Organ-targeted biomarkers of sepsis: A systematic review reveals the value of inflammation and lipid metabolic dysregulation.

Sepsis, a life-threatening systemic inflammatory response to infection, progresses to multiorgan dysfunction involving the heart, lungs, kidneys, brain, and liver. Its heterogeneous pathogenesis involves intertwined pathways such as inflammationmetabolism imbalance, immune dysregulation, mitochondrial dysfunction, and programmed cell death. While traditional biomarkers such as CRP and PCT lack organ-specific sensitivity, emerging multiomics technologies enable systematic network analysis beyond single-molecule detection. Current organ-specific biomarkers focus on four categories: ① inflammator…

Genetic regulation of ARID3B confers cleft lip with/without cleft palate susceptibility through LLPS-mediated transcriptional program.

Genome-wide association studies have identified numerous loci associated with nonsyndromic cleft lip with/without cleft palate (nsCL/P). However, the causal genes within these loci remain to be systematically investigated. Through a multiomics screening strategy, we identified 20 candidate genes, with ARID3B emerging as the most significant risk gene for nsCL/P. Focusing on the ARID3B locus, we found a casual variant at rs1821848 that diminished the binding affinity of NR2C2, resulting in the upregulation of ARID3B. Notably, we observed the formation of ARID3B granules through liquid-liquid ph…

Why Has Biomarker-Guided Fluid Resuscitation for Sepsis Not Been Implemented in Clinical Practice?

Sepsis is a dysregulated, potentially fatal host response to infection, characterized by heterogeneity in clinical presentation and organ failure mechanisms. Early hemodynamic resuscitation and antibiotics are crucial treatments. Current guidelines recommend a one-size-fits-all approach of 30 mL/kg fluids, which may worsen vascular leakage and organ dysfunction in some patients. Personalized strategies using biomarkers and dynamic fluid responsiveness assessments offer a more tailored approach, potentially preventing fluid overload while ensuring perfusion. A recent multiomics analysis identif…

Lubiprostone in chronic kidney disease: Insights into mitochondrial function and polyamines from a randomized phase 2 clinical trial.

Chronic kidney disease (CKD) is a life-threatening condition, and constipation is a progressive risk factor. We evaluated changes in uremic … am group. Mild-to-moderate gastrointestinal events occurred in the placebo and 16-microgram groups. Multiomics analysis revealed that lubiprostone modulated the gut microbial agmatine pathway and increased spermidine levels, thereby improving renal mitochondrial function. Lubiprostone is a previously unknown and safe therapeutic option to mitigate renal decl …

Spaceflight alters host-gut microbiota interactions.

The ISS rodent habitat has provided crucial insights into the impact of spaceflight on mammals, inducing symptoms characteristic of liver disease, insulin resistance, osteopenia, and myopathy. Although these physiological responses can involve the microbiome on Earth, host-microbiota interactions during spaceflight are still being elucidated. We explore murine gut microbiota and host gene expression in the colon and liver after 29 and 56 days of spaceflight using multiomics. Metagenomics revealed significant changes in 44 microbiome species, including relative reductions in bile acid and butyr…

ARDaC Common Data Model Facilitates Data Dissemination and Enables Data Commons for Modern Clinical Studies.

Modern clinical studies collect longitudinal and multimodal data about participants, treatments and responses, biospecimens, and molecular and multiomics data. Such rich and complex data requires new common data models (CDM) to support data dissemination and research collaboration. We have developed the ARDaC CDM for the Alcoholic Hepatitis Network (AlcHepNet) Research Data Commons (ARDaC) to support clinical studies and translational research in the national AlcHepNet consortium. The ARDaC CDM bridges the gap between the data models used by the AlcHepNet electronic data capture platform (REDC…

Hepatocyte FBXW7-dependent activity of nutrient-sensing nuclear receptors controls systemic energy homeostasis and NASH progression in male mice.

Nonalcoholic steatohepatitis (NASH) is epidemiologically associated with obesity and diabetes and can lead to liver cirrhosis and hepatocell … homeostasis, and NASH pathogenesis with attenuated FBXW7 expression as a feature of advanced NASH. Multiomics and pharmacological intervention showed that FBXW7 loss-of-function in hepatocytes disrupts a metabolic transcriptional axis conjointly controlled by the nutrient-sensing nuclear receptors ERRα and PPARα, resulting in suppression of fatty acid oxida …

Multidimensional definition of the interferonopathy of Down syndrome and its response to JAK inhibition.

Individuals with Down syndrome (DS) display chronic hyperactivation of interferon signaling. However, the clinical impacts of interferon hyperactivity in DS are ill-defined. Here, we describe a multiomics investigation of interferon signaling in hundreds of individuals with DS. Using interferon scores derived from the whole blood transcriptome, we defined the proteomic, immune, metabolic, and clinical features associated with interferon hyperactivity in DS. Interferon hyperactivity associates with a distinct proinflammatory phenotype and dysregulation of major growth signaling and morphogenic …

GRHL2 and AP2a coordinate early surface ectoderm lineage commitment during development.

Ectodermal dysplasias including skin abnormalities and cleft lip/palate result from improper surface ectoderm (SE) patterning. However, the connection between SE gene regulatory networks and disease remains poorly understood. Here, we dissect human SE differentiation with multiomics and establish GRHL2 as a key mediator of early SE commitment, which acts by skewing cell fate away from the neural lineage. GRHL2 and master SE regulator AP2a balance early cell fate output, with GRHL2 facilitating AP2a binding to SE loci. In turn, AP2a restricts GRHL2 DNA binding away from de novo chromatin contac…

Spatially confined sub-tumor microenvironments in pancreatic cancer.

Intratumoral heterogeneity is a critical frontier in understanding how the tumor microenvironment (TME) propels malignant progression. Here, we deconvolute the human pancreatic TME through large-scale integration of histology-guided regional multiOMICs with clinical data and patient-derived preclinical models. We discover "subTMEs," histologically definable tissue states anchored in fibroblast plasticity, with regional relationships to tumor immunity, subtypes, differentiation, and treatment response. "Reactive" subTMEs rich in complex but functionally coordinated fibroblast communities were i…

Dedifferentiation and neuronal repression define familial Alzheimer's disease.

Identifying the systems-level mechanisms that lead to Alzheimer's disease, an unmet need, is an essential step toward the development of therapeutics. In this work, we report that the key disease-causative mechanisms, including dedifferentiation and repression of neuronal identity, are triggered by changes in chromatin topology. Here, we generated human induced pluripotent stem cell (hiPSC)-derived neurons from donor patients with early-onset familial Alzheimer's disease (EOFAD) and used a multiomics approach to mechanistically characterize the modulation of disease-associated gene regulatory …

AI-driven multi-omics deciphering of extrachromosomal circular DNA rescues hidden targets in colorectal cancer.

BACKGROUND: Extrachromosomal circular DNA (eccDNA) is an emerging tumor biomarker notable for its tumor-specific amplification and contribution to genomic heterogeneity. However, the substantial heterogeneity of eccDNAs poses a significant challenge for characterization. Current identification tools rely predominantly on single data types, overlooking the integrative potential of multi-omics layers and often discarding sparse but biologically meaningful signals. METHODS: To address this, we developed eccDNAOmix, an AI-empowered framework that integrates a specialized eccDNA sequencing pipelin

Integrated multi-omics analysis reveals key molecular mechanisms underlying fibroproliferation and immune dysregulation in keloids.

Keloids are benign fibroproliferative skin lesions characterized by excessive collagen deposition and growth beyond the original wound margins. However, the cellular heterogeneity and molecular mechanisms underlying keloid pathogenesis remain incompletely understood. Publicly available single-cell RNA sequencing (scRNA-seq) data from keloid tissues of eight patients were obtained from the NCBI Gene Expression Omnibus (GEO) database. High-dimensional weighted gene co-expression network analysis (hdWGCNA) was performed to identify disease-associated gene modules. Mendelian randomization (MR) ana

Multimodal foundation models in colorectal cancer: from prediction to trustworthy clinical insight.

Colorectal cancer (CRC) is characterized by profound, multi-layered heterogeneity that limits the precision of conventional single-modality clinical tools. The emergence of multimodal foundation models (MFMs) represents a conceptual paradigm shift, moving beyond static biomarkers to capture the dynamic and evolving nature of CRC. MFMs integrate histopathology, radiology, multi-omics data (including the critical regulatory layer of epigenomics), and clinical variables into shared high-dimensional representational spaces. This integration enables improved prognostication, refined molecular subty
