Image evaluation of ophthalmic devices.

Methods used in the ophthalmic industry to evaluate the optical image quality of correcting lenses have remained the same over many years. This is especially true for single-vision ophthalmic lenses. With the development of new optical designs and materials for correcting refractive errors and presbyopia, new procedures for evaluating lens performance are being explored. One such method is the measurement of the modulation transfer function. Over the last 2 years, we have been measuring modulation transfer functions on a variety of single-vision and bifocal contact lenses, progressive addition…

The influence of age on the chronatic aberration of the eye.

The axial chromatic aberration of the eye was measured in 58 persons varying from 10 to 80 years of age. It was found that the chromatic aberration diminished significantly after the onset of presbyopia. It is suggested that this decrease in axial chromatic aberration may be due to increase in the index of refraction of the vitreous thereby producing a negative optical surface in the eye which more or less cancels the chromatic aberration of the cornea and anterior surface of the lens. This supposition is supported by measurements of chromatic aberration made on 10 aphakic patients. It is also…

Awareness and knowledge of common eye diseases among the academic staff (non-medical faculties) of University of Malaya.

A cross sectional study was conducted to assess the level of awareness and knowledge of common eye diseases (cataract, glaucoma, diabetic retinopathy and refractive errors) among 473 academic staff (non-medical faculties) of University Malaya. The awareness of cataract was in 88.2%, diabetic retinopathy in 83.5%, refractive errors in 75.3% and glaucoma in 71.5% of the study population. The knowledge about all the above common eye diseases was moderate, except presbyopia which was poor. Multivariate analysis revealed that females, older people, and those having family history of eye diseases we…

Age-related vision changes: a primary care guide.

Visual impairment becomes more prevalent with age. A person has "low vision" when legally blind or partially sighted. In addition, many older patients have functional visual impairment that affects their ability to read, even with corrective lenses. Normal visual changes that occur with aging include presbyopia, decreased contrast sensitivity, decreased dark/light adaptation, and delayed glare recovery. The four most prevalent age-related ocular diseases are macular degeneration, open-angle glaucoma, cataract, and diabetic retinopathy. The primary care practitioner can identify patients with v…

Visualization of the macula during elective pars plana vitrectomy in the presence of a dual-optic accommodating intraocular lens.

UNLABELLED: We describe the case of a 69-year-old patient who had pars plana vitrectomy (PPV) for epiretinal membrane removal 18 months after cataract surgery with implantation of a dual-optic accommodating IOL (Synchrony). During PPV, visualization of the macula was perfect, and the postoperative visual outcome was excellent. To our knowledge, this is the first report of retinal visualization during PPV in the presence of a dual-optic accommodating IOL. When choosing a presbyopia-correcting IOL, surgeons should consider the advantage of macula visualization with an IOL with monofocal optics. …

Lipids as pharmacological targets in age-related lens disease.

Cataract and presbyopia are the leading causes of age-related vision impairment worldwide, yet non-surgical management options remain limited. Age-related changes in the lipid composition of the crystalline lens have been implicated in their pathophysiology, highlighting the lens lipidome as a potential therapeutic target. In this review we summarize how recent advances in high-resolution and spatial lipidomics have clarified age- and region-specific changes in the lens lipidome, and we evaluate recent research efforts to utilize topical lipid-replenishing formulations and lipid-modifying smal…

Quantitative Evaluation of Human Lens and Lens Capsule Elasticity by Optical Coherence Elastography Based on a Rayleigh Wave Model.

Evaluating the biomechanical properties of the lens and lens capsule is important for the clinical diagnosis and treatment of age-related cataracts and presbyopia. In this study, we developed an optical coherent elastography technique to assess the elasticity of the lens and lens capsule in the human eye. With age, the mean Young's modulus of the lens increased from 12.28 ± 0.87 kPa to 18.59 ± 1.45 kPa, and the lens capsule increased from 6.33 ± 0.36 kPa to 13.33 ± 0.74 kPa. The results showed that the Young's modulus of the lens capsule and lens increased with age, with the Young's modulus of…

Defocus curves: focusing on factors influencing assessment.

Defocus curve assessment is used to emulate defocus over a range of distances and is a valuable tool that is used to differentiate the performance of presbyopia-correcting intraocular lenses. However, defocus curves are limited by a lack of standardization, and multiple factors can affect their generation and interpretation. This review discusses key factors that influence the assessment of defocus curves, including pupil size, level of contrast, sphere vs cylinder defocus, viewing distance, monocular vs binocular assessment, use of Snellen vs logMAR charts, and diopter range and step size. Mo…

Visualization of Light Propagation with Multifocal Intraocular Lenses Using the Ouzo Effect.

The number of presbyopia correcting intraocular lenses (IOLs) is increasing and new technologies are constantly emerging with the aim of correcting the loss of accommodation after cataract surgery. Various optical designs have been proposed to implement multifocality or an extended depth of focus (EDOF). Depending on the optical principle of an implanted lens, the visual performance often is deteriorated by superposition of individual image planes and halos of varying intensity. This experimental study presents a concept to visualize the light fields and especially the halos of mono- and multi…

Cataracts.

An estimated 95 million people worldwide are affected by cataract. Cataract still remains the leading cause of blindness in middle-income and low-income countries. With the advancement of surgical technology and techniques, cataract surgery has evolved to small-incisional surgery with rapid visual recovery, good visual outcomes, and minimal complications in most patients. With the development of advanced technology in intraocular lenses, the combined treatment of cataract and astigmatism or presbyopia, or both, is possible. Paediatric cataracts have a different pathogenesis, surgical concerns,…

Multifocal intraocular lenses: An overview.

Multifocal intraocular lenses are increasingly used in the management of pseudophakic presbyopia. After multifocal intraocular lens implantation, most patients do not need spectacles or contact lenses and are pleased with the result. Complications, however, may affect the patient's quality of life and level of satisfaction. Common problems with multifocal lenses are blurred vision and photic phenomena associated with residual ametropia, posterior capsule opacification, large pupil size, wavefront anomalies, dry eye, and lens decentration. The main reasons for these are failure to neuroadapt, l…

The mechanical response of the porcine lens to a spinning test.

The pig lens has been used as a model for presbyopia as pigs lack accommodative ability. Previous studies using microindentation have indicated that the shear modulus distribution is qualitatively similar to that of the aged human lens and that the lens does not alter its refractive power due to equatorial stretching. A lens spinning test was used to determine whether prior lens stiffness data obtained from a sectioned porcine lens were reliable and whether the testing conditions significantly influence the lens' mechanical properties. The elastic modulus distribution determined for fresh lens…

Reversal of a presbyopic LASIK treatment.

We report on a 72 year-old pseudophakic patient who had been implanted with a monofocal intraocular lens, and who underwent a unilateral Supracor laser-assisted in-situ keratomileusis (LASIK) procedure to correct presbyopia and hyperopia. Uncorrected near visual acuity was improved, but the patient complained of glare, halo, reduced distance vision, and poor night driving vision due to treatment decentration. One year following the surgery, a reversal procedure was conducted with a wavefront-guided aspheric treatment to reverse the presbyopic correction while still maintaining the hyperopic co…
