Orbital fractures in children.

OBJECTIVE: To determine if the pattern of orbital fractures may be influenced by the changing craniofacial ratio of the growing child, as the orbit is the boundary between the face and the cranium. DESIGN: Retrospective case series of 40 patients between the ages of 1 year and 16 years with orbital fractures. SETTING: The Albany (NY) Medical Center Hospital, a tertiary level 1 trauma center. OUTCOME MEASURES: The sex, age, site, and mechanism of injury, associated injury, and treatment methods for children admitted to the Albany Medical Center Hospital with orbital fractures between July 19…

Orbital trauma and reconstruction.

This article reviews the most recent topics of investigation concerning orbital trauma and reconstruction. During the past year there have been many articles of interest regarding management of the sequelae to orbital trauma, pathophysiologic process in orbital injury, diagnostic techniques, as well as operative methods, materials, and complications. Recent research has focused on the pathophysiology of traumatic optic neuropathy and visual compromise in orbital hemorrhage. Theories regarding the etiology of atraumatic spontaneous enophthalmos are described. In the realm of orbital reconstruct…

Inflammatory orbital pseudotumor.

1. Inflammatory orbital pseudotumor, a term used to describe any idiopathic inflammatory lesion of orbital tissue that simulates a neoplasm of the orbit, is a definite clinical entity masquerading in various forms and covering a wide spectrum of periocular lesions. 2. Inflammatory orbital pseudotumor has a good prognosis with steroid therapy. Once it is suspected, prompt and effective treatment must be given as soon as possible, even before laboratory work-up has been completed. 3. A great deal of investigative study must be done to rule out any other life-threatening diseases. Long-term follo…

Orbital xanthogranuloma in adults.

The onset of periorbital xanthogranuloma in adults is rare and may be accompanied by haematological abnormalities and malignancy. The appearance of the eyelid lesions is virtually diagnostic, producing readily recognisable diffuse, yellow plaques, and affected patients should be investigated and reviewed regularly for systemic disease. Three cases are described, in which periorbital cutaneous plaques were associated with abnormal tissues in the superior part of the orbit; these abnormal tissues caused displacement or restricted movement of the globe or upper eyelid. The possibility that two ca…

Orbital tumors in children.

There are many reviews of pediatric orbital tumors in the literature. Most of the series originated from the tumor registries of large pathology referral centers, whereas others are from large medical institutions and pediatric hospitals. The distribution of types of tumors reported in each series are unavoidably biased by the nature of the evaluating facility. The findings were also affected by the geographic location of the clinical facility. We present an analysis of 141 orbital tumors in children and review 9 other series (totaling 1,229 cases) for an aggregate total of 1,370 cases. Our cl…

Orbital expansion in plagiocephaly.

In moderate to severe plagiocephaly, the transverse diameter of the orbit on the affected side is consistently decreased. In these cases, the supraorbital rim has been expanded by the required amount and bone grafted. This makes for greater orbital symmetry and ease in obtaining the correct supraorbital convexity. Ten patients have been handled in this way with satisfactory postoperative results.

Herniated orbital fat.

Three patients with acquired orbital fat herniation are presented. Each was felt to have a dehiscence in Tenon's capsule, allowing fat to spontaneously herniate forward. Orbital fat herniation may be confused with other lesions, such as dermolipomas. The clinical features and differential diagnosis of orbital fat prolapse are discussed.

Orbital reconstruction with proplast.

Proplast was implanted in 15 cases of orbital reconstruction. Seven cases were orbital floor fractures, four cases were phthisical enophthalmos, and four cases were anophthalmic enophthalmos. In orbital floor fracture cases, thin (2-mm) Proplast was placed onto the fractured orbital floor after the incarcerated orbital tissue was released. To correct the enophthalmic socket, Proplast was shaped into small pieces (10 x 5 x 2 mm), then inserted into the subperiosteal space from all directions to fill up the orbital cavity. The results of these 15 cases were satisfactory. The average follow-up pe…

Orbital apex syndrome.

A case is presented of a 16-year-old anticoagulated, immunocompromised male presenting with a blind, proptotic left eye, total ophthalmoplegia, and central retinal artery occlusion. Ophthalmic examination revealed other features of the orbital apex syndrome. Mucormycosis misdiagnosed as a traumatic orbital hemorrhage led to the patient's death. In this review, differential diagnosis of the orbital apex syndrome (loss of cranial nerves II, III, IV, ophthalmic division of V, and VI) is outlined, and features, diagnosis, and treatment of mucormycosis are discussed. Although mucormycosis is most c…

Sequelae of orbital fractures.

Many injuries and complications of orbital fractures are instantaneous and unpreventable, some develop over time, and some are a result of surgery. Most complications can either be managed or prevented and are anticipated based on the fracture location and size. A thorough search for probable complications is mandatory, especially ocular injuries and intracranial complications. Most patients are carefully followed for 7-14 days, and the significant residual sequelae are managed. Some complications, however, require urgent care.

Evaluation of orbital injuries.

Trauma to the orbit is evaluated with a careful history, a thorough physical examination, and specific radiographic studies. Orbital trauma involves a wide range of conditions, including soft tissue adnexal injury, orbital contusion, optic nerve damage, ocular injury, intraorbital foreign bodies, and orbital fractures. This article emphasizes the importance of an accurate history and a comprehensive examination. It highlights relevant questions to ask, discusses the meaning of pertinent complaints, describes steps in the examination, discusses the diagnostic importance of the physical findings…

Congenital orbital teratomas.

Teratomas of the orbit, rare congenital tumors consisting of normal tissues derived from all three germ cell layers, are probably choristomas rather than true neoplasms, and must be considered in the differential diagnosis of neonatal orbital masses. They typically grow rapidly after birth, causing destructive proptosis and exposure keratopathy; however, they may take a slowly progressive course, expanding slightly over several years. We studied two cases of histopathologically proven orbital teratomas. In one patient, a 2-month-old girl, the tumor was solid and enlarged rapidly after birth. I…

[The syndrome of enlarged orbits].

Combined orbital fractures cause hematomas and subsequent rigidity of the ligament apparatus around the inferior rectus muscles, while the few true blow-out fractures may lead to an adherence of connective tissues to the floor. The authors describe a third mechanism: Enlargement of the orbit impairs motility, especially the elevation of the globe: Diagnostic pressing of the orbital, parabulbar contents improves elevation. In 9 such cases that were free of the two other knows mechanisms motility was improved by reducing orbital volume. This was performed by implants (4 cases), by instillation o…

Orbital and childhood sarcoidosis.

Orbital infiltration, anterior uveitis, disc edema, choroiditis, erythema nodosum, and polyarticular arthritis due to sarcoidosis occurred in a five-year-old child who presented with unilateral proptosis. Diagnosis was enhanced by computerized tomography of the orbit, and confirmed by synovial biopsy findings. This patient's course illustrates the importance of thorough and frequent ocular examinations in childhood uveitis associated with systemic disease and demonstrates an unusual finding of orbital sarcoidosis. Childhood sarcoid arthritis may be misdiagnosed as juvenile rheumatoid arthritis…

Orbital pneumosinus dilatans.

Pneumosinus dilatans consists of an abnormal dilatation of the paranasal air sinuses which contain air only and are lined by normal epithelium. The condition is most commonly observed when it affects the sphenoid sinus as a response to a local meningioma of the tuberculum sellae or planum sphenoidale. Pneumosinus dilatans affecting the walls of the paranasal sinuses which form the boundaries of the orbit is less common; six patients reported here presented with either unilateral or bilateral exophthalmos. The frontal sinus and ethmoid cells were affected unilaterally in four patients and the m…
