OBJECTIVE: The objective of this study was to determine the relation of plasma zinc (Zn) status to growth and maturation in children with SS genotype sickle cell disease.
STUDY DESIGN: A cross-sectional study of 104 subjects who were 50% female and ranged in age from 0.4 to 18 years was performed. Measures included plasma Zn concentration (Znp), height, weight, skinfold thicknesses, elbow breadth, upper arm muscle area, and fat-free mass and fat mass by total body electrical conductivity. Skeletal maturation was assessed by hand-wrist x-ray evaluation and sexual maturation by Tanner stage.

A case of non-secretory multiple myeloma presenting as primary plasma cell leukaemia in a 65 year old woman is presented. Bone pain was the initial clinical manifestation. Laboratory analysis showed 20% of circulating immature plasma cells. Despite the presence of osteolytic lesions, no M-component could be demonstrated in serum protein electrophoresis, and serum and urine immunoelectrophoresis. Serial measurements of plasma fibrinogen concentrations have been made in nine homozygous sickle cell disease (SCD) patients during vaso-occlusive crisis. Fibrinogen was measured by the clot-weight technique. Changes in fibrinogen concentration showed a typical pattern, rising sharply and significantly to a maximum on approximately the second day of the onset of pain crisis and after treatment was instituted. The clinicopathologic features of two rare cases of plasma cell granuloma of the endocrine glands are described and compared with extrapulmonary cases reported in the literature. One was localized in the adrenal gland and was revealed by amenorrhea and a large inflammatory syndrome; it is the first case reported to our knowledge. The other case developed in the thyroid, was asymptomatic, and represents the third case in the literature.

Two cases of meningeal invasion by non-myelomatous plasma cell dyscrasias--a plasma cell leukaemia and an extramedullary plasmacytoma--are described. Both were secretors of IgD paraprotein and both were diagnosed in life, characteristics which we have not found in any other published case of plasma cell leptomeningitis. A ten-year-old girl presented with an asymptomatic tumor of the lung. X-ray morphology suggested malignancy. Laboratory investigations revealed hypochromic anaemia, strong signs of inflammation, and hypergammaglobulinaemia. By means of surgery the tumor was completely removed. Histologic analysis established the diagnosis of a plasma cell granuloma. Postoperatively, laboratory parameters normalized. During the follow-up period of now 30 months there has been no relapse.

A child had a pulmonary plasma cell granuloma/fibrous histiocytoma that recurred after lobectomy in 1952 and required pneumonectomy in 1953, at which time it extended into the mediastinum at the pulmonary hilus. A metastatic nodule was removed from within the diaphragmatic musculature. The long-term favorable outcome suggests a good prognosis after adequate surgical therapy. The possible relationship of fibrous histiocytoma/plasma cell granuloma to modified measles is discussed.

The clinical and laboratory studies of 6 patients with primary plasma cell leukaemia are described. The leukaemic cells had a variable morphology, ranging from lymphoplasmacytic and mature plasma cells to poorly differentiated blasts.
