Manometric diagnosis of Hirschsprung's disease in the neonatal period.

A newly designed semiconductor manometer was assembled for anorectal manometry in the neonatal period. Sixty apparently healthy neonates and 17 patients who presented gastrointestinal obstructive symptoms were examined by the eighth day of life. All 60 apparently healthy neonates showed a normal fluctuating wave and rectoanal reflex. Prematurity and postnatal age do not influence the normal rectoanal reflex. Among 17 patients, 5 were diagnosed as having Hirschsprung's disease based on absence of the reflex. There were no false negative or false positive results among these cases. It appears th

[Risk of chickenpox during pregnancy and the neonatal period].

A newborn fell ill from congenital chickenpox eight days after delivery, a chickenpox rash having been noted in the mother two days before delivery. Administration of zoster-hyperimmune serum to the mother at the onset of the rash and to the child immediately after birth had no demonstrable effect on the course of the disease. Autopsy, the baby having died on the eleventh day post-partum, revealed focal necrotizing pneumonia of all the lobes. Varicella-zoster virus was isolated from lung material. On the basis of present knowledge the risk of varicella infection in the fetus and neonate is 1-7

[Periodic laryngeal dyspnea in bi-opercular syndrome].

Periodic bouts of laryngeal dyspnea in a 77-year-old man were observed early in the course of a syndrome due to bilateral opercular infarcts. Laryngoscopy showed alternate slow opening and closure of the glottis superimposed on respiratory vocal cord movements. The disorder could result from bilateral removal of cortical control of bulbar centers, its periodicity suggesting analogies with Cheyne-Stokes' dyspnea.

Diagnosis and management of cardiac arrhythmias in the postoperative period.

Various rhythm disturbances occur in the postoperative patient. Proper management requires an awareness of clinical circumstances in which they are most likely to happen. Often, the appearance of new arrhythmias in the postoperative period is a manifestation of underlying remediable medical problems. Direct antiarrhythmic therapy is unnecessary in many patients when these precipitating conditions are properly treated.

The relationship of insulin receptors to hypokalemic periodic paralysis.

Single muscle fibers from a typical case of hypokalemic periodic paralysis (HOPP) have been found to be depolarized in all test media and to be cathodally blocked after exposure to insulin. The diseased fibers also bind more insulin and consume more oxygen than controls. The hypothesis proposed is that increased amount or affinity of insulin binding on the HOPP muscles causes continual depolarization in the presence of normally subthreshold concentrations of circulating insulin. presence of normally subthreshold concentrations of circulating insulin. This is because of steady effects of the ho

[CNS tumors of the perinatal period (author's transl)].

23.4% of CNS tumors which occur during the first year of life show a close time relationship to the perinatal period. These tumors are subdivided in 3 groups: neoplasm in stillbirths, those with histological verification in the first week of life, and those with symptoms since the first week of life and later on performed histological investigation. Most of the intracranial tumors are localized supratentorially. Teratomas predominate, their relative frequency decreases with increasing age. 36.2% of the spinal tumors in infancy are time-related to the perinatal period, most of them are lipomas.

Ultrasound screening for abdominal masses in the neonatal period.

A total of 36 patients, 6 weeks of age or younger, were examined by ultrasound for a suspected abdominal mass during the four-year period between May 1977 and May 1981. A total of 46 abdominal masses were detected in 29 patients. The ultrasound diagnosis was correct or directly contributory to the final diagnosis in 96% (43/45) of the cases. The diagnosis was incorrect or misleading in only 4% (2/45) of the cases. When no pathologic mass was demonstrated by ultrasound, this observation was confirmed to be correct in every case. The study was safe, reliable, and quickly performed with no discom

Periodic EEG complexes in infectious mononucleosis encephalitis.

The presence of periodic EEG complexes in patients with an acute viral encephalitis is generally held to suggest that infection is due to herpes simplex. We now report a patient with clinical and laboratory findings of infectious mononucleosis, and neurologic involvement manifested by lymphocyte meningitis, coma, seizures, aphasia, hemiparesis and hemianopsia. Serial EEGs showed periodic, predominantly left-sided slow wave complexes occurring every 4 to 5 seconds, which disappeared with clinical resolution of the illness. In view of our findings and the similar findings reported previously by

Acute effects of acetazolamide in hyperkalemic periodic paralysis.

We studied the effect of acetazolamide on plasma potassium in normals and in two patients with hyperkalemic periodic paralysis. Administration of acetazolamide for 48 hours lowered mean plasma potassium in normals from 4.01 to 3.56 mEq per liter (p less than 0.001) and in the patients from 4.55 to 4.00 mEq per liter (p less than 0.001). This kaliopenic effect of acetazolamide may account for its therapeutic action in hyperkalemic periodic paralysis.

[Periodic paresis due to hypokalaemia (author's transl)].

A case of periodic paralysis is reported that had to be treated by mechanical ventilation. Symptoms and course of this attack are demonstrated and it is shown how diagnosis was reached. Provocative factors of periodic hypokaliemic paralysis are discussed and their importance in anaesthesia is emphasised.

Sudden death in the six-month postinfarction period.

Complex ventricular arrhythmias that appear during convalescence following an MI and up to a period of 6 months post MI are known to increase the risk of sudden death. The majority of subjects with this complication have above-normal left ventricular filling pressures, a hemodynamic measure of congestive heart failure. In this clinical setting, it is therefore important to consider and search for evidence of congestive heart failure. Drug therapy for arrhythmias is more effective in patients whose congestive heart failure has been adequately treated. Not infrequently, after cardiac compensatio
