Increasing the number of midwives is necessary but not sufficient: using global data to support the case for investment in both midwife availability and the enabling work environment in low- and middle-income countries.

BACKGROUND: Most countries are off-track to achieve global maternal and newborn health goals. Global stakeholders agree that investment in m … ical study to examine the association between the number of midwives per 10,000 population and: (i) maternal mortality, (ii) neonatal mortality, and (iii) caesarean birth rate in LMICs. We developed a composite measure of the strength of the midwifery profession, and examined its relationship with maternal mortality. RESULTS: In LMICs (especially low-income countri …

Counting indirect crisis-related deaths in the context of a low-resilience health system: the case of maternal and neonatal health during the Ebola epidemic in Sierra Leone.

Although the number of direct Ebola-related deaths from the 2013 to 2016 West African Ebola outbreak has been quantified, the number of indirect deaths, resulting from decreased utilization of routine health services, remains unknown. Such information is a key ingredient of health system resilience, essential for adequate allocation of resources to both 'crisis response activities' and 'core functions'. Taking stock of indirect deaths may also help the concept of health system resilience achieve political traction over the traditional approach of disease-specific surveillance. This study respo

Births in Finland and Estonia from 1992 to 1996: convergent differences?

OBJECTIVE: To describe the differences in childbearing, in prenatal and obstetrical practices, and in perinatal health outcome in Finland and Estonia. DESIGN: Registry study using the data from the Finnish and Estonian medical birth registries for years 1992 to 1996 (in total 324,021 and 74,297 newborns, respectively). RESULTS: In 1992 the birth rates were 51 per 1,000 women aged 15 to 49 in Finland and 48 per 1,000 in Estonia. The birth rate declined in the study period in both countries, but the decline was more rapid in Estonia (-26%) than in Finland (-6%). In the same period the rates of

Improvements in mother-child health indicators in Turkey.

Turkey has a young population as a result of high fertility and growth rates in the recent past. Thirty-five percent of the population is le … per thousand between 1978-1983 and 60.9 per thousand in 1993; it is currently 50 per thousand. The maternal mortality rate was greater than 200/100,000 in 1995. During the last five years the proportion of women receiving antenatal care has increased from 43 to 63 percent. The proportion of safe deliveries was 76 percent. Thirty-nine percent of all deliveries occur …

Effects of global health initiatives on the quality of care for maternal health services in sub-Saharan Africa: a scoping review report.

INTRODUCTION: Global health initiatives (GHIs) have played a significant role in expanding access to healthcare worldwide, particularly in maternal health. For instance, many regions in sub-Saharan Africa have seen a notable rise in the availability of skilled birth attendants. However, despite these gains in access, maternal mortality rates in low-income and middle-income countries within the African Region remain alarmingly high. Although GHIs have invested heavily in maternal health, there is limited evidence regarding their effects on the quality of maternal healthcare, an essential factor…

Maternal near miss at Kathmandu Medical College: An analysis of severe maternal morbidity at a Nepalese tertiary care facility.

BACKGROUND: The World Health Organization (WHO) 'near miss' tool has been extensively used to audit maternal morbidity in low- and middle-income countries. Analysis of the cases of 'near miss' enables a better understanding of the associated factors, identifies deficiencies in the provision of maternity services and lays a foundation for better preventive measures in the future. AIMS: To understand the epidemiology, aetiology and determine the aspects of preventability of maternal 'near miss' (MNM) at Kathmandu Medical College. MATERIALS AND METHODS: A prospective audit of MNM and maternal d

Near miss maternal morbidity.

Audit of severe maternal morbidity is a potent tool in determining standards of maternity care. This study determines the incidence of sever … maternal deaths yielding an incidence of 1.4/1000 for severe maternal morbidity and 5.4/100,000 for maternal mortality. The severe maternal morbidity to mortality ratio was 26.5:1. Massive obstetric haemorrhage requiring acute blood transfusion of > or = 5 units of packed red cells occurred in 77% of cases. This study identifies the feasibility of audit of severe ma …

Maternal deaths on arrival: A 5-year retrospective study in a health district in South Africa.

UNLABELLED: Maternal deaths on arrival remain one of the blind spots in maternal death surveillance and response programs. There are no policy and guideline addressing maternal deaths on arrival in South Africa. Main objective was to analyse maternal deaths on arrival at the health facilities in a South African district. All maternal deaths on arrival at the health facilities at the Ekurhuleni district over a period of five years and nine months (July 2014 to March 2020), were retrospectively reviewed. Analysis included maternal mortality ratio, demography, gestational age at booking, delivery…

Assessing implementation of the Maternal Death Surveillance and Response (MDSR) framework in North Aceh District, Indonesia: a qualitative study.

BACKGROUND: Effective implementation of the Maternal Death Surveillance and Response (MDSR) framework is critical for reducing maternal mortality. In Aceh Province, Indonesia, maternal mortality remains high, with rates exceeding the national average. Despite national policy adoption, limited evidence exists on how MDSR is implemented at the subnational level. This study assessed the implementation of the MDSR framework in North Aceh District by examining its five stages using eight implementation outcome variables. METHODS: A qualitative study was conducted using in-depth interviews with 60 …

Global and regional causes of maternal deaths 2009-20: a WHO systematic analysis.

BACKGROUND: Maternal mortality is not on track to meet Sustainable Development Goal (SDG) target 3.1 of a global maternal mortality ratio below 70 per 100 000 livebirths by 2030. Updated evidence on causes of death is needed to accelerate progress. METHODS: We conducted a multi-strategy systematic review to identify causes of maternal deaths occurring in 2009-20. Data sources included civil registration and vital statistics systems data from the WHO Mortality Database, reports published by Member States, and national and subnational journal articles identified via bibliographic databases. We …

Transitioning from the "Three Delays" to a focus on continuity of care: a qualitative analysis of maternal deaths in rural Pakistan and Mozambique.

BACKGROUND: The Three Delays Framework was instrumental in the reduction of maternal mortality leading up to, and during the Millennium Development Goals. However, this paper suggests the original framework might be reconsidered, now that most mothers give birth in facilities, the quality and continuity of the clinical care is of growing importance. METHODS: The paper explores the factors that contributed to maternal deaths in rural Pakistan and Mozambique, using 76 verbal autopsy narratives from the Community Level Interventions for Pre-eclampsia (CLIP) Trial. RESULTS: Qualitative analysis …

Addressing maternal healthcare needs in the counterinsurgency environment.

All pregnant women are at risk of obstetric complications, most of which occur during labor and delivery among women with no previously identified risk factors. More than 95 percent of these deaths occur in developing countries. In sub-Saharan Africa, a region of the world currently experiencing significant humanitarian crises, the lifetime risk of maternal death is one in 30 whereas the lifetime risk in developed countries is one in 2,800.1 The majority of maternal deaths from obstetric complications are due to hemorrhage, eclampsia, sepsis, or obstructed labor, each of which is treatable. Caesarean section for the dead baby--an unhappy reality.

Caesarean section for the dead baby is sometimes still needed to prevent maternal complications. The objective of the study is to critically analyse the characteristics of the mother and indications for the operation in women who delivered stillbirths following caesarean section. The study period covered a 2 years span from January, 2003 to December, 2004. During this time 121 mothers (study group) underwent caesarean section for the dead baby representing 1.41% of all caesarean section operations done in the hospital. The study group was compared to the overall caesaren section done during th
