[Indications for selective embolizations in maxillofacial pathology].

There has been remarkable progress in the therapy of maxillofacial disorders, mainly as a result of the development of Djinjian and Merland's techniques for the exploration and selective embolization of the region supplied by the external carotid artery. These methods facilitate management of bone and skin angiomas and help to resolve problems of therapeutic arteriography for orofacial tumors. Two particularly hypervascular types of tumor: nasopharyngeal angiofibroma and carotid glomus tumor, as well as the hemorrhagic complications of facial epitheliomas, are conditions in which extremely use

Long-term evaluation of complications after osteosynthesis of the jaws in patients with head and neck trauma: an analysis from a German highest level trauma center 2007-2023.

We present an analysis of 388 patients with major complications of a total of 13,392 hospitalized patients after osteosynthesis from our Maxillofacial Trauma Registry, which has 13,392 patients from 2007 to 2023. This retrospective study aimed to provide a detailed overview of all moderate to severe complications in head and neck injuries after surgery. The AO system and the modified Clavien-Dindo classification serve as the basis for our clinical procedure (flow chart presented). The statistical analysis included descriptive methods and χ2-test. In seventeen years, the complication rate was 2

Total Joint Replacement for Severe Bilateral Condylar Resorption--Gorham-Stout Syndrome.

Gorham-Stout syndrome is an extremely rare bone disorder characterized by osteolytic bony resorption. It is of unknown etiology and thought to result from localized endothelial proliferation of lymphatic vessels that cause the bony resorption. Diagnosis of Gorham-Stout can only be made after excluding other common causes of bony resorption such as those caused by infection, inflammation, malignancy, and endocrine. The clinical symptoms vary from minor pain and swelling to non-healing fractures from minor injuries. Due to the rarity of the disease, the overlap of symptoms with other diseases, a

Frontal sinus fractures: anterior skull base.

The frontal sinus is fractured in 30% of patients sustaining maxillofacial injury. The CT scan has simplified the treatment plan allowing the surgeon to be more selective in the treatment choice, surgical approach, and follow-up. Fractures may involve the anterior, posterior, or both tables of the sinus. The frontal sinus repair or surgery takes place when there is minimal displacement and the frontal ducts are in tact. Obliteration of the sinus is suggested when the ducts are injured, but there is no significant bone loss or comminution of the posterior table. More extensive injuries of the p

Comparative analysis of closed versus open reduction and internal fixation in mandibular fractures: A prospective study.

Mandibular fractures are among the most frequently encountered maxillofacial injuries. Yet, the optimal management strategy between closed and open reduction with internal fixation (ORIF) remains a topic of considerable debate, given differences in functional outcomes and complication rates. Therefore, it is of interest to evaluate and compare the clinical, functional and radiographic outcomes of closed reduction (CR) using maxillomandibular fixation and ORIF using titanium miniplates in patients with isolated mandibular fractures. Eighty patients were randomly allocated into two groups-Group …

The impact of tube and equipment choice in nasotracheal intubation: Protocol for a scoping review.

BACKGROUND: Nasotracheal intubation is commonly used in oral and maxillofacial surgeries due to its improved access to the surgical field. However, the technique can lead to complications like epistaxis and mucosal injuries. The choice of tube and other equipment may impact patient outcomes. This review aims to explore how different tubes and equipment affect patient outcomes during nasotracheal intubation. METHODS: In accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) statement, we plan to carry out a scoping revi

Congenital Loss of Permanent Teeth in a Patient With Congenital Insensitivity to Pain With Anhidrosis due to 2 Novel Mutations in the NTRK1 Gene.

Congenital insensitivity to pain with anhidrosis (CIPA), also known as hereditary sensory and autonomic neuropathy type IV, is an extremely rare autosomal recessive disorder. This study investigated the oral and craniofacial manifestations of a 7-year-old Chinese boy affected by CIPA and identified 2 novel mutations in the NTRK1 gene, and a new feature of the disorder was identified. The patient had typical features, including insensitivity to pain, anhidrosis, and mental retardation; recurrent fractures and osteoporosis also were noted. His oral and craniofacial manifestations included congen

Secondary surgical decompression of the inferior alveolar nerve after overfilling of endodontic sealer into the mandibular canal: Case report and literature review.

The authors report the case of a 43-year-old woman who underwent endodontic treatment of the right second mandibular molar with substantial extrusion of endodontic material into the mandibular canal. The patient presented at the Department of Oral and Maxillofacial Surgery with a persistent total anaesthesia of the lower lip and chin after two months. 2D panoramic view and 3D CT-scan examination highlighted the overfilling into the mandibular canal with a more than 50% stenosis of the canal and a consequently significant compression of the dental pedicle. A surgical decompression of the inferi

Outcome and complications after treatment of facial fractures at different times in polytrauma patients.

Finding the correct point of time for operative treatment of facial fractures in multiply injured people remains one of the most important challenges in modern emergency medicine. Findings relating to pathophysiological mechanisms after severe trauma argue against the early operative treatment of non-life-threatening injuries. Our retrospective analysis investigated the effects on complications and outcome of different time points for operative treatment of maxillofacial fractures in multiply injured patients. Over a period of 10 years (2003-2012) we could identify 1543 patients, of whom 553 h

Combined orthognathic and facial aesthetic surgery with case reports.

There are multiple hard- and soft-tissue considerations in facial surgery. The expertise of the oral and maxillofacial surgeon in treating patients with multiple facial injuries has shown time and time again that hard- and soft-tissue reconstruction can be performed either as simultaneous or as delayed procedures. The benefits of a total facial analysis approach to facial surgery are based upon aesthetic, functional, rejuvenative, and self-esteem issues. Proper diagnosis coupled with a comprehensive treatment plan utilizing a team approach provides for both simultaneous and delayed procedures

Orbital roof fractures in the pediatric population.

Twenty-three patients aged 3.3 +/- 1.6 years (mean +/- SD) presented between January of 1984 and September of 1987 with fronto-orbital trauma resulting in fractures of one (N = 20) or both (N = 3) orbital roofs. All patients had computed tomography (CT) with axial and coronal sections that revealed three fracture patterns of the orbital roof (nondisplaced, superiorly displaced, and inferiorly displaced fractures). Orbital dystopia was exhibited in 35 percent (N = 8) of the patients. Exophthalmos was noted in 61 percent (N = 14) of the patients. Only 30 percent of the patients (N = 7) sustained

Surgical management of Stenson's duct injury by using double J stent urethral catheter.

BACKGROUND: Parotid duct or gland injury can be caused by assault with a knife, bottle, electrical-saw, road traffic accident, or rarely gunshot and fractures of the facial skeleton. The injury can be in the form of laceration, ductal exposure, total cutting, or crushing of the duct. These conditions are difficult to diagnose because of complex anatomy and variable forms of the injury. A successful management of parotid duct injuries depends on early diagnosis and appropriate intervention; improper surgery may lead to complications such as sialocele or salivary fistula CASE REPORT: A 27-years

Dural Exposure as a Rare Complication in Surgical Management of Temporalis Myositis Ossificans.

Myositis ossificans is a rare condition characterized by heterotopic bone formation in soft tissues, often following trauma. Although it can occur in the maxillofacial region, cases involving the temporalis muscle are infrequent. We report a case of a 33-year-old man who developed severe limitation of mouth opening due to myositis ossificans following closed trauma to the temporal region. Surgical excision was performed. The excision resulted in dural exposure and a small tear, requiring repair with a temporalis fascial graft. This case highlights a rare complication of myositis ossificans in
