Addressing Spiritual and Religious Influences in Care Delivery.

PURPOSE: The impact of spiritual and religious factors on health and care delivery has been identified in case management field research. The purpose of this article is to examine how case managers, taking a holistic, patient-centered approach, are required under professional and ethical standards to address the spiritual and religious influences that may impact the individual's health, care choices, and care delivery. PRIMARY PRACTICE SETTINGS: Case managers across health or human services must be able to identify and address the spiritual and/or religious factors that may influence a care p

Physiotherapy students' perceptions of managing patients with mental illness: A qualitative exploration at a South African university.

BACKGROUND: Mental illness (MI) is a leading cause of global ill health and disability, with nearly half of the population affected by mental health (MH) conditions at some point in their lives. Consequently, all healthcare professionals, including physiotherapy students, will inevitably encounter patients with MI in clinical practice. However, their preparedness and ability to manage these patients remain unclear. OBJECTIVES: To explore physiotherapy students' perceptions of their preparedness and ability to manage patients with co-existing (undiagnosed or known) MH conditions alongside a pr

Trajectories of racial discussion and parental depression in Asian American families.

ABSTRACTRacial-ethnic socialization (RES) is a critical parenting practice among Asian American parents, yet little is known about its impact on parental mental health. Drawing on Lazarus and Folkman's (1984) transactional model of stress and coping, this longitudinal study examined trajectories of three RES domains (awareness of discrimination, avoidance of outgroups, minimization of race) and their associations with depressive symptoms among 209 Asian American parents (mean age 41.68; 79.43% female) over 12 months. Using growth mixture modeling, three distinct trajectory classes emerged for

Concurrent psychiatric, somatic, and social vulnerabilities in patients at increased cardiovascular risk in the Netherlands.

OBJECTIVE: To evaluate clinical, psychiatric, demographic, and socioeconomic vulnerability in a Dutch primary care setting. METHODS: We included 34,392 patients aged 40-70 years who were registered for cardiovascular risk management in general practice before 2018. Latent class analysis was performed using 17 indicators across seven themes: somatic morbidity, adverse lifestyle, psychiatric, age, sex, ethnicity, and socioeconomic factors. We compared five-year risk of major adverse cardiovascular events using Fine-Gray competing risk models and assessed annual medical expenditures (2018-2022)

It Knows My Emotions Better Than I Do: Digital Phenotyping and the Contest Over Symptom Interpretation.

Digital phenotyping is increasingly presented as a means of detecting mental distress through smartphones, wearables, and other sensor-rich personal devices. In this article, we argue that the central philosophical and clinical issue raised by digital phenotyping extends beyond predictive accuracy to interpretive authority, which concerns who gets to name what a feeling, behavioral pattern, or physiological fluctuation means. Psychiatry has always involved uncertainty, proxy reasoning, and the negotiation of testimony, but digital phenotyping intensifies these dynamics by translating lived sta

The experiences of autistic adults who were previously diagnosed with borderline or emotionally unstable personality disorder: A phenomenological study.

Autistic people face many barriers to receiving an autism diagnosis. Often, they may be misdiagnosed with borderline personality disorder instead. For our study, we interviewed 10 autistic adults who had previously been diagnosed with borderline personality disorder. This helped us to better understand their experiences. They explained how borderline personality disorder is quite stigmatised and may suggest that people are to blame for their differences in behaviour. They found the treatments they had to try for borderline personality disorder to be harmful. For example, these treatments promo

Mutualistic processes in the development of psychopathology: The special case of borderline personality disorder.

Borderline personality disorder (BPD) is a serious mental illness characterized by instability in affective, cognitive, and interpersonal domains. BPD co-occurs with several mental disorders and has robust, positive associations with the general factors of psychopathology (p-factor) and personality disorders (g-PD). Consequently, some researchers have purported BPD to be a marker of p, such that the core features of BPD reflect a generalized liability to psychopathology. This assertion has largely stemmed from cross-sectional evidence and no research to date has explicated the developmental re

Snore-associated sleep fragmentation in infancy: mental development effects and contribution of secondhand cigarette smoke exposure.

OBJECTIVE: The link between sleep-disordered breathing and neurocognitive functioning in preschool and school-aged children now has been established clearly. Within these age groups, isolated studies have examined the potential effect of snoring without gas exchange abnormalities on aspects of cognitive competence. The goal of the study was to test the potential association between snoring and decrements in developmental performance among infants. METHODS: Thirty-five healthy community infants (8.2 +/- 0.4 months) were administered the Bayley Scales of Infant Development, including the Mental

Preliminary findings from a formative evaluation of the Indigenous peer support specialist train-the-trainer manual: a culturally grounded approach to recovery in American Indian and Alaska Native communities.

INTRODUCTION: American Indians and Alaska Natives (AIANs) are rich in culture, heritage, language, and resilience. Chronic underfunding, limited healthcare infrastructure, and barriers to quality health services contribute to some of the most severe health disparities in mental health and substance use disorders among AIAN populations. METHODS: This formative evaluation aims to document the process and early implementation feedback on an Indigenous Peer Support Specialist Train-the-Trainer Manual (IPSSTTM) designed for use across Indigenous, community, clinical, and university settings. Diagnostic accuracy of paroxysmal spells: Clinical history versus observation.

Our goal was to evaluate how accurate neurologists are at differentiating between different paroxysmal events based on clinical history versus observation of the spell in question. Forty-seven neurologists reviewed 12 clinical histories and videos of recorded events of patients admitted in the Epilepsy Monitoring Unit (EMU). They were asked to diagnose events as epileptic seizures, non-epileptic behavioral spells (NEBS), or other physiologic events as well as rate their confidence in their diagnosis. The median diagnostic accuracy for all paroxysmal events was 67% for clinical history and 75%

A comparison of mental state examination documentation by junior clinicians in electronic health records before and after the introduction of a semi-structured assessment template (OPCRIT+).

OBJECTIVES: The mental state examination (MSE) provides crucial information for healthcare professionals in the assessment and treatment of psychiatric patients as well as potentially providing valuable data for mental health researchers accessing electronic health records (EHRs). We wished to establish if improvements could be achieved in the documenting of MSEs by junior doctors within a large United Kingdom mental health trust following the introduction of an EHR based semi-structured MSE assessment template (OPCRIT+). METHODS: First, three consultant psychiatrists using a modified version

[Münchausen syndrome by proxy in a forensic psychiatric evaluation--the description of a case and ethical controversy].

AIM: Presentation of the difficulties in diagnosing Münchausen syndrome by proxy. METHOD: Comparison of four different conclusions in forensic psychiatric opinions issued in one case, due to the need of answering to the questions of the court, in what periods and why was the deterioration of the child's health, in connection with an allegation that the mother has acted to his detriment. In the first medical-legal opinion the treating physician, while being the head of the ward and the person informing the police of the action against a child by the mother, recognised Münchausen syndrome by pr

Learning during simulation training is prone to retroactive interference.

CONTEXT: Retroactive interference occurs when newly acquired information inhibits recall of previously learned information. This has been shown to influence recall of sounds, tastes and word associations, and is typically seen when learners receive training on one area of content and are then exposed to new content before being evaluated on the original content. Thus far, retroactive interference has received little attention in medical education and has not been studied during simulation training. Our objective was to evaluate whether retroactive interference occurs during simulation training

Development and evaluation of the GEMS (gait, eyesight, mental state, sedation) tool as an aid to predict outcome after hospitalization.

AIM: Discharge planning can be a lengthy process. Prediction of a patient's rehabilitation potential and likely discharge destination, early on in their admission, could be a useful guide for medics, therapists, patients and their carers. Reliable prediction could be used to improve efficiency of discharge planning. The aim of the present study was to identify factors linked to discharge to a residential or nursing home placement and to develop a tool to guide rehabilitation requirements. METHODS: This was a three-phase prospective observational study with blinded end-point evaluation in two

Capacity for decision-making in Alzheimer's disease: selfhood, positioning and semiotic people.

The aim of this article is to explore some of the ethical issues surrounding the assessment and determination of capacity of people with dementia in light of their meaning-making ability as shown through discourse. Discourse analysis, two illustrative cases, social construction theory and positioning are used to highlight some of the ethical dilemmas involved in basing a determination of capacity on the diagnosis of dementia and neuropsychological tests of cognitive function. Although neuropsychological tests have their place in assessing some aspects of cognitive function, aspects cognition s

Bariatric surgery for severely overweight adolescents: concerns and recommendations.

As the prevalence of obesity and obesity-related disease among adolescents in the United States continues to increase, physicians are increasingly faced with the dilemma of determining the best treatment strategies for affected patients. This report offers an approach for the evaluation of adolescent patients' candidacy for bariatric surgery. In addition to anthropometric measurements and comorbidity assessments, a number of unique factors must be critically assessed among overweight youths. In an effort to reduce the risk of adverse medical and psychosocial outcomes and increase compliance an
