The relationship of plasma imipramine and N-desmethylimipramine to improvement in agoraphobia.

Plasma tricyclic concentrations were assessed in 15 agoraphobic patients receiving combined imipramine and behavioral treatments. Imipramine but not N-desmethylimipramine plasma levels were found to significantly correlate with improvement. The results suggest that imipramine's effect in agoraphobia might be mediated predominantly through the serotonergic action of the parent drug. There was also suggestive evidence for differential antipanic and antiphobic-antidepressant effects. Implications for future studies of the mechanism of action of drug effects in agoraphobia are discussed.

Sensory experiences during treatment of phobias by in vivo exposure.

The author describes several patients undergoing in vivo exposure therapy for simple phobias who reported transient sensory distortions without other evidence of psychosis. The experiences occurred mainly in the tactile, kinesthetic, and visual modes. Their nature and significance are undetermined, but they appear to resemble conversion symptoms, hypnotic sensory distortions, and other nonpsychotic sensory experiences stimulated by emotionally charged ideas.

Incidence of DIS/DSM-IV social phobia in adults.

OBJECTIVE: The aim of the study was to estimate the incidence of social phobia in the general population. METHOD: The Baltimore cohort of 3481 subjects, sampled during the 1981 Epidemiologic Catchment Area study, was traced. A total of 1920 subjects were re-interviewed from 1993 to 1996 using the Diagnostic Interview Schedule (DIS). A subsample of 349 subjects was interviewed by psychiatrists using the Schedules for Clinical Assessment in Neuropsychiatry. RESULTS: The estimated incidence of DIS/DSM-IV social phobia is 4-5/1000/year. New cases were found in all age groups, with the highest ra

Characteristics of worry in GAD patients, social phobics, and controls.

Phenomenological features of worry such as thought content, subjective experience of worry, and efforts to control were investigated in the present interview study, as well as retrospective information about possible origins. To examine the clinical specificity of worrying in Generalized Anxiety Disorder (GAD), 36 GAD patients were compared to a normal control group (N = 30) and to a clinical control group (N = 22 social phobics). GAD patients differed from both groups in having higher frequency of worry, higher number of different worry topics, lower subjective controllability, more accompany

Predictors of quality of life in a long-term followup study in panic disorder patients after a clinical drug trial.

In a prospective 4 year followup study, 423 panic disorder patients participated in one of two international multicenter drug trials. They were to be reinterviewed 4 years after leaving the trials; 367 (86.8%) interviews were completed. At followup, 3 of 5 patients still suffered from at least occasional panic attacks, and 2 of 5 were still agoraphobic, but only about 20 percent of the patients were still disabled. The only baseline variables that predicted disabilities at followup were the disability measures at baseline themselves. Neither panic attack frequency nor phobic avoidance at basel

Exposure to potentially traumatic events in early childhood: differential links to emergent psychopathology.

OBJECTIVE: To examine associations between exposure to potentially traumatic events (PTEs) and clinical patterns of symptoms and disorders in preschool children. METHOD: Two hundred and thirteen referred and non-referred children, ages 24 to 48 months (MN = 34.9, SD = 6.7 months) were studied. Lifetime exposure to PTEs (family violence and non-interpersonal events) and recent stressful life events were assessed with the Preschool Age Psychiatric Assessment (PAPA) and Child Life Events Scale. Child psychiatric symptoms and disorders were assessed with parent-reports in the PAPA, a comprehensiv

Clinical anxiety and its treatment.

Anxiety occurs as a normal phenomenon and as a symptom of numerous psychiatric and physical illnesses, while it is the dominant feature of generalised anxiety, panic and phobic disorders. The relationship between these and other disorders is not as straightforward as diagnostic classifications suggest, and this is at least in part due to previous research having been carried out on atypical cases referred to hospital rather than patients in the real world of general practice and the general community. There are particular difficulties in distinguishing anxiety disorders from depressive illness…

Long-term goals in the management of acute and chronic anxiety disorders.

Many anxiety disorders are not treated to remission (symptom-free state); however, this should be the minimum goal of therapy. Antidepressant therapies have shown significant beneficial effects in the management of anxiety disorders, with some variability in results in specific disorders. In social anxiety disorder, selective serotonin reuptake inhibitors and venlafaxine extended release (XR) have demonstrated efficacy, with response rates varying between 40% and 68%. Monoamine oxidase inhibitors and cognitive-behavioural therapies are also effective. In patients with generalized anxiety disor

Childhood stressful events, HPA axis and anxiety disorders.

Anxiety disorders are among the most common of all mental disorders and their pathogenesis is a major topic in psychiatry, both for prevention and treatment. Early stressful life events and alterations of hypothalamic pituitary adrenal (HPA) axis function seem to have a significant role in the onset of anxiety. Existing data appear to support the mediating effect of the HPA axis between childhood traumata and posttraumatic stress disorder. Findings on the HPA axis activity at baseline and after stimuli in panic disordered patients are inconclusive, even if stressful life events may have a trig

The long-term course of panic disorder and its predictors.

Whereas lifetime prevalence rates of panic disorder--as established in epidemiologic surveys--range between 1.6 and 3.5%, 1-month rates usually amount to much less than one half of the lifetime rates. This finding indicates that a substantial proportion of patients who had panic disorder at some stage in their life must have remitted. In contrast to these results, clinicians tend to regard panic disorder as a chronic condition because, as a rule, they see panic patients only several years after onset of the disorder. A number of small, prospective, long-term studies of such clinical population

Long-term outcome after acute treatment with alprazolam or clonazepam for panic disorder.

The relative effectiveness of the available treatments for panic disorder may best be understood in the context of the longitudinal course of the disorder. This study examines a number of clinically relevant issues, including long-term outcome after acute treatment, the proportion of patients remaining on single-agent treatment or requiring multiple medications or nonpharmacologic interventions over time, evidence for dose escalation during maintenance high-potency benzodiazepine therapy, and predictors of acute and long-term response to treatment. Fifty-nine panic disorder patients originally

Sleep disturbance and psychological distress are associated with functional dyspepsia based on Rome III criteria.

BACKGROUND: Functional dyspepsia (FD) is considered a bio-psychosocial disorder. The role of psychosocial factors in FD pathogenesis remains unclear. METHODS: This study evaluated sleep quality and mood symptoms in patients with FD, assessing the associations of FD severity, disordered sleep, and psychological symptoms. One-hundred-and-fifteen adult patients with typical FD symptoms were enrolled alongside 61 healthy volunteers. Rome III criteria were used to evaluate FD symptoms; sleep disorder was assessed with the Pittsburgh Sleep Quality Index (PSQI), and Symptom Checklist-90-Revised (SCL
