STUDY DESIGN: Prospective cohort study.
OBJECTIVES: To study the predictive value of magnetic resonance imaging (MRI) findings of thoracolumbar spine fractures concerning the radiologic and clinical outcome.
SUMMARY OF BACKGROUND DATA: Disagreement about the proper treatment of thoracolumbar spine fractures is caused by insufficiency of conventional imaging techniques. Previous studies have shown that MRI is capable of distinguishing injury to all structures of the fractured spine and thus may help develop schemes with higher predictive power.
METHODS: A total of 53 patients with 71 fractur

Dual-energy computed tomography (DECT) yields precise anatomic and functional images by exploiting differences in the interactions of high- and low-energy photon spectra with different tissues' and materials' atomic components to more precisely differentiate the chemistry of tissues and disease processes than is possible with traditional single-energy CT scan acquisitions. This article introduces the history of DECT, its physical basis, scanner designs, radiation dose considerations, and postprocessing techniques. DECT's clinical applications also are described, and this relatively new imaging

PURPOSE: Anterior cervical corpectomy and fusion (ACCF) to C2 (ACCF-C2) for multilevel lesions is a challenging procedure that is indicated for massive ossification of the posterior longitudinal ligament (OPLL) extending to C2 or stenosis at the upper cervical region accompanied by kyphosis. However, there is little information on the effectiveness of and complications related to ACCF-C2. The purpose of this study was to investigate the overall surgical results and postoperative complications of ACCF-C2 for cervical myelopathy.
METHODS: Sixteen patients who underwent ACCF-C2 for OPLL and cerv

Since 1971 the Orthopaedic Oncology Service at the Massachusetts General Hospital has performed 150 resections and allograft implantations for the management of mostly tumorous bone conditions. The procedures used cadaveric segments harvested from donors and stored at -80 degrees after exposure of the cartilage to 10% glycerol as a cryopreservative. Following resection of the tumor, the selected part was implanted and held with plates and screws or intramedullary rods, and the patients were observed closely for alterations suggestive of "rejection" (none was seen), presence of recurrences or m

UNLABELLED: Background/Objectives: Spinal fusion with static fixation-surgically joining two or more vertebrae to eliminate motion-is commonly employed to treat degenerative spinal disease. However, the rigidity imposed by static constructs and the increased load on the adjacent segments frequently result in complications such as disc or facet degeneration, spinal stenosis (SS), and segmental instability. This study investigates the effectiveness of pedicle-based dynamic stabilization using the Dynesys system, particularly in a dynamic-transitional optima (DTO) hybrid configuration, in mitigat

Anatomical repair of distal biceps tendon ruptures has been shown to restore elbow supination and flexion strength. Here, we report the outcomes of distal biceps tendon reattachment using the ToggleLoc fixation device with ZipLoop technology through a single incision. This was a retrospective study of 38 patients with a mean age of 49.5 years. The mean follow-up time was 15 months (range 4/28). The average time to surgery was 21 days. The fixation button was introduced in a bone tunnel and the tendon passed through a bone window using the ToggleLoc™, which allows the tendon to be tensioned usi

In pediatric neurosurgery departments in India, craniosynostosis is being increasingly identified and dealt with during the past several years. The management of this problem is well established in units that have a strong pediatric bias, whereas it is still in infancy in certain departments. Some misconceptions exist regarding this condition with reference to clinical, genetic aspects and management-in particular, the surgical indications. The experience gained for more than 2 decades of treating this condition as well as the problems faced in the management of this condition will be discusse

BACKGROUND: Spinal instrumentation in adolescent idiopathic scoliosis (AIS) aims to correct spinal deformity and maintain long-term spinal stability until bony healing is ensured. The purpose of this study was to compare the minimum 2-year postoperative radiographic and clinical results of posterior spine correction and fusion with all-hook instrumentation versus hybrid segmental instrumentation versus pedicle screw instrumentation for AIS from a single institution.
METHODS: A total of 105 patients with AIS who underwent a posterior spinal fusion with segmental pedicle screw (35), hook (35),

BACKGROUND: The most common congenital chest wall deformities are pectus excavatum and pectus carinatum. Various techniques have been described for correction of pectus deformities. We describe our experience with surgical repair of pectus deformity (PD) in adults, including our new technique, which uses a resorbable plaque for fixation of the sternum.
METHODS: We reviewed the records of 317 patients who underwent surgical correction of PD between October 1997 and December 2005.
RESULTS: All of the patients were male and the median age was 21.3 years (range, 16-32 years). Of 317 patients, th

BACKGROUND: Formation of bacterial biofilms on implants is a severe complication following orthopaedic surgery. In the present study we addressed the role of the immune response, particularly with regard to the pathogenesis of the disease.
METHODS: In a prospective study comprising 74 patients with implant-associated post-traumatic osteomyelitis, peripheral blood cells as well as cells recovered from the infected site during surgery were characterised phenotypically and functionally.
RESULTS: We found massive infiltration of polymorphonuclear neutrophils (PMN), which were highly activated, p

Lumbosacral dislocations are rare high-energy injuries most often reported as isolated cases. Improved outcomes from major trauma means that these cases will likely become more common. We report six cases treated at our institution in the last decade. Nonoperative care in two cases resulted in severe deformity and pain and required late reconstruction. Surgical decompression, reduction, and stabilization with posterior pedicle screw instrumentation resulted in a stable lumbosacral junction, although in one case, implant failure required revision with anterior column reconstruction to achieve t

6 cases of aneurysmal bone cyst of the spine are presented. The clinical picture, diagnostic procedures and operative treatment will be discussed. Although aneurysmal bone cyst is a benign, non-neoplastic lesion its spreading tumor growth with destruction of important bony structures of the vertebral body, consecutive instability and cord compression presents a considerable menace to the mostly teenage-patient. Spinal computerized tomography is the diagnostic method of choice. The goal of surgical therapy of the aneurysmal bone cyst of the spine should include radical tumor excision in order t

Background Bone marrow aspirate (BMA) is widely used in orthopedic and regenerative procedures due to its content of mesenchymal stromal cells (MSCs), hematopoietic progenitors, and supportive cellular elements that contribute to bone healing and tissue repair. Conventional bone marrow aspiration techniques are limited by progressive hemodilution, in which peripheral blood increasingly contaminates the aspirate as volume is drawn from a fixed location. This process both reduces MSC and progenitor concentrations and increases variability in aspirate samples. Objectives The purpose of this study

UNLABELLED: Osteoporotic fracture (OPF) remains a major clinical challenge for skeletal regeneration. Impaired osteogenesis and excessive remodeling result in prolonged and poor quality of fracture healing. To augment bone formation and inhibit excessive resorption simultaneously, we constructed a biodegradable magnesium-based implant integrated with the anti-catabolic drug zoledronic acid (ZA); this implant exhibits controllable, sustained release of magnesium degradation products and ZA in vitro. The extracts greatly stimulate the osteogenic differentiation of rat-bone marrow-derived mesench
