Neurological sequelae of boxing.

Blunt trauma to the head results in acceleration of the brain within the skull. This takes 2 forms: linear or translational acceleration which produces focal lesions, and rotational acceleration which results in 'sheering stresses' with stretching of nerves and bridging veins. Deceleration of the brain within the skull occurs when the head strikes a stationary object (e.g. floor, ring post). Cerebrovascular events are not infrequently encountered. The most common vascular sequalae is the subdural haematoma, which is also the most frequent cause of death in boxers. Epidural bleeds rare, and are

Neurologic manifestations of AIDS.

An understanding of the biologic characteristics and cellular tropism of human immunodeficiency virus (HIV) is critical to appreciate the diverse neurologic manifestations of HIV infection in patients with the acquired immunodeficiency syndrome (AIDS). Only carefully designed prospective studies can provide information regarding prevalence, incidence, and natural history of the full spectrum of neurologic complications of HIV infection. A degree of tropism for monocyte/macrophages and possibly for cells within the CNS seems certain. One of the most frequent complications is AIDS-related dement

[Neurologic sequelae of alcohol].

The consequences of alcoholism on the peripheral and central nervous system are discussed. Polyneuropathy is present in 30% of the alcoholics, whilst cranial nerve involvement is found in 5-25%. Alcoholic myopathy is only very rarely seen. Wernicke's encephalopathy is found at post mortem investigation in 1.8% of alcoholics, but is rarely clinically diagnosed. The Marchiafava-Bignamy syndrome and central pontine myelinolysis are rarely seen; alcoholic amblyopia which is seen in 0.5% of the hospitalised alcoholics is more frequent, but still a rare finding. Cerebral seizures are common in chron

Neurologic complications of carcinoid.

We reviewed the records of all patients treated for carcinoid tumors at Memorial Sloan-Kettering Cancer Center from 1974 through 1984. Of 219 patients, 90 developed metastatic complications, and of these, 36 developed neurologic complications. Metastases, the most common neurologic complication, included epidural spinal cord compression (14 patients), intracranial metastases (13 patients), leptomeningeal metastases (1 patient), and peripheral nerve lesions (5 patients). Nonmetastatic complications were hepatic encephalopathy (six patients), herpes zoster infection (two patients), cerebral infa

[Neurologic emergencies in alcoholism].

Acute neurological syndromes related to nutritional thiamine depletion or to toxic effects of ethanol are presented. Convulsive conditions linked to alcoholism, alcoholic hypoglycemia, withdrawal syndromes and the rare central pontine myelinolysis are described. Pathogenesis, appropriate treatment and prognosis of all these acute neurological disorders are discussed.

Neurologic complications of migraine.

Migraine can be associated with hemiplegia, ophthalmoplegia, retinal and vertebrobasilar insufficiency, amnesia, confusion, altered perception, stupor and even death. Migrainous complications must be differentiated from structural lesions, metabolic disorders, convulsive states are cerebrovascular thromboembolism. Treatment of complicated migraine is directed toward prophylaxis of vasoconstriction. Agents that produce vasoconstriction, such as ergot preparations, and known migraine precipitants should be avoided in migraineurs.

Neurological complications of immunization.

Vaccines prepared from whole, killed organisms (pertussis and possibly influenza) may cause neurological allergic reactions producing encephalopathy. These reactions are characterized by acute, monophasic demyelinative processes and occur with no greater frequency than 1 per 100,000 vaccine recipients; onset is within 4 days of immunization, and recovery is usually complete. No evidence suggests that these vaccines produce an insidious, progressive encephalopathy. Only with the swine influenza program of 1976 has Guillain-Barré syndrome appeared to follow immunization. Vaccines prepared from l

Neurological complications in hemophilia.

The clinical data of 59 patients with hemophilia A or B are reviewed. Intracranial bleeding was observed in 6 patients and a minor bleeding episode was assumed in a further 8 patients. Neurosurgical evacuation of the hematoma was necessary in 2 cases and the remaining patients were treated solely with factor VIII or IX. In 10 patients a peripheral nerve lesion was observed, paresis of the femoral nerve being the most frequent (5 cases). Two patients showed a lesion of the lumbar and sacral plexus, 2 patients a lesion of the radial nerve and one patient a lesion of the cutaneous femoris lateral

[Neurological symptoms in poisoning].

Acute and chronic intoxications become manifest in primary neurological symptoms. After a definition of poisoning the autonomic, neurological and psychological disturbances are briefly discussed and the therapeutic measures presented in a table. Later, the neurological symptoms are described with reference to oberservations of cases of lead, thallium, E 605 (parathion), carbon monoxide, mercury, amphetamine and botulin poisoning. Four table and 9 figures supplement the text.

Electronystagmography in neurological diagnosis.

Electronystagmography (ENG) is a technique for recording nystagmus and other eye movements; our technique, based on the utilization of a bioelectrical potential which exists between the retina and the cornea, has been reported in detail elsewhere in a monograph [29]. ENG has been used particularly by otologists to record caloric nystagmus and study labyrinthine function [2, 4, 16, 19]. This technique should have even greater applications in the diagnosis of neurological disease, since nystagmus and abnormalities of ocular and vestibular functions are symptoms frequently encountered by the neur

The neurological examination in dementia.

The thesis of this chapter is that the neurological phenomena discussed and numerous others omitted or not yet recognized relate primarily to a release of primitive activity when diffuse brain damage erodes cerebral inhibition mechanisms. Anatomical correlations are always difficult when there are multiple lesions, and those that have been made have not completely explained these phenomena. A phylogenetic explanation is not completely adequate either but certainly does not bar further investigations. Indeed such a concept points out the necessity to continue to search for other fetal or develo
