The parameters of the general and cerebral hemodynamics were studied in 36 patients with chronic insufficiency of the cerebral circulation following papaverine administration. In patients with the early stages of the disease, papaverine elicited a decrease in the arterial pressure and the peripheral resistance to the blood flow. The amplitude of the rheographic curve increased significantly two minutes after the intravenous injection of papaverine. Oculoplethysmography was used to evaluate 66 patients with transient ischemic attacks prior to cerebral angiography. Fifty-eight (87.9%) symptomatic internal carotid arteries had anatomically significant stenosis. Only 69 per cent of these 58 arteries had positive OPG test. Thirty-one per cent of the arteries were well compensated hemodynamically with collaterals and had a false negative test. A negative OPG test does not rule out an anatomically significant internal carotid artery stenosis.

A vascular surgeon must recognize that strokes may be caused by embolism from or obstruction of the extracranial afferent arteries to the brain. Two hundred fifteen patients with a history of either stroke, transient ischemic attack (TIA), or asymptomatic carotid bruit underwent noninvasive carotid artery testing using oculopneumoplethysmography. Of patients with hemodynamically significant stenosis, 51 (40.8%) underwent endarterectomy, and 74 (59.2%) were treated nonoperatively. The incidence of stroke in the nonoperated group was 12/74 (16.2%) compared with only 1/51 (1.9%) in the operated group. 500 consecutive patients were evaluated for extracranial disease of the internal carotid arteries by an automated, air-filled, digital oculoplethysmographic system (OPG) of the Kartchner type (Zira) and by supraorbital (SO) and supratrochlear (ST) directional Doppler ultrasonography. Patients with asymptomatic carotid bruits require some noninvasive method for detecting which would benefit from angiography and possible prophylactic operative repair of the suspected atherosclerotic lesion. This report describes the use of the ocular pneumoplethysmograph (OPG) in this regard. Data for establishing OPG criteria of significant carotid stenosis were derived from studies in symptomatic patients who had also undergone angiography.
