Roux-en-Y gastric bypass (RYGBP) is an increasingly used procedure when treating morbid obesity. Due to the extensive gastrointestinal rearrangement, diagnostic evaluation of patients with gastric bypass and acute abdominal pain can be difficult. We present a case of a perforated duodenal ulcer in a RYGBP operated patient, where free abdominal fluid, but hardly any pneumoperitoneum was seen on a computed tomography. Free intraperitoneal fluid is an important finding and should give suspicion of the need for emergency surgery in RYGBP operated patients with abdominal pain.

The rising number of traffic accidents increases the number of traumatic ruptures of the diaphragm. Often there are no characteristic symptoms or those of other injuries may dominate. Sometimes a long time elapses before signs of hernia appear. Therefore the mass X-ray screening of the population is very important to demonstrate such cases. Chest X-ray, barium meal and pneumoperitoneum are required to detect a late herniation. Prompt surgical repair will give patients the best chance of recovery, thus avoiding complications which greatly increase the mortality rate.

Generalized peritonitis with sepsis is a common general surgery emergency. A 36-year-old man was diagnosed with polyarteritis nodosa (PAN). He presented with weight loss, hypertension, and mono-neuritis. Pneumoperitoneum and septicaemia developed and laparotomy revealed necrosis of a segment of the ileum. The patient was HBsAg/HBeAg and HBV DNA positive. Liver biopsy showed active cirrhosis. The PAN symptoms remitted upon treatment with prednisolone and lamivudine. Seroconversion from HBeAg to anti-HBe occurred six months later. One year after the end of treatment, the patient was still HBeAg and HBV DNA negative, with normal liver enzymes and no symptoms of PAN.

Plain x-rays of the abdomen remain an integral feature of the diagnostic work-up in cases of acute abdomen. Chest x-rays, often recommended in the USA, are standard routine at only one third of Swedish radiology units. The article outlines the advantages of including chest x-ray in the abdominal examination: it facilitates the detection of pneumoperitoneum, it yields (pre-operative) cardiopulmonary information, and it may show abdominal pain to be of extra-abdominal aetiology. Regarding the latter point, a study of 555 cases showed the chest x-ray to yield valuable information in 14 cases.
