We studied the effects of intravenous clonidine treatment in a group of 24 patients with acute myocardial transmural anterior and/or lateral wall infarction. Clonidine, known as an antihypertonic agent, was administered as a bolus of 1-2 micrograms/kg body weight and repeated every 2-3 hr. The indications for operation to correct acute mechanical defects after myocardial infarction are clearly established. Less clear is the use of surgical procedures for nonmechanical complications such as persistent ischemia or circulatory collapse. Eighteen patients who survived an acute myocardial infarction were found to have a normal coronary arteriogram. Seven patients were younger than 35 years and six were female. Complete left bundle branch block often masks old as well as acute myocardial infarctions. However, a diagnosis of acute myocardial infarction in the presence of complete left bundle branch block can be made when the acute injury current is large enough to modify the secondary repolarization abnormalities of left bundle branch block. Under these circumstances the classical ST-T changes of an acute infarction may evolve in serial electrocardiograms.

Simple criteria were used to select a low-risk group of patients after acute myocardial infarction. The criteria depended on the presence or absence of diabetes, pulmonary oedema, serious rhythm disorders, and recurrent cardiac pain. Twenty-five patients with recent or old myocardial infarction were studied because they had life-threatening ventricular arrhythmias that required repeated cardioversions and were intractable to medical management. In 12 patients with acute myocardial infarction the hemodynamic effects of a single intravenous injection of 30 mg of pentazocine were investigated. The administration of pentazocine resulted in no significant hemodynamic changes. In particular, there was no increase in peripheral vascular resistance and no evidence of decreased left ventricular function or respiratory depression. In the occasional patients who demonstrated an elevation of pulmonary arterial pressure, it was probably due to a direct effect of the drug on the pulmonary vasculature.

Adult male and female Sprague-Dawley rats were subjected to an isoproterenol-induced myocardial infarction. Six weeks before the induction of myocardial infarction, some of the animals had been orchiectomized or ovariectomized. A retrospective investigation of 420 patients who had survived the acute phase of myocardial infarction revealed 63 reinfarctions (in 57 patients) within three months.
