The effects of bone pâté on human osteoblasts cell cultures.

The aim of the present study was to evaluate the effect of bone pate on human osteoblast differentiation by measuring cell viability, alkaline phosphatase activity and expression of the transcription factors and of the major components of the extracellular matrix. Although bone paté has been used in ear surgery for many years and when placed in contact with mastoid and external auditory canal bone become viable, the cellular mechanisms that lead to its osteointegration have never been described. Bone paté taken from four patients subjected to mastoidectomy and affected by middle ear and mastoi…

The feasibility of endoscopic transcanal approach for insertion of various cochlear electrodes: a pilot study.

To determine the feasibility of inserting various types of electrode arrays using an endoscopic transcanal approach into the cochlea via the round window membrane (RWM). All the procedures were performed by the first author and started with a cortical mastoidectomy. A six o'clock vertical incision was made in the meatal skin, and a posterior tympano-meatal flap was elevated transmeatally to expose the middle ear cavity using a rigid 0° endoscope (diameter 3 mm, length 14 cm). The chorda tympani nerve (CTN) and body of the incus were exposed. The RWM was incised, and the electrodes were passed …

Recurrence rate of cholesteatoma with autoclaved incus autograft.

OBJECTIVE: To study the long-term recurrence rate of cholesteatoma with autoclaved incus autograft. STUDY DESIGN: Prospective observational study between 1994 and 2009. The incus remnants were autoclaved at 134°C for 24 minutes at 205 kPa. The mean follow-up duration was 65.5 months, with a minimum follow-up duration of 31 months and a maximum follow-up duration of 186 months. SETTING: Secondary referral center in a district general hospital. PATIENTS: Thirty-seven cases of cholesteatoma treated with canal wall down mastoidectomy with primary ossicular reconstruction with autoclaved incus a…

Incidence of acute mastoiditis in Colorado children in the pneumococcal conjugate vaccine era.

BACKGROUND: Acute otitis media is among the most common reasons young children seek medical care, with Streptococcus pneumoniae the most com … hildren's Hospital Colorado. The study included patients with documentation of acute mastoiditis or mastoidectomy and excluded those with chronic mastoiditis, chronic otitis media or cholesteatoma. RESULTS: The annual incidence of acute mastoiditis in children <2 years/100,000 population was 11.0 in 2001 before decreasing to 4.6 in 2002 and 4.5 in 2003. The in …

Laser Doppler vibrometry measurements of human cadaveric tympanic membrane vibration.

OBJECTIVE: To determine the feasibility of measuring tympanic membrane (TM) vibrations at multiple locations on the TM to differentiate normal eardrums from those with associated ossicular pathologies. DESIGN: Cadaveric human temporal bone study. SETTING: Basic science laboratory. METHODS: A mastoidectomy and facial recess approach was performed on four cadaveric temporal bones to obtain access to the ossicles without disrupting the TM. Ossicles were palpated to ensure normal mobility and an intact ossicular chain. Laser Doppler Vibrometry (LDV) measurements were then taken on all four TMs.…

Comparisons of auditory performance and speech intelligibility in children with cochlear implants placed using different approaches.

OBJECTIVE: To demonstrate whether there are differences in the outcomes of children with cochlear implants using the techniques between mastoidectomy with posterior tympanotomy (MPTA) and suprameatal approach (SMA). PATIENTS: A total of 44 congenitally deaf children who underwent cochlear implantation before the age of 5 years at the Sixth Hospital Affiliated to Shanghai Jiao tong University from January 2005 to March 2008 were included in this study. Children with severe mental retardation or with cochlear malformations were excluded. Each group had 22 patients; however, 4 patients in the SM…

Intraparotid neurofibroma: review of the literature.

OBJECTIVE: To perform a literature review of intra-parotid neurofibromas. These tumours present a challenge because of their risk of maligna … rmed, on which a cerebrospinal fluid (CSF) leak originating from the stylomastoid foramen occurred. Mastoidectomy revealed an intramastoid division of the facial nerve. The leak coming from the facial nerve was successfully controlled with anastomosis of the major division with the distal nerves. Measurement of the facial nerve caliber in facial palsy: implications for facial nerve decompression.

OBJECTIVES: The clinical effectiveness of facial nerve decompression remains controversial. To investigate this problem, we observed changes in the facial nerves of patients with and without facial palsy after this procedure. STUDY DESIGN: Retrospective case review. SETTING: Tertiary referral center. PATIENTS AND METHODS: Fifteen cases who underwent opening of the facial canal under total mastoidectomy were enrolled for this study. Among these, 7 patients with Bell's palsy (House-Brackmann grade VI) underwent facial nerve decompression. The remaining 8 patients with temporal bone tumors did…

Management of an "Ear Camp" for children in Namibia.

OBJECTIVE: In this publication, we present our experiences with managing an "Ear Camp" in northern Namibia, where the population is predominantly black. Medical coverage for ear problems is poor in this part of the country. METHODS: Within 10 days, 38 children (median age 12 years) were operated mainly for (sub) total defects of the tympanic membrane. In two cases, an open cavity had to be created because of a cholesteatoma. We performed a tympanoplasty type I in 18 cases and a tympanoplasty type III in 20 cases. Additionally, in 8 cases an antrotomy and in another 8 cases a mastoidectomy was…

An analysis of the single-stage tympanoplasty with over-underlay grafting in tympanosclerosis.

OBJECTIVES: Our objective was to analyze the results of tympanosclerosis surgery using over-underlay tympanoplasty and to find out the effect of single-stage surgery on hearing results. MATERIALS AND METHODS: Forty-two patients who were operated on for tympanosclerosis between July 1998 and February 2002 were included in the study. These were one-stage tympanoplasties, because second-stage operations and revisions were not included. Operative records and audiograms of the patients were obtained. Of the patients, 48% had bilateral tympanosclerosis. In three (7.1%) of the patients, tympanoscler
