Splints/orthoses in the treatment of rheumatoid arthritis.

BACKGROUND: Splints/orthoses are often recommended to patients with rheumatoid arthritis (RA) to decrease pain, decrease swelling and/or prevent deformity. These orthoses include resting hand splints, wrist supports, finger splints and special shoes and shoe inserts. OBJECTIVES: To assess the effectiveness of splints/orthoses in relieving pain, decreasing swelling and/or preventing deformity and determine the impact of splints/orthoses on strength, mobility and function in people with RA. SEARCH STRATEGY: We searched MEDLINE, EMBASE, the PEDro data base and Current Contents up to January 200

Metatarsalgia and rheumatoid arthritis--a randomized, single blind, sequential trial comparing 2 types of foot orthoses and supportive shoes.

OBJECTIVE: To compare the effects of semi-rigid and soft orthoses worn in supportive shoes, and supportive shoes worn alone, on metatarsal phalangeal (MTP) joint pain. MTP joint synovitis, and lower extremity function in patients with rheumatoid arthritis. METHODS: Twenty-eight subjects referred to occupational therapy received in random order 3 interventions for 12 week trials, separated by 2 week washouts. A crossover design compared effectiveness of interventions. RESULTS: Twenty-four subjects completed the study. A reduction in mean pain scores from baseline to final visits showed that s

Depression in children with hemophilic arthropathy and poliomyelitis: a preliminary report.

The aim of this study was to evaluate children with chronic disorders like hemophilia and poliomyelitis from the psychological perspective, to determine the frequency of depression, to identify the risk factors and to investigate the relation between disability and depression. Thirty-five patients with disability due to poliomyelitis and 12 patients with hemophilic arthropathy were included in the study. Thirty-six healthy children from the district schools served as controls. The Children's Depression Inventory (CDI) was used to assess the extent of depression. For the hemophilia group, joint

Unusual occipitoatlantal fracture dissociation with no neurological impairment. Case report.

The authors describe an unusual case of a complex traumatic fracture-dissociation injury of the craniovertebral junction, which the patient survived with no neurological damage. This case featured the rare combination of an avulsion of both the right occipital condyle and clivus and a fracture of the left lateral mass of the atlas. Because of the craniocervical ligament injury and the slight anterior occipitoatlantal dislocation, the lesion was considered to be unstable and was treated successfully with a cervical collar. The authors emphasize that thin-slice computerized tomography scanning w

Aspects of spinal deformity in familial dysautonomia (Riley-Day syndrome).

Familial dysautonomia (FD) is a rare autosomal recessive disease occurring in Jews of Ashkenazi descent, with only some 500 recognized cases. The causative gene was identified on chromosome 9. FD is of considerable orthopedic interest, because of the prevalence of skeletal deformity. About 90% of surviving dysautonomic children will develop a spinal curvature, commonly a scoliosis. The scoliotic curve is usually kyphotic rather than lordotic, and appears during the first decade of life. Fifty-one of the 90 reported cases of familial dysautonomia in Israel involved patients who were seen at the

The effect of ultrasonographic screening on the incidence of developmental dislocation of the hip.

The effect of ultrasonographic screening and treatment with a Pavlik harness on the incidence of developmental dislocation of the hip has been studied retrospectively. Observations were made at three different consecutive periods; in the first, clinical screening was carried out, in the second, ultrasonography was added for children up to 3 months of age, and in the third ultrasonography was used within the first few days of birth. A significantly decreased dislocation rate was demonstrated in each group, suggesting that early ultrasonography was of value in the early detection of dislocation.

Congenital hip dislocation: the importance of early detection and comprehensive treatment.

Hip dislocation is the musculoskeletal condition most commonly missed during neonatal examinations. Failure to diagnose the condition can lead to long-term disability and is a common target in pediatric legal suits concerning damage to the musculoskeletal system. Early neonatal assessment for hip dislocation includes an examination using the Barlow and Ortolani tests. Later examinations include assessment of gluteal folds, knee height and the degree of hip abduction. The preferred treatment is use of the Pavlik harness, an outpatient treatment regime that provides effective reduction in 90 per

[Myelomeningocele--what can be done? Aids for evaluating rehabilitation from the orthopedic viewpoint].

Shortly before or shortly after the delivery of a child suffering from congenital paraplegia (myelomeningocele or spina bifida) the obstetrician is obliged to give advice as to what will follow after neurosurgical closure of the cele and shunting of the hydrocephalus. The parents' understanding of therapy and integration of the patient is developed to assess and utilise the future chances of the disabled child. Integration of children with congenital paraplegia is performed by an interdisciplinary team of therapists. However, the obstetrician's advice is of great value in preparing the parents

A nasal trumpet orthosis to maintain nares openings and respiratory function for patients with facial burns: a case report.

Management of facial burns is a challenge to the burn team because it may lead to functional and cosmetic compromise. Severe scarring of the nares may lead to nasal occlusion. This article introduces a method of maintaining nasal patency that allows respiratory exchange through the use of a custom-fabricated, semirigid tubular orthosis. The technique for fabrication is reviewed, and the use of the device is addressed through a case report. This inexpensive, readily available device is useful in preventing nasal occlusion that results from scar formation.

Brachial plexus injuries: an integrated treatment concept.

An integrated concept is presented for the treatment of lesions of the brachial plexus. In such patients, nerve repair is useless as a single method of therapy. Only the combination of nerve repair with both conventional and new methods of muscle and tendon transfer, including the free muscle transplant innervated by vascularized nerve grafts, can give the patient functional use of the paralyzed arm.

The prevention of recurrent fractures of the lower extremities in severe osteogenesis imperfecta using vacuum pants: a preliminary report in four patients.

Severe osteogenesis imperfecta results in frequent fracturing of lower extremity bones when the child assumes upright weightbearing. To prevent such breakage and to provide simple, comfortable standing, a special "vacuum pant" orthosis was designed and assessed in four patients. Not only has this enabled the family to help their children stand up safely and comfortably, but the frequency of fracturing has diminished. Bone densitometry studies of the tibia revealed an increase in both the mineral content (0.39 to 0.70 g/cm) and the bone density (0.44 g/cm2) above that expected for growth alone

Habitual toe-walking: evaluation and approach to treatment.

Habitual toe-walking has been presented as a prolongation of a normal stage of development that requires conservative treatment to prevent or ameliorate associated gait abnormalities such as tripping and falling. An approach to the evaluation of a child with toe-walking should include (1) medical history (prenatal, intrapartum, and postnatal), (2) gait evaluation, (3) musculoskeletal examination, and (4) neurologic examination. Pathologic entities producing toe-walking have been explored in order to differentiate those conditions from idiopathic (habitual) toe-walking. The most common etiologi

Lower extremity fractures in paraplegics: pattern, treatment, and functional results.

Eighty-seven lower extremity fractures in 71 paraplegics were retrospectively evaluated to determine adequacy of treatment, functional result, and range of motion of the hip and knee. Most fractures treated closed healed without difficulty, but with a complication rate of 19%. Length of hospitalization averaged almost 3 months in the group treated closed, and residual hip and knee stiffness was the major complication. This was not a problem except in high demand patients, such as wheelchair athletes, in whom decreased range of motion was a significant limitation. Treatment with circular casts

Surgical correction of spastic equinovarus deformity in the adult head trauma patient.

The results of 59 surgical procedures for correction of spastic equinovarus deformity of the foot using the split anterior tibial tendon (SPLATT) were reviewed in 54 adults with traumatic head injury. The mean time of follow-up was 49.7 months. Thirty-nine individuals had hemiplegic involvement, three had triplegic involvement, and 12 were quadriplegic. Evaluation of the patterns of lower extremity muscle activity preoperatively by dynamic electromyography in 33 patients showed no significant difference from that seen in the hemiplegic stroke population, namely, spastic calf muscles with overa

Use of the Pavlik harness in nonoperative management of congenital dislocation of the hip.

The Pavlik harness and its application are described. A series is reported of 25 patients with congenital dislocation of the hip, aged between one day and eight months, all of whom were successfully treated in the harness. Since the harness was found to be effective in treating older infants, it is suggested that treatment of the newborn could be delayed until the age of two or three months, thus avoiding unnecessary treatment of those hips which will reduce spontaneously.

Humeral shaft fractures. Treatment with a simple hand sling.

Complications of operative treatment of closed fractures of the shaft of the humerus occur in about 12 per cent of cases. Fixation of the arm in some form of cast gives better results but the complication rate is still 5.5 per cent and there is a risk of shoulder-hand-finger syndrome. In order to minimize these complications the author treated patients with humeral shaft fractures by supporting the hand only, in a simple sling. The method is described and its advantages discussed. All the fractures, including transverse ones, united in 3 to 6 weeks without any serious complications.

The halo vest: an evaluation of motion and forces across the neck.

Flexion-extension motion and compression-distraction forces across the cervical spine were evaluated in 6 patients in halo vests and in 1 patient in a halo cast. Motion in the vest was evaluated in supine and upright positions and averaged 31% of normal motion at the levels tested. Compression-distraction forces showed great individual variation and frequent compression. The average distraction force varied in different positions by nearly 20 pounds in the vest and over 30 pounds in the cast. Methods of improving cervical spine stability in halo orthoses are discussed.
