A 78-year-old woman with medullary thyroid carcinoma (MTC) demonstrated persistently elevated serum calcitonin despite prior surgery and radiotherapy. 99m Tc-octreotide imaging failed to detect skeletal metastases, while 68 Ga-FAPI PET/CT revealed extensive skeletal involvement. Biochemical markers were disproportionately low relative to disease burden, consistent with a low-secretory MTC. This case highlights the limitations of SSTR-targeted imaging in MTC and underscores the diagnostic superiority of FAP-targeted imaging.

Congenital hyperinsulinism (CHI) is the most frequent cause of severe, persistent hypoglycemia in infancy and childhood. We report a case of CHI with diffuse pancreatic abnormality diagnosed preoperatively using the 68Ga octreotide (DOTA NOC) positron emission tomography scan. Genetic analysis revealed homozygous ABCC8 splicing mutation. Subtotal (95%) pancreatectomy was done, and the infant remained euglycemic and was discharged on breast feeds. The patient is continuously followed up and is asymptomatic until 9 months.

A 25-year-old woman presented with metastatic pancreatic neuroendocrine tumor with carcinoid syndrome. She was refractory to octreotide and did not respond well to chemotherapy. Although surgical debulking remains the primary approach for managing these tumours, it entails inherent risks, including potentially exacerbating carcinoid syndrome. We strategically delivered the one Peptide Receptor Radionuclide Therapy cycle before tumor debulking, a decision that yielded a remarkable response, stabilizing her condition.

Cushing's syndromes caused by ectopic corticotropin secretion represent 10 to 15% in the causes of Cushing's syndrome. Somatostatin receptor scientigraphy can be successful in the localization of ACTH-secreting neuroendocrine tumors. We report a case of Cushing's syndrome caused by ectopic corticotropin secretion from a thymic carcinoid tumor. Conventional localization techniques failed but the tumor was detected by a double binding thoracic scintigraphy using 111Indium-octreotide and 99mTechnetium-albumine macroagregates.

The management of peptic ulcer haemorrhage still poses questions and controversies and this applies to the pharmacological mode. This review suggests that high-dose acid suppression is beneficial in reducing rebleeding and surgery rates. The choice of acid suppressant is far from conclusive, but emerging evidence suggests that proton pump inhibitors may be more effective than H2-antagonists. The only other drug which may be useful in selected patients is octreotide, but its universal use cannot be recommended.

The authors present a case of Cornelia de Lange syndrome with a rare presentation of antenatally diagnosed chlyothorax presenting at birth with respiratory distress.
