Immunophenotyping of 515 cases of acute lymphoblastic leukemia in China.

Using cell surface markers and a panel of monoclonal antibodies, 515 cases of acute lymphoblastic leukemia (ALL) were immunophenotyped. T cell type ALL (T-ALL), non-T cell type ALL (Non-T-ALL) including common ALL (C-ALL), Null-ALL and B cell type ALL (B-ALL) were found. These major subtypes of ALL were further divided according to their phenotypes in detail. It was noticed that the phenotypes of these subtypes of ALL reflected basically the phenotypes of normal T or B cells at various differentiation stages or certain population of lymphocytes. The diagnosis of cell lineage was more precise w

[Clinical evaluation of severe toxicity in two patients with acute lymphoblastic leukemia receiving outpatient methotrexate therapy].

Methotrexate (MTX) is now widely used for the treatment of acute leukemia and non-Hodgkin lymphoma in the pediatric oncology field and is thought to be one of the key drugs for this treatment. A regimen utilizing high dose MTX (HD-MTX) with leucovorin rescue is being investigated as effective chemotherapy in the patients with these kinds of cancer. Relatively large amounts of MTX (225 mg/m2) are given to such outpatients by intravenous push as a course of maintenance therapy. It is said that those amounts will infuse safely. However, we experienced two serious cases-patients T.H. and M.Y.--whi

Radiotherapy of the central nervous system in acute leukemia.

The central nervous system (CNS) is a site of occult and overt involvement with acute lymphoblastic leukemia (ALL) in children. Prophylactic treatment of the cranial and spinal meninges can significantly reduce the incidence of CNS relapse. This review addresses the issues associated with the role of radiation therapy in the treatment of the CNS in ALL.

Gating strategy for immunophenotyping of leukemia and lymphoma.

Clinical specimens of blood, bone marrow, lymph node, extranodal tissue, and body fluid were collected from 67 cases of hematologic neoplasms (including chronic lymphoid leukemias, T- and B-cell lymphomas, and acute lymphoblastic and myelogenous leukemias) for comparison between the right-angle light scatter (RALS)/CD45 and the forward-angle light scatter (FALS)/RALS gating combinations. One to three diagnostic markers were selected from each case, yielding 124 paired results for comparison. We found that the percentage of tumor cell isolation and the total cell count in the tumor cell gate we

Extramedullary CD20-positive B-lymphoblastic lymphoma in a 5-year-old child: A diagnostic challenge.

Lymphoblastic leukaemia/lymphoma may present as an isolated extramedullary mass, which includes the musculoskeletal region involvement with normal or near-normal blood counts. The tumour may be in the form of B or T-lymphoblastic leukaemia/lymphoma. The clinical features and histological morphology of extramedullary B-lymphoblastic lymphoma (B-LBL) may mimic mature B-cell neoplasms, thus posing a diagnostic challenge. Arriving at the right diagnosis is crucial because these two diseases differ in their prognosis and management. A high index of suspicion is therefore important so as not to miss

Adult Patients with Philadelphia-Positive B-Cell Acute Lymphoblastic Leukemia Treated with a Pediatric-Inspired Multiagent Chemotherapy Regimen, in Combination with a TKI, Do Not Require Routine alloSCT.

Tyrosine kinase inhibitors (TKIs) added to chemotherapy have improved outcomes of adult patients with Philadelphia-positive B-cell acute lymphoblastic leukemia (Ph+ B-ALL). These improvements initially led to a larger proportion of patients realizing allogeneic stem cell transplantation (alloSCT), long considered essential for cure, but there has been a re-evaluation of alloSCT. At Princess Margaret Hospital (PM), adult patients with Ph+ B-ALL have been treated with a pediatric-inspired chemotherapy protocol with mostly imatinib. In the last two decades, we have witnessed many iterative change

Comparative Analysis of Treatment Outcomes and Prognostic Factors in Early T-Precursor Versus Non-ETP Acute Lymphoblastic Leukemia Following Allogeneic Hematopoietic Stem Cell Transplantation in Adolescents and Young Adults.

This study evaluates the outcomes and risk factors associated with allogeneic hematopoietic stem cell transplantation (allo-HSCT) in adolescents and young adults (AYA) diagnosed with early T-precursor (ETP) acute lymphoblastic leukemia (ALL) compared to non-ETP ALL.We retrospectively analyzed 178 AYA patients with T-ALL who were treated between September 2014 and December 2022. Out of 178 patients, 128 underwent allo-HSCT, with a median age of 25 years (range: 18-35). Among them, 49 were in the ETP group and 79 in the non-ETP group. Neutrophil and platelet engraftment times were comparable bet

Pretransplant Blinatumomab Improves Outcomes in B Cell Acute Lymphoblastic Leukemia Patients Who Undergo Allogeneic Hematopoietic Cell Transplantation.

BACKGROUND: Blinatumomab, a bispecific monoclonal antibody, effectively controls refractory B cell acute lymphoblastic leukemia (ALL) and promotes measurable residual disease (MRD) negativity. This study investigated the impact of pretransplant blinatumomab on allogeneic hematopoietic cell transplantation (HCT) outcomes in B cell ALL patients. METHODS: We analyzed the effect of pretransplant blinatumomab on transplant outcomes of 117 adults undergoing allogeneic HCT for B cell ALL at Princess Margaret Hospital, Toronto, between 2010 and 2021. Outcomes assessed included overall survival (OS),

Prognostic factors of children and adolescents with T-cell acute lymphoblastic leukemia after allogeneic transplantation.

Acute lymphoblastic leukemia (ALL) is the most common cancer during childhood, and some high-risk patients with ALL require hematopoietic stem cell transplantation (HSCT). Mainly due to small patient numbers, studies focusing specifically on children and adolescents with T-cell ALL (T-ALL) are limited. Using a nationwide registry, we retrospectively analyzed data from patients under 20 years old who underwent their first HSCT for T-ALL between 2000 and 2018. As a result, total 484 patients were included, and their median follow-up period was 6.9 years after HSCT for survivors. While patients r

Differential expression of cyclin-dependent kinase 6 in cortical thymocytes and T-cell lymphoblastic lymphoma/leukemia.

Cyclin-dependent kinase-6 (CDK6) is the earliest inducible member of the CDK family in human T lymphocytes, involved in growth factor stimulation and cell cycle progression. CDK6 is one of the targets of p16 and p15, CDK inhibitors encoded by MTS1 and MTS2, two tumor suppressor genes that are frequently deleted in T-cell leukemia. In this study we have investigated CDK6 expression in normal and neoplastic lymphoid tissues using immunohistochemistry and flow cytometry. In normal (six samples) and hyperplastic (four samples) thymuses, strong CDK6 expression was observed in a discrete proportion

Ibrutinib is not an effective drug in primografts of TCF3-PBX1.

AIM: The Bruton's tyrosine kinase (BTK) inhibitor Ibrutinib (PCI-32765) is effective in patients with multiple myeloma, non-Hodgkin lymphoma and chronic lymphoblastic leukemia. We previously showed that primary cells of children with TCF3-PBX1 positive B-cell precursor acute lymphoblastic leukemia (BCP-ALL) express BTK and are sensitive to ibrutinib in vitro. However, preclinical studies in mice are lacking that justify clinical implementation. METHODS: Immunocompromised NSG mice were engrafted with a luciferase-positive TCF3-PBX1 leukemic cell line or primary leukemic cells and treated with

B-1 progenitor acute lymphoid leukemia.

In this issue of Blood, Yin et al demonstrated that engineered BCL6 interacting corepressor protein (Bcor) mutation in hematopoietic stem/progenitor cells of sensitized mice induced acute lymphoid leukemia (ALL) of B-1 progenitor phenotype, which has been recently identified by them. The intriguing aspect of this report is the complete penetrance of B-1 progenitor leukemia by Bcor mutation, which selectively developed among other various progenitors in the bone marrow (BM).

Catheter-related bloodstream infection by a newly proposed "Mycobacterium toneyamachuris" in a boy with B-cell precursor acute lymphoblastic leukemia.

This report describes a 6-year-old boy who developed non-tuberculosis mycobacteria (NTM) catheter-related bloodstream infection (CRBSI) during treatment for B-cell precursor acute lymphoblastic leukemia. A Hickman catheter was inserted before starting treatment. He developed a fever during chemotherapy, and blood culture was drawn from the catheter. Blurred Gram-positive bacilli were found in Gram stain, looked positive in Ziehl-Neelsen stain, and were identified by matrix-assisted laser desorption/ionization-time of flight mass spectrometry as Mycobacterium mucogenicum phocaicum group.
