Urinary monoamine metabolite excretion in disorders of movement. Effects of amantadine and levodopa.

We have studied the urinary excretion of 1,4-methylhistamine (1,4-MeHm), 5-hydroxyindole-3-acetic acid (5-HIAA) and homovanillic acid (HVA) in patients with Parkinson's disease, choreiform movements and essential tremor. The effect of amantadine on urinary excretion has been measured in each group of patients as well as the effect of levodopa in patients with Parkinson's disease. In patients with Parkinson's disease, excretion of 1,4-MeHm and HVA was significantly lower than in controls. Patients with choreiform movements had a reduced excretion of HVA but trends toward low levels of 1,4-MeHm

Exposure and Response Prevention for Comorbid Symptoms of Obsessive-Compulsive Disorder and Psychogenic Movement Disorder.

Psychogenic movement disorders (PMDs) are common, associated with significant psychosocial impairments, and frequently comorbid with other physical and psychiatric conditions. However, treatment of PMDs has been largely unsuccessful. Recently, treatments focused on conditions comorbid with PMDs have been hypothesized as a possible improvement to PMD-focused treatments. This case report describes a patient initially diagnosed with a PMD and referred for psychotherapy after appointments with neurologic and psychiatric providers. Upon referral to psychotherapy, the patient received a diagnosis of…

Copy number variants from 4800 exomes contribute to ~7% of genetic diagnoses in movement disorders, muscle disorders and neuropathies.

Various groups of neurological disorders, including movement disorders and neuromuscular diseases, are clinically and genetically heterogeneous. Diagnostic panel-based exome sequencing is a routine test for these disorders. Despite the success rates of exome sequencing, it results in the detection of causative sequence variants in 'only' 25-30% of cases. Copy number variants (CNVs), i.e. deletion or duplications, explain 10-20% of individuals with multisystemic phenotypes, such as co-existing intellectual disability, but may also have a role in disorders affecting a single system (organ), like…

Impulse control disorder and rapid eye movement sleep behavior disorder in Parkinson's disease.

INTRODUCTION: The relationship between ICD and RBD is still not yet understood and the results from the current literature are contradictory in PD. We aimed to explore the association between rapid eye movement (REM) sleep behavior disorder (RBD) and impulse control disorder in Parkinson's disease. METHODS: Ninety-eight non-demented patients with Parkinson's disease underwent one night of video-polysomnography recording. The diagnosis of RBD was established according to clinical and polysomnographic criteria. Impulse control disorders were determined by a gold standard, semi-structured diagno

Abnormal H-reflexes in periodic limb movement disorder; impact on understanding the pathophysiology of the disorder.

OBJECTIVE: To get more insight in the pathophysiological basis of periodic limb movement disorder (PLMD) with or without restless legs syndrome (RLS), we investigated whether these patients have spontaneous changes in H-reflexes or show altered reflex patterns after (external) inhibition or excitation of the relevant spinal segment. METHODS: The ratio of the peak-to-peak values of the maximal soleus H-reflex and the maximal direct muscle potential (H/M ratio), H-reflex recruitment curves, vibratory inhibition and recovery curves of the soleus H-reflex in double stimulus experiments were measu

A bibliometric analysis of rapid-eye-movement sleep behavior disorder.

Rapid-eye-movement sleep behavior disorder(RBD) is a parasomnia characterized by the loss of normal muscle atonia during rapid eye movement (REM) sleep, leading individuals to physically act out vivid, often intense or violent dreams, which may result in self-injury or harm to others. In recent years, RBD has become a prominent research focus. The bibliometrics can clearly and intuitively display the research profile, relationships, and clustering within the field. This study examines the development of RBD research using bibliometric analysis. As of 18 January 2026, articles on RBD in the Web

What eye-movements tell us about Disorders of Consciousness?

In everyday life, eye movements (including fixation, smooth pursuit and saccades) are essential for interacting with the visual environment. In patients with Disorders of Consciousness (DoC), visual fixation and pursuit are considered among the earliest clinical signs of awareness recovery. However, the traditional distinction between reflexive and voluntary eye movements may be insufficient to interpret these behaviours, as movements that appear automatic can still recruit cortical networks. This review re-examines eye movements in DoC through a neuroanatomical and translational lens. We char

Mark Hallett: scientist and humanitarian in modern movement disorder neurology.

Mark Hallett, MD (1943-2025) was a highly influential neurologist in the modern history of movement disorders. Over several decades, his work transformed the field from a predominantly descriptive clinical specialty into a neuroscientific discipline. Through pioneering investigations in neurophysiology, cortical excitability, dystonia, tremors, myoclonus, functional movement disorders, and transcranial magnetic stimulation, Hallett helped establish conceptual frameworks that continue to shape contemporary research and clinical practice. Beyond his scientific contributions, he is recognized as …

Should Paratonia Be Considered a Movement Disorder? Implications for Clinical Management.

Paratonia is an involuntary and variable resistance to passive movement that emerges with cognitive decline in dementias of different etiologies, with prevalence approaching 90% to 100% in end-stage disease. It is common in Parkinson disease and the atypical parkinsonisms. The involuntary resistance is frequently interpreted as volitional refusal of care and may be managed with psychotropic medication. We propose that paratonia meets the criterion by which a movement disorder is defined, with a reproducible and distinct clinical phenomenology that is operationalized in a validated bedside inst

The Rehabilitation Nurse's Role in the Care of a Stroke Patient With a Movement Disorder.

Hyperkinetic movement disorders are defined as excessive, abnormal involuntary movements such as chorea, ballism, and athetosis ( Mehanna & Jankovic, 2013 ). Hemiballismus/hemichorea is the most common movement disorder to occur after a stroke ( Bansil et al., 2012 ; Tater & Pandey, 2021 ). These movements are described as vigorous, irregular movements on one side of the body ( Mehanna & Jankovic, 2013 ). While most poststroke movement disorders are temporary, there is a chance that they may persist long term, which can significantly affect that person's quality of life. When symptoms are iden…

Physiological and introspective antecedents of tics and movements in adults with tic disorders.

OBJECTIVE: To investigate the subjective phenomenon and the neural underpinnings of tics compared with voluntary movements in patients with tic disorders. METHODS: We recorded electroencephalographic and electromyographic data while subjects completed a Libet clock paradigm. Patients and healthy volunteers reported the times of W (willing to move) and M (movement occurrence) while performing voluntary movements. This was repeated only for the patients for the tics. RESULTS: In the patients, W and M times preceding voluntary movements and tics did not significantly differ from voluntary movem

A Case Report of Baclofen- and Clozapine-Induced Dyskinesia: A Movement Disorder.

Movement disorder is a broad term comprising multiple disorders which result in either an excess or a paucity of voluntary and involuntary movements. There are numerous pieces of literature on drug-induced dyskinesia, although the exact mechanism underlying this phenomenon is yet to be understood. Drug-induced movement disorder is a complex and often neglected clinical presentation. There are various interactions of drugs with the dopaminergic, GABAergic, and serotonergic pathways in the body that seem to be the foundation, leading to these movement disorders. Further research and clinical tri…

Sleep-related head jerks: toward a new movement disorder.

STUDY OBJECTIVES: Sleep-related head jerks (SRHJ) are often considered as a physiological motor phenomenon, occurring mainly during rapid eye movement (REM) sleep. Their clinical relevance and links with other sleep parameters are unclear. We characterized the clinical and polysomnographic features of patients with excessive SRHJ and compare them with healthy controls and patients with isolated REM sleep behavior disorder (iRBD). METHODS: A total of 30 patients (19 males, 27.5 y.o., 16.0-51.0) with a REM-HJ index >30/h were identified over a period of 5 years. All had a video-polysomnographic

Research progress in rapid eye movement sleep behavior disorder.

Rapid eye movement (REM) sleep behavior disorder (RBD) is characterized by abnormal dream acting behavior such as vocalization and twitching related to dream content during REM sleep. The diagnosis requires polysomnography demonstrating a loss of normal skeletal muscle atonia during REM sleep. Both idiopathic RBD and secondary RBD are highly related to synucleinopathy including Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy. Almost all idiopathic RBD patients will develop synucleinopathy after a few years. Therefore, RBD may be an early marker in the progression of
