BACKGROUND: Prior allogeneic hematopoietic stem cell transplantation (HSCT) can complicate HLA and ABO typing of deceased organ donors because circulating hematopoietic cells and organs may originate from different sources.
CASE PRESENTATIONS: We describe two deceased organ donors with prior HSCT. In Case 1, historical records demonstrated discordance at HLA-C and HLA-DQ between the HSCT donor and the constitutional pre-HSCT typing. Peripheral-blood HLA typing reflected the HSCT donor, while a kidney blood-vessel specimen yielded four DPB1 alleles from mixed sources, permitting only a presump

Translational research using animal models is essential for addressing the clinical issues in organ transplantations. Although mouse orthotopic liver transplantation (LT) is a useful tool for investigating the mechanisms of liver regeneration, ischemia-reperfusion injury, and immune responses following LT, technical challenges in this procedure restrict its feasibility. The key aspects for successful mouse orthotopic LT are vascular reconstruction in recipients, including the suprahepatic inferior vena cava (SHIVC), portal vein (PV), and infrahepatic inferior vena cava (IHIVC). In addition to

Patients with the alpha actin 2 genetic mutation suffer early onset aneurismal and vascular-occlusive conditions due to dysfunctional smooth muscle contractility. Outcomes of free flap reconstruction in this patient population are unknown. Here we report the case of a 21-year-old woman with alpha actin 2 mutation who required decompressive hemicraniectomy following an acute stroke. The entire Cushing flap underwent necrosis, requiring debridement and exposing dura. This condition was treated with a free latissimus myocutaneous flap. The patient's post-operative course was complicated by venous

The kidney is subject to a large variety of injurious factors before, during, and after hematopoietic stem cell transplantation (HCT), leading to a high incidence of acute kidney injury in the peritransplant period. Chronic kidney disease is estimated to impact 15% to 20% of HCT recipients. Although renal biopsies may be deferred in the setting of thrombotic microangiopathy, acute self-limited impairment, or slowly progressive functional decline, in many patients renal biopsy yields important diagnostic insight to guide treatment. Light microscopic, immunofluorescence, and ultrastructural anal

In this study we present the first experimental vascularized penile allogenic transplantation model in the rat. In group 1 (n = 6), the vascular anatomy of the male rat perineal region was determined. In 12 Lewis rats of group 2 (n = 24), the penile composite graft was transplanted as isotransplant by microvascular anastomosis of the pedicle of the penis to the saphenous vessels and in another 12 Lewis rats of group 2, penile composite graft was transplanted without vascular anastomosis. In group 3 (n = 12), penis composite graft was transplanted from 6 Lewis-Brown-Norway donors to 6 Lewis rec

Acute polyhydramnios in the second trimester is a typical complication in monozygous twin pregnancies. It is caused by a feto-fetal transfusion with anemia on the donor and polycythemia on the recipient twin. Contrary to the chronic hydramnios, there is no increase in malformations. In view of the high mortality rate (100%, according to most authors), the clinical management has to be reconsidered. During the years 1979 to 1983, 10 cases of acute polyhydramnios have been observed at the University Hospital in Zurich. This corresponds to an incidence of 9% in our twin population. All cases inve

OBJECTIVE: Conventional methods for reconstructing multiple digital soft tissue defects often result in significant donor-site morbidity and suboptimal functional outcomes. This study aimed to evaluate an innovative application of the "like-with-like" split toe flap (STF) technique for single-stage reconstruction of multiple digits while optimizing functional and esthetic outcomes and preserving donor-site function.
METHODS: A prospective case series was conducted in six patients presenting with multiple digital soft tissue defects. Technical innovations included layered debridement, precisio

BACKGROUND: Complex lower limb injuries involving soft tissue defects, fractures, and vascular compromise present significant reconstructive challenges. Free tissue transfer is often the gold standard for reconstruction; however, reverse sural cross-leg flaps provide a reliable alternative in resource-limited settings or when the recipient's vessels are compromised. This case series highlights the utility of this technique in challenging scenarios, including comminuted fractures, vascular injuries, and extensive soft tissue defects.
METHODS: Six male patients with complex lower limb injuries

BACKGROUND: Chimeric flaps are often used in reconstructive fields for multiple defects, different functional defects, and extensive defects. In this article, we present the results of the use of thoracodorsal artery perforator (TDAp) chimeric flaps including a latissimus dorsi (LD) or serratus anterior (SA) muscle to prevent pedicle compression for lower extremity reconstruction.
METHODS: Nineteen TDAp chimeric flaps were used to prevent pedicle compression. Seven were female and 12 male. Patients' age ranged from 32 to 73 years. After harvesting TDAp skin flap, LD or SA muscle could be harv

OBJECTIVES: This study aimed to compare changes in coronary endothelial function, systemic endothelin-1 (ET-1) levels, and vascular remodeling in heart transplant recipients randomized to cyclosporin A (CyA) or tacrolimus (Tac) immunosuppression.
BACKGROUND: Functional endothelial abnormalities and intimal thickening are sensitive measures of early cardiac allograft vasculopathy (CAV).
METHODS: The randomized, prospective study was performed in two groups of 22 patients, maintained on Tac or CyA and mycophenolate mofetil immunosuppression, 1 and 12 months after heart transplantation. We inve

Flexible bronchoscopy is routinely utilized to monitor allograft function after lung transplantation (LT). While transbronchial lung biopsy (TBLB) is valuable as it permits direct histological assessment of the allograft, unexpected findings are occasionally encountered. We report a 72-year-old male with advanced chronic obstructive pulmonary disease who underwent a right single LT. His early posttransplant course was uneventful, and he presented for 6-week surveillance bronchoscopy. Although TBLB of the right lower lobe showed no evidence of rejection, one section revealed a small blood vesse

Deep burns sequelae involving the upper limb are challenging even for experienced surgeons, mainly because local reconstructive options and donor sites are often compromised. The use of free flaps for this type of reconstruction remains difficult due to the small recipient vessel diameter and tendency to vasospasm. Moreover, pediatric cases bring the challenge to another level. We present the case of a 13-year-old girl presenting major retractile sequelae of the upper left limb, including complete wrist immobilization combining wrist hyper-extension, ulnar deviation deformity, and a ulno-carpa

OBJECTIVE: Because of the complicated blood supply and vascular structure of the pancreas, blood vessel reconstruction and reshaping are generally required during pancreas transplantation. We modified the vascular preparation procedure for the donor pancreas (i.e., no vascular reconstruction was performed) based on experiences in our department and in other domestic and international transplantation centers.
METHODS: Twelve donor pancreas preparations without vascular reconstruction were performed. The patch (Carrel patch), celiac trunk, and superior mesenteric artery were preserved as arteri

Lymphedema is a chronic debilitating disease, affecting a considerable part of the population; it results from impairment of the lymphatic system. It is highly prevalent among patients subjected to axillary and groin nodal dissection after surgery for breast cancer, abdominopelvic surgery, and lymphadenectomy after melanoma surgery. Interestingly, among the surgical treatment options for lymphedema, groin lymph node transfer is gaining popularity; however, in some cases, dissection at this site can cause significant morbidity, including possible development of iatrogenic lymphedema. The impact of denervation and delymphatization (DD) on liver tissue remains a topic of major interest in liver transplantation, as the organ or its part continues to function well. Various chemical and/or surgical interventions have been developed to achieve in situ liver denervation and delymphatization, but it is evident that DD can be achieved only through "ectomia", ie, complete isolation of the organ from adjacent and connected structures. This Study describes a liver DD model based on a technique of autotransplantation (replantation), maximally preventing ischemia in the transplanted (re

OBJECTIVE: Neointimal hyperplasia is an inflammatory and proliferative process that occurs as a result of injury to the vessel wall. We have shown that the homeostatic protein A20 prevents neointimal hyperplasia by affecting endothelial cell (EC) and smooth muscle cell (SMC) responses to injury. In this work, we questioned whether A20 impacts other pathogenic effectors of neointimal hyperplasia including homing of monocyte/macrophages and EC/SMC precursors to the site of vascular injury, vascular endothelial growth factor (VEGF) secretion, and adventitial neovascularization.

New flow-through perforator flaps with a large, short vascular pedicle are proposed because of their clinical significance and a high success rate for reconstruction of the lower legs. Of 13 consecutive cases, the authors describe two cases of successful transfer of a new short-pedicle anterolateral or anteromedial thigh flow-through flap for coverage of soft-tissue defects in the legs. This new flap has a thin fatty layer and a small fascial component, and is vascularized with a perforator originating from a short segment of the descending branch of the lateral circumflex femoral system.
