Review of non-invasive intracranial pressure measurement techniques for ophthalmology applications.

Assessment and monitoring of intracranial pressure (ICP) are important in the management of traumatic brain injury and other cerebral pathologies. In the eye, ICP elevation and depression both correlate with optic neuropathies, the former because of papilledema and the latter related to glaucoma. While the relationship between ICP elevation and papilledema is well established, the relationship between low ICP and glaucoma is still poorly understood. So far, ICP monitoring is performed invasively, but this entails risks including infection, spurring the study of non-invasive alternatives. We re

Adenovirus-related epidemic keratoconjunctivitis outbreak at a hospital-affiliated ophthalmology clinic.

Adenovirus-associated epidemic keratoconjunctivitis (A-EKC) is a cause of large and prolonged outbreaks in ophthalmology clinics and can result in substantial morbidity. A-EKC outbreaks are often the result of contaminated ophthalmologic equipment, surfaces, or hands. Contaminated multidose eye drops are also a likely culprit, but few prior studies provide clear epidemiologic evidence that adenovirus transmission resulted from contamination of eye drops. We describe an A-EKC outbreak at a large, hospital-affiliated eye clinic that affected 44 patients. The unique epidemiology of the outbreak p…

[Next-Generation Sequencing: A Quantum Leap in Ophthalmology Research and Diagnostics].

Many eye diseases have a genetic basis, and most can be caused by mutations in many different genes (extensive genetic heterogeneity). The retinal dystrophies are a good example: More than 200 genes have been identified for the isolated forms (Leber's congenital amaurosis, retinitis pigmentosa, cone-rod dystrophy, congenital stationary night blindness), and for syndromes that comprise additional dysfunctions or malformations of extraocular tissues and organs. Selecting genes for diagnostic testing has been difficult, and their analysis with the hitherto predominant DNA sequencing method (Sange

Flashes and floaters: a survey of Canadian ophthalmology residents' practice patterns.

BACKGROUND: Residents are often the first point of contact in assessing patients who present with "flashes and floaters" in the emergency ophthalmology clinic. Most often, these symptoms are attributed to a posterior vitreous detachment (PVD), which may be associated with a retinal tear in up to 14% of cases. A proper peripheral retinal exam is, thus, imperative, and techniques other than scleral depression may not be sufficient. We conducted a cross-sectional survey of Canadian ophthalmology residents-in-training to understand the current resident practice patterns for examination of the peri…

The expanding role of vascular endothelial growth factor inhibitors in ophthalmology.

Vascular endothelial growth factor (VEGF) plays an important role in both physiologic and pathologic angiogenesis and contributes to increased permeability across both the blood-retinal and blood-brain barriers. After 2 decades of extensive research into the VEGF families and receptors, specific molecules have been targeted for drug development, and several medications have received US Food and Drug Administration approval. Bevacizumab, a full-length antibody against VEGF approved for the intravenous treatment of advanced carcinomas, has been used extensively in ophthalmology for exudative age…

[Janus-faced?: Effects and side-effects of interferon therapy in ophthalmology].

Interferon alpha (IFN-α) and interferon beta (IFN-β) are naturally occurring cytokines, which belong to the type I interferons and share the same receptor leading to very similar therapeutic effects. The immunomodulatory effect of type I interferons includes modulation of antibody production, inhibition of lymphocyte proliferation, inhibition of delayed-type hypersensitivity and enhancement of T-cell and NK-cell cytotoxicity. An increasing number of open clinical studies and case reports have demonstrated the efficacy of IFN-α for severe ocular inflammation in patients with Behçet's disease an

Role of the macular optical coherence tomography scan in neuro-ophthalmology.

BACKGROUND: Recent improvements in optical coherence tomographic (OCT) resolution and automated segmentation software have provided a means … ity to differentiate and localize ambiguous causes of vision loss and visual field defects in neuro-ophthalmology. Ganglion cell layer analysis in volume OCT data may provide yet another piece of the puzzle to understanding structure-function relationships and its application to diagnosis and monitoring of optic nerve diseases, while similar structure-function …

Extraocular muscle damage associated with endoscopic sinus surgery: an ophthalmology perspective.

BACKGROUND: Orbital complications associated with endoscopic sinus surgery are well documented. Damage to the medial rectus muscle results in complicated strabismus and disturbing diplopia. The aim of this study was to characterize the types of extraocular muscle injury and the number of muscles involved that may complicate endoscopic sinus surgery and correlate its occurrence to factors in the surgical procedure itself. METHODS: A retrospective chart review was performed of 14 patients with strabismus after endoscopic sinus surgery. Operative notes of the surgical procedure, pathology report

Clinical research in pediatric ophthalmology: the Pediatric Eye Disease Investigator Group.

The Pediatric Eye Disease Investigator Group (PEDIG) is a network of university-based and community-based pediatric eye care practitioners that is conducting multiple clinical research studies. The group has conducted the Congenital Esotropia Observational Study, which assessed the early course of esotropia in infants, and the Amblyopia Treatment Studies, a series of randomized trials, the first of which compared atropine and patching for treatment of moderate amblyopia in children 3 to <7 years old. Herein, the results of these studies are summarized, and the current and future studies of the

Ophthalmology update for primary practitioners. Part I. Update on optic neuritis.

Optic neuritis is a common cause of acute visual loss. It is typified by sudden onset of visual impairment and pain with eye movements, followed by spontaneous recovery of vision over several months. Pathologically, optic neuritis is an acute demyelinating event affecting the optic nerve. Objective physical findings are typically few, including an afferent pupillary defect or Marcus-Gunn pupil, whereas subjective psychophysical findings abound (ie, diminished central visual acuity, color vision, decreased contrast sensitivity, and visual field abnormalities). These characteristics have made th

Human leukocyte antigens associated with hyperthyroid Graves ophthalmology in Japanese patients.

PURPOSE: To evaluate the association of human leukocyte antigen (HLA) types in Japanese patients with Graves ophthalmopathy. METHODS: Japanese patients with severe ophthalmopathy (48 patients) and patients without ophthalmopathy (46 patients) were selected prospectively from 234 consecutive patients with hyperthyroid Graves disease. The criteria for severe ophthalmopathy were proptosis of 19 mm or more, or a 3-mm difference in exophthalmometry measurement between the two eyes, or diplopia at the primary eye position. The criteria for the absence of ophthalmopathy were proptosis of 16 mm or le

The plain radiograph in ophthalmology: a wasteful and potentially dangerous anachronism.

The indications for 822 consecutive referrals for skull radiography were prospectively studied in a large eye hospital over a one year period. In 85.9% of patients the results were normal, and in 89% of the remainder they had no positive effects on management; all patients in whom a 'beneficial' effect could be identified would have been more appropriately investigated by other means. Fourteen of 25 patients whose skull radiographs were normal were shown by computed tomography or magnetic resonance imaging to have orbital or intracranial lesions. Views of the optic canals, orbits or paranasal

The first German textbook of ophthalmology "Augendienst" by G. Bartisch, 1583.

This book contains various illustrations, portraits and an exact index, testimonials proving the author's professional successes as well as an accurate list of the qualities that should be demanded from any ophthalmologist. The anatomy of the head and eye is described according to Galen's ideas and Vesalius' book. Many remedies, prescriptions and medical treatments are discussed, partly showing the mystic influences of the Middle Ages. Bartisch reports several diseases for the first time: Allergic reactions, sympathetic ophthalmia, hemeralopia, photoelectric keratoconjunctivitis, amaurosis due
