of neurobehavioral comorbidities in temporal lobe epilepsy.

OBJECTIVE: In temporal lobe epilepsy (TLE), patients often present with neurobehavioral comorbidities encompassing affective and cognitive difficulties. Whereas the latter have been related to atypical connectivity of mesiotemporal and frontotemporal circuits, the brain basis of affective symptoms remains incompletely understood. Here, we assessed functional network substrates of affective symptoms and examined their relationship to structural magnetic resonance imaging (MRI), cognition, and clinical parameters. METHODS: We studied 42 drug-resistant patients who underwent multimodal 3-T MRI a

Specific atrophy patterns distinguish tau and TDP-43 pathology: a longitudinal MRI ante-mortem study.

TDP-43 pathology frequently co-occurs with Alzheimer’s disease (AD), indicative of Limbic-predominant Age-related TDP-43 Encephalopathy neuropathologic changes (LATE-NC), which is associated with more severe medial temporal lobe (MTL) atrophy and cognitive decline compared to ‘pure’ AD. As both tau and TDP-43 target overlapping MTL structures and are associated with similar clinical manifestations, disentangling their respective contributions to neurodegeneration is essential yet challenging. In this study, we investigated the differential impact of tau and TDP-43 pathologies on MTL subregions

The role of brain MR and FDG-PET in the diagnosis of neurodegenerative disease.

Alzheimer disease (AD) is the most common dementing disorder, affecting 55 million people worldwide. Brain MRI plays an integral role in the diagnostic evaluation of patients with cognitive symptoms. When interpreting brain MRI for cognitive impairment, radiologists should assess the following four key features: (1) white matter ischemic burden, (2) structural changes to suggest normal pressure hydrocephalus, (3) locoregional pattern of brain atrophy, and (4) presence of microhemorrhage or superficial siderosis, particularly for determining eligibility for anti-amyloid monoclonal antibody (MAB

COVID-19 is associated with changes in brain function and structure: A multimodal meta-analysis of neuroimaging studies.

The actual role of coronavirus disease 2019 (COVID-19) in brain damage has been increasingly reported, necessitating a meta-analysis to collate and summarize the inconsistent findings from functional imaging and voxel-based morphometry (VBM) studies. A comprehensive voxel-wise meta-analysis of the whole brain was conducted to identify alterations in functional activity and gray matter volume (GMV) between COVID-19 patients and healthy controls (HCs) by using Seed-based d Mapping software. We included 15 functional imaging studies (484 patients with COVID-19, 534 HCs) and 9 VBM studies (449 pat

MRI findings in autoimmune encephalitis.

Autoimmune encephalitis encompasses a spectrum of conditions characterized by distinct clinical features and magnetic resonance imaging (MRI) findings. Here, we review the literature on acute MRI changes in the most common autoimmune encephalitis variants. In N-methyl-D-aspartate (NMDA) receptor encephalitis, most patients have a normal MRI in the acute stage. When lesions are present in the acute stage, they are typically subtle and non-specific white matter lesions that do not correspond with the clinical syndrome. In some NMDA receptor encephalitis cases, these T2-hyperintense lesions may b

TDP-43 Pathology Exacerbates Cognitive Decline in Primary Age-Related Tauopathy.

OBJECTIVE: Primary age-related tauopathy (PART) refers to tau neurofibrillary tangles restricted largely to the medial temporal lobe in the absence of significant beta-amyloid plaques. PART has been associated with cognitive impairment, but contributions from concomitant limbic age-related TDP-43 encephalopathy neuropathologic change (LATE-NC) are underappreciated. METHODS: We compare prevalence of LATE-NC and vascular copathologies in age- and Braak-matched patients with PART (n = 45, Braak stage I-IV, Thal phase 0-2) or early stage Alzheimer disease neuropathologic change (ADNC; n = 51, Bra

Correlated Resting-State Functional MRI Activity of Frontostriatal, Thalamic, Temporal, and Cerebellar Brain Regions Differentiates Stroke Survivors with High Compared to Low Depressive Symptom Scores.

BACKGROUND: One in three survivors of stroke experience poststroke depression (PSD). PSD has been linked with poorer recovery of function and cognition, yet our understanding of potential mechanisms is currently limited. Alterations in resting-state functional MRI have been investigated to a limited extent. Fluctuations in low frequency signal are reported, but it is unknown if interactions are present between the level of depressive symptom score and intrinsic brain activity in varying brain regions. OBJECTIVE: To investigate potential interaction effects between whole-brain resting-state ac
