Diagnostic approach to vision loss.

PURPOSE OF REVIEW: This review emphasizes the differential diagnosis of visual loss for the neurologist. RECENT FINDINGS: As an expert on the CNS, of which the eye is a part, the neurologist is expected to be able to evaluate a patient's report of visual loss and provide at least a cursory examination of the ocular apparatus and visual pathways. To appropriately localize the lesion within the eye and to generate a diagnosis, the neurologist must at least be aware of the other clinical entities that can cause visual loss, especially sudden visual loss, other than optic nerve damage. Once the p

Recurrent carpal tunnel syndrome.

Carpal tunnel release is a common procedure with a high rate of alleviating the patient's symptoms. The incidence of recurrent or persistent carpal tunnel syndrome is rare, although likely underestimated. Complaints of worsening numbness, tingling, or weakness should alert the physician to possible nerve injury and need for early exploration. The overall results of revision carpal tunnel procedures are less successful than primary surgery; however, surgery should be performed when indicated, as it may alleviate or improve symptoms.

[Ischemic stroke: A rare complication of liver hydatid cyst].

Hydatid cyst of the liver (HCL) is a widespread disease in North African countries. We report the case of a 39-year-old patient who was admitted in our intensive care unit because of anaphylactic shock due to a cracked HCL fortuitously discovered. This accident was also complicated by an ischemic stroke witch underline mechanisms are discussed.

Arm Squeeze Test: a new clinical test to distinguish neck from shoulder pain.

PURPOSE: Objective of this study is to evaluate the diagnostic values of the Arm Squeeze Test. The test consists in squeezing the middle third of the upper arm. METHODS: 1,567 patients were included in this study. Diagnosis of cervical nerve root compression or shoulder disease was clinically formulated and confirmed with imaging before performing test. 350 healthy volunteers were recruited as controls. The test was positive when score on a VAS Scale was 3 points or higher on squeezing the middle third of the upper arm compared to acromioclavicular (AC) joint and anterolateral-subacromial are

Feasibility and repeatability of thermal quantitative sensory testing in normal dogs and dogs with hind limb osteoarthritis-associated pain.

The objectives of this study were to determine whether thermal quantitative sensory testing (QST) can be performed in client-owned dogs, is repeatable and whether QST differs between normal dogs and dogs with hind limb osteoarthritis (OA). This clinical, prospective, observational study used clinically normal dogs (n=23) and dogs with OA-associated hind limb pain (n=9). Thermal QST was performed in standing dogs using a high-powered light source delivered by a previously validated system. Dogs were tested on two occasions, 2 weeks apart. Five tests were performed on each hind limb at each time

Hypokalemia mimicking a herniated vertebral disc.

BACKGROUND CONTEXT: A herniated vertebral disc is a common cause of paralysis. Other causes include infections, tumors, and neurologic diseases. A rare and dangerous but in most cases easily treatable cause is hypokalemia. Clinically, the acute symptoms may resemble a herniated vertebral disc, but hypokalemia per se is life-threatening by causing heart arrest through ventricular tachycardia or fibrillation. PURPOSE: A patient with back pain and neurologic deficit in the lower extremities after a history of a herniated vertebral disc presented, who finally receives the diagnosis of hypokalemia

Abduction extension cervical nerve root stress test: anatomical basis and clinical relevance.

PURPOSE: While the Lasègue straight leg raising test is an established test for lumbar nerve root compression, an established equivalent for cervical nerve root compression is missing. The aim of this bi-modal study was to find the most effective way to stretch the cervical nerve roots anatomically in cadavers and to assess its value in the clinical setting. METHODS: Three positional maneuvers of the upper limb were tested on three cadavers to determine the displacement by stretch of the nerve roots C5, C6 and C7. The maneuver which was most efficient in nerve root displacement was applied in

Reference programme: diagnosis and treatment of headache disorders and facial pain. Danish Headache Society, 2nd Edition, 2012.

Headache and facial pain are among the most common, disabling and costly disorders in Europe. Correct diagnosis and treatment is important for achieving a high quality of care. As a national organisation whose role is to educate and advocate for the needs of patients with primary headaches, the Danish Headache Society has set up a task force to develop a set of guidelines for the diagnosis, organisation and treatment of the most common types of headaches and for trigeminal neuralgia in Denmark. The guideline was published in Danish in 2010 and has been a great success. The Danish Headache Soci

Neuro-ophthalmology in horses.

A complete neuro-ophthalmologic assessment is relatively simple, requires minimal instrumentation and should be performed as part of every complete ophthalmic and neurologic examination. This article has summarized the tests that comprise and the species-specific details of the complete neuro-ophthalmologic of the equine patient. Selected causes of sudden blindness in the horse were summarized and some common neuro-ophthalmic conditions with significant ophthalmic consequences, such as facial nerve paralysis, were discussed. Split-lid tarsorrhaphies, which are indicated but rarely used in case…

[Patients at the memory clinic with choreatic movements].

In this case-report we present two patients who were referred to the memory clinic. In both cases there is doubt about the earlier diagnosis. During the physical examination choreatic movements were observed. After repeating diagnostic tests, both patients were diagnosed with Huntington's disease. The dementia with this disease is often preceded by psychiatric symptoms, which makes it quite difficult to diagnose, especially in an early stage of the disease. That is why it is essential that, in case of even the slightest doubt or confronted with new insights, geriatricians should see patients a

Recurrent lumbar disk herniation.

Recurrent lumbar disk herniation is the most common complication following primary open diskectomy. It is defined as recurrent back and/or leg pain after a definite pain-free period lasting at least 6 months from initial surgery. Careful neurologic examination is critical, and laboratory tests should be ordered to evaluate for infection. Imaging demonstrates disk herniation at the previously operated level. It is important to differentiate recurrent disk herniation from postoperative epidural scar because the latter may not benefit from reoperation. Treatment of recurrent lumbar disk herniatio…

Interictal myoclonus with paroxysmal kinesigenic dyskinesia.

We report a new association between interictal myoclonus and paroxysmal kinesigenic dyskinesia (PKD) in 2 patients. By definition, PKD is transient, but the overexcitability of the neuronal system that induces these attacks may be permanent. Interictal myoclonus could be a manifestation of permanent overexcitability.

[Idiopathic intracranial hypertension].

BACKGROUND: Idiopathic intracranial hypertension is a rare syndrome of increased intracranial pressure without a massive lesion or abnormal cerebrospinal fluid. Common symptoms include headache and visual disturbances such as transient visual obscurations or visual loss. Treatment may alleviate symptoms and prevent serious complications. In this study, we present 10 cases illustrating aspects of the disease including causes and treatment options. We also evaluated the effect of treatment and compared the incidence with other studies. MATERIAL AND METHODS: We registered all patients and review

[Guidelines for medical examination of children with mental retardation in pediatric clinics which are specialized for developmental disorders--recommendation based on the current selection and usefulness of diagnostic examinations for children with mental retardation and/or pervasive developmental disorder].

We assessed the present status of choice and usefulness of medical examinations of children with mental retardation (MR) and/or pervasive developmental disorder (PDD). Children with severe MR received more examinations than those with mild MR. Many abnormal findings were demonstrated by MRI in cases of severe MR. Cases of PDD without MR rarely showed abnormal results. Cases of PDD with MR underwent fewer examinations, but showed more abnormal results. We presented guidelines regarding medical examinations for children with MR in pediatric clinics which are specialized for developmental disorde

Benign idiopathic occipital epilepsy: report of a case of the late (Gastaut) type [corrected].

We report the electro-clinical characteristics of a case with Gastaut type idiopathic occipital epilepsy. The visual seizures, a cardinal feature of the syndrome, are described and illustrated by video sequences. A number of stereotyped seizures are shown where the child, an 11-year-old boy, clearly describes the visual phenomena experienced. Prognosis of this common form of partial idiopathic epilepsy is relatively good but, as in our case, seizures may be very frequent during a long period of time. In such cases the term 'benign' is probably not the most appropriate.

Postmeningoencephalitic dysautonomia: report of one case.

We present a patient with postinfectious dysautonomia, which developed 2 weeks after a meningoencephalitic episode. The chief dysautonomic symptoms and signs included constipation, urinary dysfunction, anhydrosis, ptosis, and orthostatic hypotension. Neurophysiologic studies revealed no involvement of the somatic nervous system. The dysautonomia recovered gradually 3 months later with minimal supportive treatment.

Dysautonomia and autonomic neuropathies.

The autonomic nervous system can be affected as part of a more diffuse peripheral nerve disease such as inflammatory polyneuropathy or diabetes, or as a primary disease, such as dysautonomia. Dysautonomia is being diagnosed with increasing frequency in dogs and other species in the Midwest. Affected animals present with absence of parasympathetic and autonomic ganglia and brainstem nuclei degenerate with minimal inflammation. The cause is unknown and treatment symptomatic.
