Perioperative complications of intraventricular neuroendoscopy: a 7-year experience.

AIM: Despite being a minimally invasive procedure, serious perioperative complications are reported during neuroendoscopy, largely generated by its unique surgical maneuvers. We report here the complications of elective neuroendoscopic surgery for the treatment of hydrocephalus and other intraventricular pathology in 298 patients over a 7-year period at our institute. MATERIAL AND METHODS: The complication rate was determined by recording intraoperative hemodynamic variables, core temperature, bleeding episodes, time to arouse from anesthesia, serum electrolytes and neurological deterioration

Comparison of visual acuity measurements and Purkinje's vessel shadow perception for prediction of postoperative visual acuity in different ophthalmological diseases.

PURPOSE: Prediction of postoperative visual acuity (VA) is extremely important to the patient and highly relevant to the surgeon. However, objective evaluation of the macula is frequently impossible in cases such as mature cataract, cataract in high myopia or vitreous haemorrhage. This study compares different preoperative examination techniques used to predict postoperative VA. METHODS: We retrospectively evaluated the charts of all patients who underwent any of the following procedures at our hospital in 2004: phacoemulsification for mature cataract or cataract in high myopia; vitrectomy fo

Effects of Burch colposuspension on the relative positions of the bladder neck to the levator ani muscle: An observational study that used magnetic resonance imaging.

OBJECTIVE: The purpose of this study was to use magnetic resonance imaging to observe the changes of the position of the levator ani muscle relative to the bladder neck after Burch colposuspension. STUDY DESIGN: Women with urodynamic stress incontinence underwent Burch colposuspension. Magnetic resonance imaging was performed 1 week before surgery and 1 year after the operation. The levator ani muscle and the bladder neck were imaged in the parasagittal and sagittal planes, respectively; and their position was measured in relation to the pubococcygeal line. The reproducibility of these measur

Circulating inhibin forms in patients with hydatidiform mole.

Circulating inhibin forms have not been evaluated in patients with hydatidiform mole (HM) before evacuation of the mole. Serum inhibin A, inhibin B, pro-alphaC and hCG levels were determined by enzyme-linked immunosorbent assay in HM before and after evacuation of the mole (inhibin B only before evacuation). Before evacuation, inhibin B levels in HM were undetectable, and inhibin A levels were similar to those in normal women at the corresponding weeks of pregnancy. Pro-alphaC and hCG levels were higher in molar pregnancy than in normal pregnancy. After evacuation, pro-alphaC levels significan

The kidney in liver transplantation.

Renal dysfunction often complicates the course of liver transplant recipients. Preoperative renal dysfunction, including hepatorenal syndrome (HRS) may be present. Assessment of renal function in the pretransplant patient with end-stage liver disease is fraught with pitfalls. Direct measurement of GFR by a method other than creatinine clearance is recommended wherever possible. Preoperative renal biopsy should also be considered in those patients with renal dysfunction in whom the diagnosis of HRS is not definite. With the routine use of veno venous bypass, renal perfusion is maintained and in

Urodynamics in patients with pheochromocytoma: a peri-operative study of 10 cases.

Alpha receptors have been demonstrated in the bladder neck, and urinary retention may be the presenting symptom in an occasional pheochromocytoma patient. This prompted us to define the urodynamic profile in pheochromocytoma patients. Ten patients were studied. Except for 2 patients, all tumors secreted norepinephrine either alone (n = 4) or mixed (n = 4). Urodynamic studies (uroflowmetry, cystometry, profilometry, response to alpha-adrenergic agents) were performed with Urodyn 5000 chain (DANTEC) connected to a water perfused Bohler's catheter. Profilometry was done according to the Brown and

[Holter electrocardiographic findings in surgical patients during the perioperative period].

Eighty patients undergoing abdominal surgery were studied to evaluate ECG changes in perioperative period and also identify the factors influencing the incidence and the severity of postoperative ventricular arrhythmia. Holter ECG was recorded with CM5 and NASA leads from the night before operation to the night of the 2nd postoperative day. Tachycardia (greater than or equal to 100 beats.min-1) was found in 46.3% of the patients preoperatively and in 55% postoperatively. Bradycardia (less than or equal to 50 beats.min-1) was found in 30% of the patients mostly in the night prior to the operati

Transesophageal echocardiography in aortopulmonary communications.

The assessment of congenital aortopulmonary communications by transthoracic echocardiography may be suboptimal, particularly postoperatively, due to limited acoustic windows. We performed intraoperative transesophageal echocardiograms in six patients with eight proven systemic-pulmonary communications. Diagnosis included truncus arteriosus (1), aortopulmonary window (1), Waterston anastomosis (3), central Gore-Tex shunt (1), and modified left Blalock-Taussig shunt (2). All communications were accurately demonstrated by transesophageal echocardiography (TEE). The transesophageal technique also

Is evaluation of the right colon necessary prior to cecocystoplasty?

Use of the ileocecal bowel segment in urological surgery has become more widespread. Thorough preoperative evaluation of the bowel is seldom performed. A case is presented in which the intraoperative finding of numerous cecal polyps led to abortion of a planned cecocystoplasty. The literature on fecal occult blood testing and screening methods for colorectal neoplasia was reviewed. Patients are classified into 2 categories, average risk and high risk, according to the relative risk for colorectal neoplasia. The data suggest that the fecal occult blood test, if properly performed, is an adequat

Evaluation of a proposed standard reporting system for preoperative angiograms in infrainguinal bypass procedures: angiographic correlates of measured runoff resistance.

Preoperative angiography is widely used to estimate runoff before peripheral vascular bypass surgery. The Ad Hoc Committee on Reporting Standards for the Society for Vascular Surgery and the International Society for Cardiovascular Surgery recently proposed an angiographic scoring system that grades the quality of vessels distal to the proposed bypass site and calculates an overall runoff score after adjusting each vessel score by an arbitrary weighting factor. We compared this score with intraoperatively measured runoff resistance in 67 patients undergoing infrainguinal bypass. Correlation be

Forces under the hallux valgus foot before and after surgery.

Abnormalities in the hallux valgus foot and changes after surgery were investigated by measuring the distribuiion of load on the foot in walking. Hallux valgus was associated with reduced load imposed on the toes, and on the medial side of the forefoot, compared with a large sample of healthy feet. Abnormalities correlated with the degree of the deformity. Both Keller's operation and a wedge displacement osteotomy of the first metatarsal not only failed to restore normal loading but increased the abnormalities of loading seen preoperatively. A large decrease in the angle between first and seco

What Are the Preoperative Outcome Measures That Predispose to Periprosthetic Fractures After Primary Total Knee Arthroplasty?

BACKGROUND: There is little known in the literature about whether preoperative patient-reported outcome measures (PROMs) would affect the risk of periprosthetic fractures (PPFs) after primary total knee arthroplasty (TKA). Our study aims to evaluate the predictive values of PROMs on PPF after primary TKA. We hypothesize that poorer PROMs are associated with a higher risk of PPF after primary TKA. METHODS: We reviewed prospectively collected data in our hospital arthroplasty registry. Patients who sustained PPF after primary TKA between 2000 and 2015 were identified. Forty-two patients were id

Patients' point of view on informed consent: a prospective study in carpal tunnel surgery.

Patients' perception of consent form (CF) is not well known and many patients tend to view the CF as an administrative act. As part of a prospective study, a questionnaire was sent to 188 consecutive patients within 1 month after carpal tunnel release. Questions focused on patients' recall about risks, benefits, alternative options, preferences about decisions process and global satisfaction with CF. Patient's understanding of the legal consequences of the CF was analysed. Risk's recall rate was 59%. CF reduced preoperative anxiety in 65% and the influence in patients' decision was relevant in

[Usefulness of preoperative lower limb venous ultrasound imaging in all gynecologic and obstetric surgical cases].

BACKGROUND: Perioperative venous thromboembolism (VTE) has been increasing in gynecologic and obstetric practice. Lower extremity venous ultrasound use in the detection of VTE has been studied and has proven both reliable and efficient. However, there have been very few reports of the preventive effect on VTE-related complication of preoperative lower limb venous ultrasonography in gynecologic and obstetric surgical cases. METHODS: Preoperative lower limb venous ultrasound imaging in gynecologic and obstetric surgical cases were performed to examine the presence or absence of deep vein thromb

Preoperative Guidelines for Coronary Artery Bypass Grafting for Health Education and Anxiety Management: Scoping Review.

INTRODUCTION: Patients in the preoperative period for coronary artery bypass grafting (CABG) frequently experience significant anxiety and depression, in addition to the physical symptoms necessitating the procedure. Preoperative health education is a crucial intervention to enhance patient knowledge, manage this psychological distress, and improve preparedness for the surgical process. OBJECTIVE: This scoping review aims to map the guidelines and strategies used for patient health education and anxiety management in the preoperative period for CABG. METHOD: A scoping review was conducted in…

A discrepancy in epiretinal proliferation detection between preoperative OCT and intraoperative observation in macular hole.

PURPOSE: Epiretinal proliferation (EP) is associated with poor visual and anatomical outcomes in eyes with macular holes (MHs). A preoperative cross-sectional OCT can detect EP; however, We observed that some cases with intraoperatively visible EP showed no evidence of EP on preoperative OCT. This study assessed the discrepancy between EP observed by OCT and during surgery. METHODS: We defined OCT-EP as epiretinal materials with homogeneous medium reflectivity observed in cross-sectional OCT. We defined surgical-EP as yellowish tissues contiguous to the hole during the membrane peeling. We de

Prognostic role of preoperative inflammatory markers in postoperative lung metastasis of colorectal cancer: a retrospective study.

BACKGROUND: This study aims to investigate the predictive significance of inflammatory markers for postoperative lung metastasis in colorectal cancer (CRC) patients. The focus is on exploring the relationship between traditional inflammatory markers, such as neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and newly introduced indices like C-reactive protein (CRP)-albumin-lymphocyte (CALLY), and their association with CRC progression and lung metastasis. METHODS: A retrospective review of medical records from 303 CRC patients at the First Affiliated Hospital of Nanjin

Preoperative handgrip strength can predict early postoperative shoulder function in patients undergoing arthroscopic rotator cuff repair.

BACKGROUND: Rotator cuff tears (RCTs) are a common musculoskeletal disorder, and arthroscopic rotator cuff repair (ARCR) is widely performed for tendon repair. Handgrip strength correlates with rotator cuff function; however, whether preoperative grip strength can predict functional outcomes in patients undergoing ARCR remains unknown. This study aimed to investigate the correlation between preoperative grip strength and postoperative shoulder function following ARCR. METHODS: A total of 52 patients with full-thickness repairable RCTs were prospectively enrolled. Baseline parameters, namely p

A Recipient and Donor Both Have COVID-19 Disease. Should We Perform a Liver Transplant?

Coronavirus 2019 (COVID-19) is a new infectious disease that continues to spread globally. There is growing concern about donor-induced transmission of Coronavirus 2 (SARS -CoV-2). For liver transplantation, the COVID-19 PCR test is routine, in addition to epidemiological history and clinical and radiological examination 24-48 h before surgery. One of the liver transplant candidates was found to be infected with COVID-19, as well as the planned donor candidate. Since COVID-19 will be a high-risk operation for both the recipient and the donor, the operation was postponed by giving medical treat
