Long-Term Outcome in Cervical Lymph Node-Positive Nasopharyngeal Carcinoma Following Intensity Modulated Radiation Therapy With One-Step Cervical Nodal Clinical Target Volume Delineation by Geometric-Anatomic Expansion From Nodal Gross Target Volume: A Double-Center Experience.

PURPOSE: To report long-term results of patients with cervical node-positive (CLN+) nasopharyngeal carcinoma (NPC) treated with intensity modulated radiation therapy with one-step nodal clinical target volume (CTVn) delineation by geometric-anatomic expansion from the nodal gross target volume (GTVn). METHODS AND MATERIALS: Patients with CLN+ NPC treated with the same one-step-CTVn delineation in two Chinese academic centers were pooled for this study. GTVn was prescribed to 70 Gy equivalent, CTVn1 was omitted, CTVn2 was prescribed to 45 to 55 Gy equivalent, and defined as GTVn + 3 mm geometr

Evaluation of pathological diagnosis using ultrasonography-guided lymph node core-needle biopsy.

BACKGROUND: Image-guided core-needle biopsy as a minimally invasive procedure has partially replaced excisional biopsy of the lymph node. However, it is still a great challenge to pathologists. The aim of this study was to survey and evaluate the accuracy of pathological diagnosis using the ultrasonography (US)-guided core-needle biopsy (CNB) of the lymph node. METHODS: Lymph node CNBs of 1119 consecutive patients from the Chinese People's Liberation Army (PLA) General Hospital were reviewed retrospectively. Biopsies were performed following outpatient procedures with direct US guiding by usi

Multicentricity of Type II First Branchial Cleft Anomalies.

OBJECTIVES: First branchial cleft anomalies (FBCAs) are the result of misguided embryologic development of the external auditory canal, resulting in a partial duplication of the structure. A recent consensus statement by the International Pediatric Otolaryngology Group (IPOG) reclassified lesions based on anatomic location rather than the classic germ cell layer classification. Type I FBCAs are external and superficial to the parotid fascia, and type II FBCAs are juxtaposed to, or involving, the parotid gland. The consensus statement supports extensive resection to prevent recurrence. We hypot

Influence of amplitude-related perfusion parameters in the parotid glands by non-fat-saturated dynamic contrast-enhanced magnetic resonance imaging.

PURPOSE: To verify whether quantification of parotid perfusion is affected by fat signals on non-fat-saturated (NFS) dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) and whether the influence of fat is reduced with fat saturation (FS). METHODS: This study consisted of three parts. First, a retrospective study analyzed DCE-MRI data previously acquired on different patients using NFS (n = 18) or FS (n = 18) scans. Second, a phantom study simulated the signal enhancements in the presence of gadolinium contrast agent at six concentrations and three fat contents. Finally, a prospecti

Sympathetically maintained pain presenting first as temporomandibular disorder, then as parotid dysfunction.

Complex regional pain syndrome (CRPS) is a chronic condition that usually affects extremities, such as the arms or legs. It is characterized by intense pain, swelling, redness, hypersensitivity in a region not defined by a single peripheral nerve and additional sudomotor effects, such as excessive sweating. The clinical criteria for the diagnosis of sympathetically maintained pain as outlined by the International Association for the Study of Pain include: Onset following an initiating noxious event (CRPS-type I) or nerve injury (CRPS-type II). Spontaneous allodynia that is not limited to perip

Mandibular Metastasis from Undiagnosed Hepatocellular Carcinoma - A Case Report.

RATIONALE: Cancer metastatic to the oral cavity accounts for 1%-4% of all oral malignancies, spreading from a distant primary site. Nevertheless, oral metastasis is often the first presentation of primary cancer. PATIENT CONCERNS: A 53-year-old male was referred to our institution because of painful swelling of the left parotid region that did not respond to medications. A firm mass extending towards the mandbular angle had caused trismus; furthermore, the patient complained about hypoesthesia in the V3 area. No noticeable findings could be seen intraorally. DIAGNOSIS: Computed tomography im…

Spontaneous malignant myoepithelioma in the parotid gland of an aged female Crl:CD (SD) rat.

An aged female Crl:CD (SD) rat developed a subcutaneous tumor in the left occipital region. Histologically, the tumor was predominantly composed of spindle-shaped cells proliferating in a loosely cohesive fascicular arrangement in a basophilic myxoid matrix with a minor population of epithelioid cells. Metastases were detected in the cervical lymph nodes, kidneys, lungs, and the pleural cavity. Duct-like structures, considered to be entrapped normal salivary ducts, were scattered within the tumor. Immunohistochemically, the tumor cells were diffusely positive for vimentin, focally positive for

Pediatric Erythroid Sarcoma Diagnostically Confirmed by Identification of a Recurrent NFIA::CBFA2T3 Fusion.

Erythroid sarcoma (ES) is exceedingly rare in the pediatric population with only a handful of reports of de novo cases, mostly occurring in the central nervous system (CNS) or orbit. It is clinically and pathologically challenging and can masquerade as a nonhematopoietic small round blue cell tumor. Clinical presentation of ES without bone marrow involvement makes diagnosis particularly difficult. We describe a 22-month-old female with ES who presented with a 2-cm mass involving the left parotid region and CNS. The presence of crush/fixation artifact from the initial biopsy made definitive cla…

Maxillofacial Surgery Within the Archive of the Greek Surgeon of the 19th Century Theodoros Aretaios (1829-1893).

One of the most important figures of the Hellenic surgery of the 19th century, professor of the Othonian University of Athens, Theodoros Aretaios (1829-1893), portrays in his personal archives a series of surgical operations in the field of maxillofacial surgery. During his career, he operated the following surgical diseases, these are adenosarcomas or inosarcomas of the parotid region, osteofibroma of the sinus antrum, osteosarcomas of the upper and lower jaw, and lycostoma (cleft palate). He was able to perform radical enucleations of the tumorous masses. He reconstructed the area as a plast…

Dentinogenic Ghost Cell Tumor in a Sumatran Rhinoceros.

An adult female Sumatran rhinoceros was observed with a swelling in the left infraorbital region in March 2017. The swelling rapidly grew into a mass. A radiograph revealed a cystic radiolucent area in the left maxilla. In June 2017, the rhinoceros was euthanized. At necropsy, the infraorbital mass measured 21 cm × 30 cm. Samples of the infraorbital mass, left parotid gland, and left masseter muscle were collected for histopathology (Hematoxylin & Eosin, Von Kossa, Masson's trichrome, cytokeratin AE1/AE3, EMA, p53, and S-100). Epithelial-myoepithelial carcinoma ex-pleomorphic adenoma of the parotid gland: report of a rare case with immunohistochemical and genetic analyses.

Epithelial-myoepithelial carcinoma (EMCa) is a rare low-grade salivary malignancy. It is rare for EMCa to occur as the carcinomatous component of carcinoma ex-pleomorphic adenoma (PA). We examined one additional case of EMCa ex-PA, immunohistochemically and genetically. The patient was an 83-year-old female, who suffered from swelling of the right parotid region. Histologically, the tumor contained a hyalinized nodule, which displayed elastosis. The main tumor exhibited a bi-layered structure, involving inner ductal cells and clear outer myoepithelial cells. Immunostaining indicated that the i…

MYB-NFIB Translocation by FISH in Adenoid Cystic Carcinoma of the Head and Neck in Nigerian Patients: A Preliminary Report.

Adenoid cystic carcinoma (AdCC) is a relatively rare malignancy of head and neck sites such as the salivary glands, lacrimal gland, sinonasal region, and pharynx and may arise in other exocrine glands. The oncologic event in AdCC is the translocation between MYB proto-oncogene transcription factor (MYB) and nuclear factor I/B (NFIB) resulting in t(6;9)(q22-23;p23-24). We carried out a preliminary evaluation of MYB-NFIB translocation by fluorescence in-situ hybridization on seven archived formalin-fixed paraffin-embedded tissues of AdCC of Nigerian patients and its clinicopathologic features. Pleomorphic Adenoma of Minor Salivary Gland Arising de novo in the Parapharyngeal Space- A Rare Case Report.

Parapharyngeal space tumours are rare. Pleomorphic adenoma is the commonest salivary gland tumour that is found in this space. This tumour may arise in the deep lobe of parotid gland and extend into the parapharyngeal space or may arise de novo from the aberrant minor salivary glands in parapharyngeal space. The latter entity is an extremely rare finding. CT scan and fine-needle aspiration cytology form the important diagnostic tools in case of these tumours. Surgery is the mainstay for the treatment of these tumours. The strategic location and also the extension of these tumours may at times

Solitary Fibrous Tumor of Nasal Cavity: A Case Report.

INTRODUCTION: Solitary fibrous tumours (SFTs) of the nose and paranasal sinuses are extremely rare. These were originally described as neoplasms of the pleura originating from spindle cells. It is further sub-classified as a benign type of mesothelial tumour. Its occurrence in many extra pleural sites have been reported earlier, mainly in the liver, parapharyngeal space, sublingual glands, tongue, parotid gland, thyroid, periorbital region, and very occasionally in the nose and paranasal sinus area. CASE REPORT: A 28-year-old man with a 6 month history of persistent progressive left nasal obs

Ipsilateral keratoconus associated with long-standing primary hydatid cyst of the orbit.

Hydatid cyst is a cyclozoonotic infection of the larvae form of a platyhelminthes Echinococcus granulosus. The majority of hydatid cysts appear in the liver (65%) and lungs (25%). Kidneys and brain are other less common sites for this disease. Only 1% to 2% cases are seen in the maxillofacial region. These commonly appear as cystic lesions located in the mandible, maxillary sinus, orbit, infratemporal fossa, pterygopalatine fossa, parapharyngeal space, tongue, and parotid and submandibular salivary gland. Hydatid cysts of the orbit are rare and account for 1% of all hydatid cysts. The article

The electrogenic Na+-HCO3- cotransporter NBCe1-B is regulated by intracellular Mg2+.

NBCe1-B, a major splice variant of the electrogenic Na+--HCO3- cotransporter (NBCe1) fulfills basic cellular functions including regulation of intracellular pH and epithelial HCO3- secretion. However, its cellular regulatory mechanism still remains elusive. Here, we provide evidence for the first time that NBCe1-B activity can be controlled by intracellular Mg2+ (Mg2+(i)), the physiologically most abundant intracellular divalent cation. Using the whole-cell patch-clamp technique, we found that recombinant NBCe1-B currents expressed in HEK293 and NIH3T3 cells were inhibited voltage-independentl
