Tonsil and adenoid surgery for upper airway obstruction in children.

We studied the outcomes of 76 children, aged 3 to 12 years, with large tonsils and/or large adenoids who underwent surgery to relieve upper airway obstruction over a 1-year period. Following surgery, nearly all patients experienced an alleviation of all symptoms, except for enuresis. We suggest that children who have large tonsils and/or adenoids will gain substantial benefit if they are removed, even children who do not have a history of severe sleep apnea or objective evidence from polysomnography.

Sleep apnoea and organic solvent exposure.

Fifteen patients referred for the evaluation of possible organic solvent encephalopathy were studied by clinical polysomnography. Seven had more than 30 apnoeas per night and an apnoea index of higher than 5, thus fulfilling the commonly used criteria of the sleep apnoea syndrome. Another group of eight workers exposed to trichlorethane, examined without prior knowledge of their individual symptoms, showed a significantly elevated number of sleep apnoeas compared with nine controls. The results indicate that organic solvent exposure can cause sleep apnoea.

Excessive fragmentary myoclonus in NREM sleep: a report of 38 cases.

We report 38 consecutive patients referred to a sleep disorder clinic who on diagnostic polysomnography showed excessive amounts of brief fragmentary myoclonus throughout all stages of NREM sleep. Almost all patients were male despite a reasonably equal sex distribution of referral. The phenomenon was found associated with sleep-related respiratory problems, periodic movements in sleep (PMS), narcolepsy, intermittent hypersomnia and (rarely) insomnia. It also occurred associated with excessive daytime sleepiness (EDS) as an isolated polysomnographic finding apart from some degree of sleep frag…

Eye movements in REM sleep could be controlled by the same conjugate system while awake.

We describe the impairment of eye movements during REM sleep in a patient with internuclear ophthalmoplegia. A 75-year-old woman, complaining of acute unsteady gait and diplopia, was diagnosed to have right medial longitudinal fasciculus (MLF) syndrome with limited adduction of the right eye. Overnight polysomnography revealed the same impairment of right adductive ocular movement during REM sleep. Our findings suggest that the MLF mediates lateral conjugation of eye movements during REM sleep as well as voluntary lateral conjugate eye movements during wakefulness.

Effect of naloxone on diurnal polysomnographic manifestations of hypersomnia with sleep apnoea.

A simple blind study with small doses of naloxone (0.8-1.6 mg i.v.) was carried out in 11 patients with hypersomnia with sleep apnoea (HSA). The effect was studied by diurnal polysomnography. It was found that the administration of naloxone was followed by significant prolongation of wakefulness and by significant shortening of the total duration of the second stage of NREM sleep. The duration of the apnoeic episodes was also significantly shortened after naloxone, although their number did not alter. Increased activity of the endorphinergic system (which naloxone inhibits by receptor competit…

Periodic leg movements in sleep: effect of clonazepam treatment.

Periodic leg movements in sleep were diagnosed in 8 patients with insomnia and in 12 patients with hypersomnia. Polysomnography did not reveal any difference in the number of leg movements in these groups, the mean duration of a leg movement, or the number of activity epochs. Those with insomnia, however, had a shorter intermovement interval than those with hypersomnia, with more leg movements in each activity epoch. Treatment with 0.5 to 2 mg clonazepam improved subjective complaints and decreased the number of leg movements without affecting the intermovement interval or movement duration.

Obesity-associated sleep hypoventilation and increased adverse postoperative bariatric surgery outcomes in a large clinical retrospective cohort.

STUDY OBJECTIVES: Although obesity hypoventilation syndrome (OHS) is associated with increased morbidity and mortality, post-bariatric surgery OHS risk remains unclear due to often nonsystematic OHS assessments. METHODS: We leverage a clinical cohort with nocturnal CO2 monitoring during polysomnography to address the hypothesis that patients with obesity-associated sleep hypoventilation (OaSH; ie, stage II OHS) have increased adverse postoperative bariatric surgery outcomes. We retrospectively analyzed data from patients undergoing pre-bariatric surgery polysomnography at the Cleveland Clinic…

Consensus roadmap on periodic leg movements during sleep: outcomes of the international PLMS taskforce.

Periodic leg movements during sleep (PLMS) are a frequently observed motor phenomenon in polysomnography, yet their biological meaning and clinical relevance are debated. Further, how these should be assessed and tabulated for optimal evaluation is uncertain. To address these gaps, an international, multidisciplinary taskforce convened a series of online meetings followed by an in person taskforce workshop held in Troina, Italy, on November 14-15, 2025, where agreement was reached through iterative discussion (qualitative; no Delphi or predefined voting thresholds). This taskforce roadmap summ…

Dise in osa: valuable tool or an overestimated technique? a systematized review.

PURPOSE: Drug-Induced Sleep Endoscopy (DISE) is increasingly used to evaluate upper airway dynamics in obstructive sleep apnea patients (OSA … cal localization of upper airway collapse and treatment planning in OSA. However, it cannot replace polysomnography as the diagnostic gold standard and should be interpreted within a comprehensive clinical context. Further standardization and high-quality studies are needed to better define its role and optimize its clinical utility.

Idiopathic sleep terror in an adult patient.

We report a case of a man in his 40s presenting with idiopathic adult-onset sleep terrors manifesting as nocturnal violent behaviour, including a concerning episode of attempted strangulation of his bed partner. Polysomnography documented an event during N3 sleep (slow-wave, deep non-rapid eye movement) with autonomic hyperactivation and complex motor behaviour. A detailed psychiatric evaluation did not reveal any diagnosable anxiety, mood or other psychiatric disorder, and there was no history of substance use, recent psychosocial stressors or sleep deprivation. Notably, there were no precipi…

Understanding Sexual Parasomnias: A Review of the Current Literature on Their Nature, Diagnosis, Impacts, and Management.

Sexual behavior during sleep, known as sexual parasomnias, has captured the interest of researchers and clinicians. These parasomnias involve various sexual activities that occur unconsciously during sleep. Although relatively rare, they can profoundly affect well-being and relationships and can carry legal consequences. Understanding their nature, prevalence, and causes is crucial for advancing knowledge in this field. This article revisits the topic of sexsomnia, presenting new data and discussing cases published from 2007 to 2023. By analyzing these cases, we aim to enhance recognition, dia

SleepContextNet: A temporal context network for automatic sleep staging based single-channel EEG.

BACKGROUND AND OBJECTIVE: Single-channel EEG is the most popular choice of sensing modality in sleep staging studies, because it widely conforms to the sleep staging guidelines. The current deep learning method using single-channel EEG signals for sleep staging mainly extracts the features of its surrounding epochs to obtain the short-term temporal context information of EEG epochs, and ignore the influence of the long-term temporal context information on sleep staging. However, the long-term context information includes sleep stage transition rules in a sleep cycle, which can further improve

Jointly modeling of sleep variables that are objectively measured by wrist actigraphy.

Recently developed actigraphy devices have made it possible for continuous and objective monitoring of sleep over multiple nights. Sleep variables captured by wrist actigraphy devices include sleep onset, sleep end, total sleep time, wake time after sleep onset, number of awakenings, etc. Currently available statistical methods to analyze such actigraphy data have limitations. First, averages over multiple nights are used to summarize sleep activities, ignoring variability over multiple nights from the same subject. Second, sleep variables are often analyzed independently. However, sleep varia

Characterization of specific spatial functional connectivity difference in depression during sleep.

Depression is a common mental illness and a large number of researchers have been still devoted to exploring effective biomarkers for the identification of depression. Few researches have been conducted on functional connectivity (FC) during sleep in depression. In this paper, a novel depression characterization is proposed using specific spatial FC features of sleep electroencephalography (EEG). Overnight polysomnography recordings were obtained from 26 healthy individuals and 25 patients with depression. The weighted phase lag indexes (WPLIs) of four frequency bands and five sleep periods we…

Screening for Excessive Daytime Sleepiness and Diagnosing Narcolepsy.

Sleep disorders such as narcolepsy can cause excessive daytime sleepiness (EDS). However, the diagnosis of narcolepsy is often delayed by years in both children and adults, reducing their quality of life. Clinicians can improve the recognition of EDS and diagnosis of sleep disorders using screening tools such as the Epworth Sleepiness Scale, collecting sleep histories, and using sleep laboratory tests. By following up with patients who present with EDS and continuing to assess until a cause is found, clinicians can lessen the time to diagnosis and the negative impact of narcolepsy by initiatin
