Lymph vessels of the rectum in health and during early stages of metastasizing rectal cancer were studied. It was shown that changes occurring at this stage constitute the "cascade" process whose stages are determined by 3 structural elements of the rectal lymphatic vasculature, i. e. capillary, postcapillary, and vessel. Compensatory adaptive changes in the lymphatic routes in rectal cancer and in cases of secondary metastases to the rectum were examined.

The main indication for colon barium enema examination is occult bleeding or bleeding noticed by the patient himself. The radiologist has to take into account all clinical details that could lead to bleeding and to document them. The most frequent causes of rectal bleeding are hemorrhoids and diverticulosis/itis which in marked cases may have a tumor-like appearance. A good double-contrast examination of the colon must not only exclude tumors, polyps or inflammatory diseases but also demonstrate other causes of bleeding. The presented paper describes typical roentgen signs and gives criteria t

The authors report two cases of periarteritis nodosa associated with carcinoma. The first case was a man with an anaplastic bronchial carcinoma, the second was a woman with a rectal adenocarcinoma. Study of the association of certain collagen diseases with carcinoma was made to emphasize the great rareness of this association.

BACKGROUND: Advanced endoscopic resection techniques are used to treat colorectal neoplasms that are not amenable to conventional colonoscopic resection. Literature regarding the predictors of the outcomes of advanced endoscopic resections, especially from a colorectal surgical unit, is limited.
OBJECTIVE: To determine the predictors of short-term and long-term outcomes after advanced endoscopic resections.
DESIGN: Retrospective case series.
SETTINGS: Tertiary care center.
PATIENTS: Patients who underwent advanced endoscopic resections for colorectal neoplasms from November 2011 to August …

Metastases from colorectal cancer can occur either through the lymphatic or by hematogenous spread. The most common metastatic sites are the lung and liver. Nasopharyngeal metastasis from colorectal cancer has never been previously reported in the literature on the internet databases we can found. In this paper, we present the case of a 79-year-old male suffering from adenocarcinoma of the rectum with distant metastases to the liver, lung, and nasopharynx. Over the previous 7 years, he had received treatment for rectal cancer including radical surgery (miles surgery), chemotherapy, hepatectomy

A 59-year-old woman with a previous history of endometrial and breast cancer was referred to the colorectal rapid access clinic with abdominal cramps, per rectal bleeding and diarrhoea. CT, MRI and flexible sigmoidoscopy confirmed a rectal tumour and due to its appearance, it was thought to be a primary rectal malignancy. The patient underwent an anterior resection and end colostomy. Histology confirmed a metastatic endometrial adenocarcinoma, which is an unusual site for endometrial metastasis. Her postoperative recovery has been unremarkable and the patient has subsequently been discharged f

Recurrent disease after curative resection occurred in 37-44% of patients. The common sites of recurrent disease are at the anastomotic site, in the liver, in the lymph nodes, and in the peritoneal linings. Computed tomography (CT), barium enema, and endoscopy detected local recurrent disease in 61-88% of the patients, whereas magnetic resonance imaging (MRI) detected 80-88% of the cases reported in some Phase 1 studies. CT and MRI are equally effective in the detection of hepatic metastases when lesions are larger than 2 cm, with detectability rate of 95-100%. They are fair (50-60% detectabil

Local excision is increasingly performed for "early stage" rectal cancer in the US; however, local recurrence after local excision has become a controversial issue in Western countries. Local recurrence is considered to originate based on the type of tumor and procedure performed, and in surgical margin-positive cases. This review focuses on the inclusion criteria of "early" rectal cancers for local excision from the Western and Japanese points of view. "Early" rectal cancer is defined as T1 cancer in the rectum. Only the tumor grade and depth of invasion are the "high risk" factors which can

A 6-year-old girl with diffuse cavernous hemangiomatosis of the sigmoid colon, rectum and anus underwent endorectal pull-through operation for sphincter-saving resection. Rectal mucosa was resected from 4 cm above peritoneal reflection to anal skin margin and the normal sigmoid colon was pulled down through the rectal muscular cuff. Ligation of the superior rectal and left internal iliac arteries at operation achieved satisfactory control of bleeding. Postoperative manometric studies showed almost normal sphincter tone and good response to rectal stimuli by balloon inflation. The endorectal pu

BACKGROUND: Over the last two decades, sacral neuromodulation (SNM) has established its role in the treatment of functional pelvic organ-/pelvic floor disorders. Even though the mode of action is not fully understood, SNM has become the preferred surgical treatment of fecal incontinence.
METHODS AND RESULTS: A literature search was carried out on programming sacral neuromodulation and long-term outcomes in treating fecal incontinence and constipation.Sacral neuromodulation was found to be successful in the long term. Over the years, the spectrum of indications has expanded, and now includes p

BACKGROUND: A total mesorectal excision for rectal cancer-although nerve- and sphincter-sparing-can give rise to significant bowel symptoms, commonly referred to as low anterior resection syndrome (LARS). The exact pathophysiology of this syndrome still remains largely unknown, and the impact of radical surgery on colonic motility has only been scarcely investigated.
METHODS: High-resolution colon manometry was performed in patients, 12-24 months after restoration of transit. Patients were divided into two groups: patients with major LARS and no/minor LARS, according to the LARS-score. Coloni
