Plasma prostaglandin E during neurogenic hypertension in tetraplegic man.

1. Arterial blood pressure and heart rate were measured before, during and after cutaneous and visceral stimulation in subjects tetraplegic due to severe cervical spinal cord damage. Stimulation resulted in marked hypertension as a result of reflex sympathetic overactivity. 2. Mixed venous blood from a catheter situated in the right atrium was obtained before and after stimulation and plasma prostaglandin E and prostaglandin F were measured. A consistent and significant rise in plasma prostaglandin E, but not plasma prostaglandin F, accompanied the hypertension. 3. It is suggested that prostag

Case Report: Good Prognosis of Mixed Alien Hand Syndrome by Verbal-Cue Rehabilitation Exercise.

Mixed alien hand syndrome is a rare disease reported in the literature. The mixed callosal-frontal variant of alien hand syndrome is associated with uncoordinated hand movements, and patients may present with an involuntary grasp reflex and intermanual conflict. There are few videos in the existing literature on the comparison of patients' condition before and after recovery of the symptoms of mixed alien hand syndrome. We presented the prognosis of mixed alien hand syndrome in the form of a video. In addition, we have included some videos on the comparison of the condition of patients before

Cardioneuromodulation for Recurrent Malignant Neurocardiogenic Syncope in Pediatric Patients: Results from a Tertiary Center.

Malignant neurocardiogenic syncope (MNS) is defined as recurrent syncope with minimal prodromes associated with significant prolonged asystole during head-up tilt test (HUTT). Although cardioneuromodulation (CNM) is an emerging therapy for cardioinhibitory reflex syncope and other functional bradyarrhythmias and is increasingly considered an alternative to cardiac pacing in selected adult patients, evidence for its use in children remains limited. This study aims to describe the procedural characteristics and clinical outcomes of CNM in pediatric patients with MNS and to identify the clinical

[A case of post-surgical inflammatory neuropathy with lower-extremity weakness after surgery for anorectal malignant melanoma that showed the effectiveness of immunotherapy].

A 59-year-old man with past histories of bronchial asthma and chronic sinusitis underwent transanal resection of anorectal malignant melanoma with general anesthesia. On the third day after surgery, he presented with subacute weakness with right dominant hypoesthesia in the bilateral lower limbs. Tendon reflexes were diminished without pathological reflexes. Blood examination showed increased eosinophils (2,058/μl) and elevated serum immunoglobulin E (675.0 IU/ml). Cerebrospinal fluid examination showed elevated protein (200 mg/dl) without pleocytosis (<5/μl). A nerve conduction study suggeste

Electroacupuncture Attenuates Neuroinflammation in PCPA-Induced Insomnia Rats by Suppressing the TLR4/MyD88/NF-κB p65 Pathway and M1 Microglial Activation.

BACKGROUND: Insomnia is closely associated with neuroinflammation, yet the therapeutic mechanism of electroacupuncture (EA) remains unclear. This animal study investigated whether EA attenuates hypothalamic neuroinflammation in para-chlorophenylalanine (PCPA)-induced insomnia rats by suppressing the TLR4/MyD88/NF-κB p65 pathway and M1 microglial activation, using TAK-242 (HY-11109), a selective TLR4 inhibitor, as a positive control. METHODS: Forty Sprague-Dawley rats (20 male, 20 female, sex-balanced across groups) were randomly allocated to Control, Model, EA, and TAK-242 groups (n=10 per gr

The neuromuscular impact of symptomatic SMN restoration in a mouse model of spinal muscular atrophy.

BACKGROUND: Significant advances in the development of SMN-restoring therapeutics have occurred since 2010 when very effective biological treatments were reported in mouse models of spinal muscular atrophy. As these treatments are applied in human clinical trials, there is pressing need to define quantitative assessments of disease progression, treatment stratification, and therapeutic efficacy. The electrophysiological measures Compound Muscle Action Potential and Motor Unit Number Estimation are reliable measures of nerve function. In both the SMN∆7 mouse and a pig model of spinal muscular a

Etomidate-Induced myoclonus in Sprague‒Dawley rats involves the activation of neocortical Calpain-2 and its decrement on KCC2 protein.

BACKGROUND: Etomidate-induced myoclonus has become a pressing clinical problem with an incidence of 50-80%. The underlying mechanism involves neocortical glutamate accumulation and N-methyl-d-aspartate (NMDA) receptor activity. However, the therapeutic target remains uncertain. METHODS: Adult male Sprague-Dawley (SD) rats were injected with etomidate (1.5 mg/kg), propofol (11.8 mg/kg), and lidocaine (4.0 mg/kg) plus etomidate (1.5 mg/kg), etomidate (3.8 mg/kg), etomidate (6.0 mg/kg) through the tail vein and behavioral scores of the rats were recorded within 5 min after anesthesia to establis

Neurosyphilis Diagnosed on the Basis of Pupillary Abnormalities: A Case Report.

Neurosyphilis is a condition in which Treponema pallidum invades the central nervous system and may occur at any stage of syphilis. Early diagnosis and treatment are crucial, as advanced disease can result in locomotor ataxia and progressive paralysis. We report a case of neurosyphilis identified on the basis of pupillary abnormalities. A man in his 20s was referred to our hospital with difficulty with near vision in the right eye, after bilateral pupillary dilation had been detected during an eye examination for contact lens prescription at another clinic. Brain MRI revealed no abnormalities,

An unusual presentation of spontaneous spinal epidural hematoma.

We report a rare case of spontaneous spinal epidural hematoma. Various presentations may occur, most commonly including neck pain, interscapular pain, radicular pain, and paralysis. This condition is frequently associated with paralysis and long-term disability. This case is unique because it presented with right hand cyanosis, in addition to pain. A 69-year-old Caucasian female presented with cyanosis of the right hand, and severe right upper extremity pain which awakened her approximately 11 hours earlier. The pain was exacerbated on extending her head. The patient further reported bilateral

Reading Epilepsy and Language Processing: A Proposed Role for Semantic Salience.

Reading epilepsy is a relatively rare reflex epilepsy syndrome that typically presents as orofacial reflex myoclonus triggered by reading. Seizures are thought to be because of activation of hyperexcitable language-related pathways in the dominant hemisphere that subsequently spreads to adjacent motor cortices. More difficult reading tasks are thought to be more provocative of seizures regardless of semantic understanding of the text. The authors sought to better characterize the role of text difficulty and comprehension in triggering seizures in a 27-year-old patient with reading epilepsy. As

The Bezold-Jarisch reflex in acute inferior myocardial infarction: clinical and sympathovagal spectral correlates.

BACKGROUND: The cardiodepressor Bezold-Jarisch reflex (BJR) in acute inferior myocardial infarction (AMI) is traditionally considered as an indicator of successful thrombolysis. HYPOTHESIS: The study aim was to elucidate the role of the autonomic nervous system in the pathogenesis of a BJR response in patients with AMI by tracing spectral profiles of heart rate variability (HRV). METHODS: We studied 32 patients who presented with BJR after starting intravenous thrombolysis for an inferior AMI. Spectral components of HRV were analyzed over the three specific 5-min periods preceding and follow

Long-term follow-up after single toxic exposure to trichloroethylene.

In an earlier report [Feldman and Lessell, 1967], neurologic findings following acute intoxication to trichloroethylene were presented. Facial anesthesia, asymmetric pupillary responses, and electrical evidence of sensorimotor neuropathy accompanied neuropsychological deficits, manifested by difficulty in solving sequential problems and poor memory affecting the acquisition of new information. Twelve years after the initial exposure, patches of hypalgesia over the malar eminences persisted and corneal reflexes remained absent, although sensation in the snout region was totally normal. Neuropsy

Establishing critical differences in ear-canal stimulus amplitude for detecting middle ear muscle reflex activation during olivocochlear efferent measurements.

Objective: Assessments of the medial olivocochlear reflex (MOCR) may have clinical utility. The MOCR is measured using contralateral inhibition of otoacoustic emissions but concurrent activation of the middle ear muscle reflex (MEMR) confounds test interpretation. MEMR activation can be detected using the change in ear-canal stimulus amplitude without versus with an MOCR elicitor. This study provides a description of how critical differences in ear-canal stimulus amplitude can be established.Design: Clicks were presented in right ears without and with a contralateral MOCR elicitor.
