Tympanostomy tubes and early post-operative otorrhea.

Otorrhea is a recognized complication of tympanostomy tube insertion. This may be caused by external contamination or by the pre-existing conditions in the middle ear cleft. Some surgeons try to reduce the risk of surgical contamination by using a non-touch technique, never allowing the gloved hand to come into contact with the tympanostomy tube. A prospective study was carried out on 66 children to evaluate the efficacy of the non-touch technique. One tympanostomy tube was inserted by the non-touch method with the contralateral ear acting as a touched control in each patient. Postoperative ot

[Synopsis of results with 1,300 children with language developmental delay from the etiopathogenetic, audiologic and speech pathology viewpoint].

This study presents the results of 1,305 children with speech disorders. The examination included ENT inspection, audiometry, speech evaluations and if required, psychological tests. In 48% we found hearing loss, most frequently in the age-group up to 4 years. In 95% hearing loss fluctuated about 20 dB, caused by malfunction of the tube or by adenoids, not only in autumn and winter, but throughout the year. In this case, the total hearing input of these children is reduced. In only 5% we found cochlear hearing loss. In hearing-impaired children the speech development was significantly depresse

The use of antibiotic/steroid ear drops to reduce post-operative otorrhoea and blockage of ventilation tubes. A prospective study.

This prospective randomized study investigates whether the use of antibiotic/steroid ear drops (Betnesol-N) is effective in reducing early post-operative otorrhoea and blockage of ventilation tubes (VTs). The study included 162 children who had bilateral VT insertion and used Betnesol-N ear drops in one ear only for three days after surgery, the other ear was left as control. These children were reviewed two weeks later and their ears were examined for VT patency, presence of blood or of mucopus. Statistical analysis of our results showed that the use of Betnesol-N ear drops has significantly

Hearing loss in Hunter's syndrome--mucopolysaccharidosis II.

Hunter's syndrome [mucopolysaccharidosis II (MPS II)] is a genetic, metabolic disease of excessive mucopolysaccharide storage leading to mental and skeletal abnormalities, distinctive facial features, and increased incidence of hearing loss. However, the hearing impairment in MPS II has not been well described in the literature. This paper examines the auditory aspects of MPS II by reviewing the literature and by presenting two affected brothers. Each subject had mixed hearing disorders, fluctuating between moderate and severe degrees associated with recurrent middle ear effusions. Hearing los

Role of laryngopharyngeal reflux in the etiology of otitis media with effusion in children.

OBJECTIVES: To analyze the relationship between laryngopharyngeal reflux (LPR) represented by pepsin and trypsin, and pathogenesis of otitis media with effusion (OME) in children with adenoid hypertrophy. METHODS: Children with OME who required adenoidectomy and tympanostomy/tympanostomy tubes placement were enrolled in OME group, whereas children with adenoid hypertrophy (AH) who only required adenoidectomy were enrolled in AH group, respectively. Pepsin and trypsin concentrations were evaluated by enzyme-linked immunosorbent assay in adenoid tissue from the OME and AH groups. Pepsin and try

Analysis of Sociodemographics in Follow-Up Post-Tympanostomy Tube Placement.

OBJECTIVE: To determine if disparities exist in patient follow-up post-bilateral myringotomy with tympanostomy tube placement. INTRODUCTION: Bilateral myringotomy with tympanostomy tube placement (BMT) is the most common surgery performed in children in the U.S. The current literature suggests there are no demographic differences in treatment of otitis media, however these studies have limited data on postoperative follow-up. This study aims to assess socioeconomic and other factors that affect follow-up after BMT. METHODS: A retrospective chart review of pediatric patients seen at a tertiar

Tympanostomy tube placement in awake, unrestrained pediatric patients: A prospective, multicenter study.

OBJECTIVES: Tympanostomy tube (tube) placement is the most common pediatric otolaryngologic surgery in the United States. Most surgeries are performed in an operating-room setting under general anesthesia due to the lack of tolerable and reliable local anesthesia methods suitable for pediatric patients, and concerns regarding myringotomy procedures in a mobile child. This study evaluated the safety and efficacy of an iontophoresis system (IPS) to achieve local anesthesia in combination with a tube delivery system (TDS) for tube placement in pediatric patients in an office setting. METHODS: A

Adenoidectomy in young children and serum IgG antibodies to pneumococcal surface protein A and choline binding protein A.

OBJECTIVE: We have previously reported that surgical removal of the nasopharyngeal adenoid in young children resulted in increased risk of nasopharyngeal colonization by pneumococci. We now investigated whether adenoidectomy influences the development of serum IgG antibodies to pneumococcal choline-binding protein A (CbpA) and pneumococcal surface protein A (PspA). METHODS: Altogether 217 children aged 12-48 months who had recurrent or persistent otitis media were randomized to undergo or not to undergo adenoidectomy. All the children underwent insertion of tympanostomy tubes. 166 children we
