Chronic neck pain of traumatic and non-traumatic origin: a population-based study.

Chronic neck pain is a common cause of disability. The effect of neck trauma on the development of chronic neck pain has been debated. In this population-based study, 8,356 persons (25-79 years) were randomly selected from a geographically well-defined area in northern Sweden. 6000 answered a self-administered questionnaire. We evaluated the data from all participants in the age range 25-64 years, a total of 4,392 persons. 18% reported chronic neck pain, defined as continuous pain of more than 6 months duration. 5% had a history of neck trauma and 13% had no such history. Of all patients with

Beyond Occam's Razor: Concurrent Non-functional Pituitary Macroadenoma and Metastatic Hepatocellular Carcinoma in a Patient With Functional Cure of Hepatitis B.

Occam's razor, the principle of parsimony, is frequently employed in medicine to derive a single diagnosis from a patient's myriad symptoms. Conversely, Hickam's dictum, which embraces the principle of plenitude by considering multiple diagnoses for a patient's presentation, is often underutilized or not as widely recognized as Occam's razor. The application of Hickam's dictum is particularly crucial when evaluating nonspecific symptoms such as fatigue, which can manifest in various diseases. This report describes the case of a 72-year-old man with a history of functional cure for hepatitis B

Recent research progress on metabolic syndrome and risk of Parkinson's disease.

Parkinson's disease (PD) is one of the most widespread neurodegenerative diseases. PD is associated with progressive loss of substantia nigra dopaminergic neurons, including various motor symptoms (e.g., bradykinesia, rigidity, and resting tremor), as well as non-motor symptoms (e.g., cognitive impairment, constipation, fatigue, sleep disturbance, and depression). PD involves multiple biological processes, including mitochondrial or lysosomal dysfunction, oxidative stress, insulin resistance, and neuroinflammation. Metabolic syndrome (MetS), a collection of numerous connected cerebral cardiova

Calcium Signaling in Liver Injury and Regeneration.

The liver fulfills central roles in metabolic control and detoxification and, as such, is continuously exposed to a plethora of insults. Importantly, the liver has a unique ability to regenerate and can completely recoup from most acute, non-iterative insults. However, multiple conditions, including viral hepatitis, non-alcoholic fatty liver disease (NAFLD), long-term alcohol abuse and chronic use of certain medications, can cause persistent injury in which the regenerative capacity eventually becomes dysfunctional, resulting in hepatic scaring and cirrhosis. Calcium is a versatile secondary m

Periodontal disease and type 2 diabetes mellitus: is the HMGB1-RAGE axis the missing link?

Periodontitis is a major chronic inflammatory disease associated with increased production of numerous proinflammatory cytokines, which leads to the destruction of the periodontal tissue and ultimately loss of teeth. Periodontitis has powerful and multiple influences on the occurrence and severity of systemic conditions and diseases, such as diabetes mellitus, cardiovascular disease and respiratory disease. Meanwhile, diabetes is associated with increased prevalence, severity and progression of periodontal disease. There is also abundant evidence showing that diabetes plays important etiologic

Diversity of autoantibodies in primary biliary cirrhosis and chronic active hepatitis.

The presence of autoantibodies in the serum of 110 patients with primary biliary cirrhosis (PBC), 50 with HBsAg negative chronic active hepatitis (HBsAg- CAH) and 30 with HBsAg positive chronic active hepatitis (HBsAg+ CAH) was assessed using two methods: indirect immunofluorescence on cells grown in tissue culture (HEp-2 cell line) or standard mouse tissue sections, and counter immunoelectrophoresis (CIE) with soluble tissue extracts. Anti-nuclear antibodies (ANA) were found in 38% of sera from patients with PBC using HEp-2 cells compared with 10% using mouse tissue. A variety of staining pat

[ACUTE MESENTERIC ISCHEMIA].

A 61-year-old man with a complex cardiovascular history and chronic kidney disease was hospitalized with an anterior STEMI and found on catheterization to have severe occlusions in multiple coronary arteries. He underwent PCI followed by urgent coronary artery bypass surgery. On the day after the surgery, he developed acute abdominal pain, and a CTA revealed thrombosis of the superior mesenteric artery (SMA) with ischemia of the small intestine - a condition that was treated surgically with bowel resection and thrombectomy.

Fibrous dysplasia masquerading as chronic maxillary sinusitis.

Fibrous dysplasia is a rare disease of unknown cause that affects one or multiple bones. In its monostotic form, only one bone is involved. The maxilla is the most commonly affected facial bone. Facial deformity with or without pain is the most common presentation of fibrous dysplasia affecting the craniofacial bones. In certain cases the disorder can present clinically as chronic maxillary sinusitis. The clinical, radiologic, and histologic findings, as well as the differential diagnosis and treatment of this condition are reviewed.

In vitro assessment of pirfenidone anti-fibrotic properties in a humanized 3D bioprinted kidney tubulointerstitium model.

Kidney fibrosis is a hallmark of chronic kidney disease (CKD), characterized by excessive extracellular matrix (ECM) deposition that progressively impairs kidney function. Fibrosis affects multiple segments of the nephron, particularly the tubulointerstitium, the space between tubule, capillaries and the intestinal tissue, where homeostasis is disrupted. Currently, no clinically approved therapies exist for kidney fibrosis. Conventional 2D cell cultures and animal models fail to replicate human disease progression, limiting therapeutic development. Three-dimensional (3D) models offer a promisi

Predictive values of the character, timing, and complications of chronic cough in diagnosing its cause.

BACKGROUND: It is not clear whether careful history taking with detailed questioning of the characteristics of cough is diagnostically useful. OBJECTIVE: To determine if the character, timing, or complications of chronic cough were helpful in determining its cause. METHODS: A prospective, descriptive study of consecutive, unselected, immunocompetent patients referred to our university outpatient clinic because of chronic cough. All patients were evaluated by a previously published and validated systematic diagnostic protocol, a self-administered questionnaire, and by observing the character

No Interaction Between Physical Activity and Body Mass Index on Chronic Pain: A Cross-Sectional Study.

BACKGROUND: Chronic pain is a widespread condition with significant individual and societal impact. Overweight has been associated with a higher risk of chronic pain, whereas physical activity may act as a protective factor. The aim of this study was to investigate whether physical activity and BMI interact in their association with chronic pain, and whether this interaction differs across age groups. METHODS: This cross-sectional study used data from the 2021 Danish Capital Region Health Survey, comprising 56,245 participants aged 16 years or older. After excluding participants with missing

Epidemiology of cardiometabolic multiple long-term conditions.

Cardiometabolic multiple long-term conditions (MLTC), defined as the coexistence of two or more cardiometabolic diseases such as diabetes, cardiovascular disease, and chronic kidney disease, are increasingly prevalent and represent a growing challenge for health systems worldwide. Despite rising interest, progress in understanding cardiometabolic MLTC has been limited by substantial heterogeneity in definitions, measurements, and analytical approaches, restricting comparability across studies and limiting translation into clinical and public health practice. Reported prevalence estimates vary

Genomics and phenomics of body mass index reveals a complex disease network.

Elevated body mass index (BMI) is heritable and associated with many health conditions that impact morbidity and mortality. The study of the genetic association of BMI across a broad range of common disease conditions offers the opportunity to extend current knowledge regarding the breadth and depth of adiposity-related diseases. We identify 906 (364 novel) and 41 (6 novel) genome-wide significant loci for BMI among participants of European (N~1.1 million) and African (N~100,000) ancestry, respectively. Using a BMI genetic risk score including 2446 variants, 316 diagnoses are associated in the

Iron imbalance in cancer: Intersection of deficiency and overload.

Iron, an essential trace element, plays a complex role in tumour biology. While iron causes cancer clearance through toxic free radical generation, iron-induced free radical flux also acts as a cancer promoter. These fates majorly guided through cellular response towards pro-oxidant and antioxidant settings in a tumour microenvironment, designate iron-induced oxidative stress as a common yet paradoxical factor in pro-tumorigenesis as well as anti-tumorigenesis, posing a challenge to laying down iron thresholds favouring tumour clearance. Additionally, complexity of iron's association with carc

Genetic variation associated with the occurrence and progression of neurological disorders.

This paper presents an overview of genetic variation associated with the onset and progression of 14 neurological disorders, focusing primarily on association studies. The 14 disorders are heterogeneous in terms of their frequency, age of onset, etiology and progression. There is substantially less evidence on progression than onset. With regard to onset, the conditions are diverse in terms of their epidemiology and patterns of familial aggregation. While the muscular dystrophies and Huntington's disease are monogenic diseases, for the other 12 conditions only a small proportion of cases is as

Psoriasis management tree based on comorbidity.

Psoriasis, a common chronic inflammatory skin disorder, encompasses various subtypes, including guttate, pustular, erythrodermic, and the most common type, plaque psoriasis. Irrespective of the subtype, psoriasis can manifest with multisystemic presentations, including psoriatic arthritis, metabolic disorders, cardiovascular disease, malignancies, chronic kidney disease (CKD), psychiatric illness, and inflammatory bowel disease (IBD). Many comorbidities and concomitant conditions must be considered when selecting the most appropriate therapy for a patient (Kaushik et al., 2019 and Monks et al.

Multiple beneficial effects of melanocortin MC4 receptor agonists in experimental neurodegenerative disorders: Therapeutic perspectives.

Melanocortin peptides induce neuroprotection in acute and chronic experimental neurodegenerative conditions. Melanocortins likewise counteract systemic responses to brain injuries. Furthermore, they promote neurogenesis by activating critical signaling pathways. Melanocortin-induced long-lasting improvement in synaptic activity and neurological performance, including learning and memory, sensory-motor orientation and coordinated limb use, has been consistently observed in experimental models of acute and chronic neurodegeneration. Evidence indicates that the neuroprotective and neurogenic effe
