Replacement of lip-split mandibulotomy by pull-through approach for T3-4 oral carcinomas.

At the study hospital, the lip-split mandibulotomy (LSM) has progressively been replaced by a pull-through (PT) approach. This study compared the outcomes of the LSM and PT approaches in a series of 192 patients with T3-T4a oral tongue and floor of the mouth squamous cell carcinoma treated over the two last decades. No difference in margin status (P = 0.254), rate of early complications (local infections) (P = 0.867), haematoma/haemorrhage (P = 0.221), delayed wound healing (P = 0.438), re-operation (P = 0.083), or Clavien-Dindo classification (P= 0.5281) was found. The LSM approach was associ

Peripheral ameloblastic fibro-odontoma-Report of 2 cases of a very rare entity.

Ameloblastic fibro-odontoma (AFO) is a rare benign mixed odontogenic tumor that affects children and young adults. AFO occurs mainly intraosseous. Extraosseous AFO is extremely rare. We report 2 cases of rare peripheral ameloblastic fibro-odontoma in 2- and 12-year-old female patients. Microscopic examination revealed a benign proliferation of odontogenic epithelium associated with a dentinoid material distributed within a cell-rich mesenchymal stroma resembling dental papilla. Simple surgical excision of the lesion is usually curative. There was no recurrence after a short period of follow-up

Prolonged Hypophosphatemia and Intensive Care After Curative Surgery of Tumor Induced Osteomalacia: A Case Report.

INTRODUCTION: Rare FGF23-producing mesenchymal tumors lead to paraneoplastic tumor-induced osteomalacia (TIO) presenting with phosphate wasting, hypophosphatemia, chronic hypomineralization of the bone, fragility fractures and muscle weakness. Diagnosis of TIO requires exclusion of other etiologies and careful search for a mesenchymal tumor that often is very small and can appear anywhere in the body. Surgical removal of the tumor is the only definitive treatment of TIO. Surgical complications due to chronic hypophosphatemia are not well recognized. CASE DESCRIPTION: The current case describe

Compound odontoma in the mandible--case study and literature review.

INTRODUCTION: Odontomas constitute a developmental defect of hard dental tissues and are classified as benign odontogenic tumours. They are composed of all dental structures and tissues: enamel, dentin, cementum and pulp. As regards histomorphological features, two types of odontomas have been differentiated: complex and compound. Odontomas represent from 4.7% to 76% of odontogenic tumours. Their aetiology has not been fully understood, yet injury, infection and genetic factors are often named among the causes. Odontomas are usually detected by chance in radiographic images taken in relation t

Histopathological Features of Secondary Squamous Cell Carcinoma Around a Dental Implant in the Mandible After Chemoradiotherapy: A Case Report With a Clinicopathological Review.

Oral squamous cell carcinoma (OSCC) around a dental implant is a rare pathologic condition. This report describes a case of recurrent OSCC surrounding a dental implant, histopathologic findings, and a literature review of this condition. A 58-year-old Japanese woman underwent chemoradiotherapy for OSCC in the right lower gingiva, resulting in a complete response. Nine years after primary chemoradiotherapy, a dental implant was placed in her atrophic mandible. Three years later, an OSCC developed around the dental implant in the right lower premolar region. Marginal mandibulectomy was performed

Osteochondroma of the right coronoid process (Jacob disease): a case report.

Oscar Jacob was the first to describe osteochondroma of the coronoid process, naming it "Jacob disease." Jacob disease rarely occurs in the oral and maxillofacial regions. The tumor usually grows progressively, leading to a mushroom-shaped enlargement of the process, and a joint-like structure is found between the coronoid process and the inner aspect of the zygomatic arch. Most of these lesions grow like a mushroom on, and do not destroy, the coronoid process. The major symptoms include restricted mouth opening and morphological changes to the zygoma. The authors present a case report on an 1

Surgical management of multiple myeloma in the mandible: a case report.

Multiple myeloma (MM), solitary bone plasmacytoma, and extramedullary plasmacytoma are different types of plasma cell neoplasia, which is a lymphoid neoplastic proliferation of ß-cells. While MM is most frequently seen in the vertebrae and long bones, 30% of MM cases are found in the jaws. This case report presents a 58-year-old man with mandibular swelling; his medical history indicated he had a fracture of the humerus 1 month earlier. Panoramic radiography revealed a radiolucent lesion in the mandibular posterior region. Biopsy was performed and the histopathological examination indicated pl

Locally infiltrative ameloblastic fibroma in a rhesus macaque (Macaca mulatta) with characterizations of its proliferating activity and biological behavior.

An 8-year-old male rhesus macaque (Macaca mulatta) presented with unilateral enlargement of the left mandible. Radiographs revealed a marked expansion of the left mandible with a multilocular radiolucent mass with abundant osteolysis. The mass was grossly firm, fleshy, and gelatinous on the cut surface. Histologically, the mass was locally infiltrative and composed of neoplastic epithelial and mesenchymal components that stained positive for cytokeratin and vimentin, respectively. Occasional densely spherical condensations of fibroblasts resembling the cap stage of odontogenesis were present i

Brown tumor of the mandible.

Brown tumor is a focal lesion differentiated from other giant-cell tumors by the presence of hyperparathyroidism. Treatment of brown tumors must be initially directed towards correcting the hyperparathyroidism. Tumor regression may occur with resolution of the hyperparathyroidism. Persistent or large destructive tumors are treated with resection or curettage.

[Mandibular reconstruction by free transfer of the fibula after cancer excision].

The authors present five cases of mandibular reconstruction using a vascularised fibula free transfer in cases of mandibulectomy for cancer. After giving a short account on the anatomy, they describe the technical details of fibula preparation and its transfer at the recipient site. The clinical observations are described. This recent technique seems to be developing, taking in account the qualities of the fibula: available bone length, possibility of performing osteotomies with preservation of the vascular supply, low morbidity at the level of the donor site, and satisfactory cosmetic and fun

Calcifying odontogenic cyst presenting as a lateral periodontal cyst--a case report.

We report a case of the calcifying odontogenic cyst occurring on the left anterior mandible of a 41-year-old male with clinical and radiographic features which made it initially to be a lateral periodontal cyst. This unusual presentation of a calcifying odontogenic cyst is discussed and its significance for the clinician is reviewed. A calcifying odontogenic cyst may present some problems for the differential diagnosis both clinically and histopathologically. The differential diagnosis of a lesion occurring in a lateral periodontal location should include the calcifying odontogenic cyst.

The use of bone imaging to detect a periapical lesion of endodontic origin.

This article reports the detection of a lesion of dental origin in the mandible of a 41-yr-old male patient during a routine bone scan used as a follow-up to cancer therapy. Unfortunately, more than 1 yr passed before the patient's signs and symptoms became severe enough for a definitive diagnosis to be made. A comprehensive dental examination is indicated for patients with positive bone scans in the area of the oral cavity. The use of bone imaging can be recommended to complement the dental examination of high risk patients with suspected metastasis or when all routine dental diagnostic measu

[The importance interdisciplinary cooperation in the prevention of radiation osteomyelitis of the jaws].

A case-history of radioosteomyelitis of the mandible, treated at the department of maxillofacial surgery which developed in a patient irradiated on account of carcinoma of the base of the mouth. Before therapy the appropriate preventive provisions were not made. The authors emphasize that before irradiation of malignant tumours of the oral cavity and pharynx a stomatologist should be consulted. Radioosteomyelitis may be, after in other respects successful antitumourous treatment, equally dangerous for the patient as the basic neoplastic disease.

Compound odontoma with displaced toothbuds: report of case with four-year follow-up.

A case of a compound odontoma of the right body of the mandible, in which the toothbuds were displaced, is described. The toothbuds are usually removed with the tumor, but in the present case they were preserved during surgical treatment. The formation and eruption of these buds were followed by serial radiographs over a period of four years.
