Derivation of the Buffalo Concussion Physical Examination risk of delayed recovery (RDR) score to identify children at risk for persistent postconcussive symptoms.

OBJECTIVE: The Buffalo Concussion Physical Examination (BCPE) is a brief, but pertinent physical examination designed for the subacute, outpatient assessment of concussion. The purpose of this study was to perform the BCPE on a larger sample and derive a scoring system to identify children at risk for Persistent Post-Concussive Symptoms (PPCS, recovery ≥30 days). METHODS: This prospective, observational cohort study from September 2016 to March 2019 was performed at three university-affiliated concussion clinics. Male and female children (n=270, 14.92±1.86 years, range 8-18, 38% female) were

Vestibular implant: does it really work? A systematic review.

INTRODUCTION: People with vestibular loss present a deficit in the vestibular system, which is primarily responsible for promoting postural control, gaze stabilization, and spatial orientation while the head moves. There is no effective treatment for a bilateral loss of vestibular function. Recently, a vestibular implant was developed for people with bilateral loss of vestibular function to improve this function and, consequently, the quality of life of these patients. OBJECTIVE: To identify in the scientific literature evidence that vestibular implants in people with vestibular deficit impro

Comparison of caloric and head-impulse tests during the attacks of Meniere's disease.

OBJECTIVES/HYPOTHESIS: To aid in diagnosis of Meniere's disease (MD) during the attacks using caloric and head-impulse tests (HITs). STUDY DESIGN: Retrospective case series review. METHODS: We analyzed the results of bithermal caloric and HITs during the attacks in 16 patients with MD. Quantitative analyses of HITs were conducted using a magnetic search coil technique. RESULTS: In unilateral MD (14 patients, 42 semicircular canals), the head impulse gain of the vestibulo-ocular reflex (VOR) was either normal (28 of 42, 67%), decreased (8 of 42, 19%), or increased (6 of 42, 14%) for each sem

The video head impulse test: diagnostic accuracy in peripheral vestibulopathy.

BACKGROUND: The head impulse test (HIT) is a useful bedside test to identify peripheral vestibular deficits. However, such a deficit of the vestibulo-ocular reflex (VOR) may not be diagnosed because corrective saccades cannot always be detected by simple observation. The scleral search coil technique is the gold standard for HIT measurements, but it is not practical for routine testing or for acute patients, because they are required to wear an uncomfortable contact lens. OBJECTIVE: To develop an easy-to-use video HIT system (vHIT) as a clinical tool for identifying peripheral vestibular defi

Postural and locomotor control in normal and vestibularly deficient mice.

We investigated how vestibular information is used to maintain posture and control movement by studying vestibularly deficient mice (IsK-/- mutant). In these mutants, microscopy showed degeneration of the cristae of the semicircular canals and of the maculae of the utriculi and sacculi, while behavioural and vestibulo-ocular reflex testing showed that vestibular function was completely absent. However, the histology of Scarpa's ganglia and the vestibular nerves was normal in mutant mice, indicating the presence of intact central pathways. Using X-ray and high-speed cineradiography, we compared

Three-dimensional analysis of human locomotion in normal subjects and patients with vestibular deficiency.

The purpose of this study was to clarify the role of the vestibular system in human locomotion. The subjects were nine healthy controls, nine patients with unilateral vestibular deficiency (UVD) and nine patients with bilateral vestibular deficiency (BVD). The UVD subjects were Ménière's disease patients who were being treated with administration of gentamicin into the tympanic cavity. BVD subjects were hearing-impaired individuals who showed no response to the ice-water caloric test. A total of 13 markers were attached to the head, trunk (C7), hip and foot in order to measure translational an

Abnormal visuo-vestibular interactions in vestibular migraine: a cross sectional study.

Vestibular migraine is among the commonest causes of episodic vertigo. Chronically, patients with vestibular migraine develop abnormal responsiveness to both vestibular and visual stimuli characterized by heightened self-motion sensitivity and visually-induced dizziness. Yet, the neural mechanisms mediating such symptoms remain unknown. We postulate that such symptoms are attributable to impaired visuo-vestibular cortical interactions, which in turn disrupts normal vestibular function. To assess this, we investigated whether prolonged, full-field visual motion exposure, which has been previous

[Steele-Richardson-Olszewski syndrome].

BACKGROUND: Basal ganglia diseases with an akinetic-rigid symptom manifest like a Parkinson disease or an atypical Parkinson syndrome. Their symptoms are similar and therefore a diagnosis often is only possible after long-standing care of the patient. In the class of the atypical Parkinson syndromes is the progressive supranuclear palsy (PSP), which is also known as Steele-Richardson-Olszewski syndrome. CASE REPORT: A seventy-year-old patient who reported unaccountable sudden drops and disturbances of balance as well as diplopia in near vision was examined and a retrospective analysis of the

Search-coil head-thrust and caloric tests in Ménière's disease.

CONCLUSIONS: Our findings suggest that canal function is substantially preserved in subjects with active vertigo attacks as a result of Ménière's disease (MD). In these subjects, the head-thrust test (HTT) may not be as sensitive to canal dysfunction as traditional caloric testing. MD may differentially affect the low-frequency sensitivity of the canals. OBJECTIVE: Caloric tests have traditionally been used to characterize semicircular canal function in vestibular disorders, including MD. The quantitative HTT provides an objective measurement of semicircular canal function in the frequency an

Processing of visual motion direction in the fronto-parallel plane in the stationary or moving observer.

To examine the effect of concurrent self-motion on the perception of the direction of object-motion, random-dot kinematograms were employed in which the strength of the directional signal was manipulated by varying the percentage of coherently moving pixels. The subject's task was to indicate the motion direction of briefly presented displays while undergoing whole body rotations with angular accelerations of 0, 5, 15, or 45 degrees/s2. The perception of the direction of visual motion in the horizontal plane was impaired only when visual and vestibular motion directions were incongruous. The i

Neurootologic findings in patients with so called Menière-like disease.

A group of 150 patients was clinically observed using a complete battery of neurootological methods. The patients were believed to suffer from Menière's disease (MD). After taking anamnestic data [NODEC III] and performing calorimetry, cranio-corpo-graphy and audiology tests we could confirm a former established diagnosis of MD in a few cases only.

Videonystagmoscopy: its use in the clinical vestibular laboratory.

Vestibular function of a population including labyrinthine-defective patients and a control group of age-matched normal healthy volunteers was evaluated using videonystagmoscopy. This device is made of one or two CCD infra-red cameras mounted on diving glasses and allows observation of ocular movements on a video monitor and/or recording on a videotape. Eye movements are observed after rotations in a Bárány chair and during passive head tilts. With this simple and non-invasive test, a screening of vestibular function at bedside or during ENT clinical investigations can be performed. A further

Motion sickness severity and physiological correlates during repeated exposures to a rotating optokinetic drum.

Fifty-two subjects were exposed to a rotating optokinetic drum. Ten of these subjects who became motion sick during the first session completed two additional sessions. Subjects' symptoms of motion sickness, perception of self-motion, electrogastrograms (EGGs), heart rate, mean successive differences of R-R intervals (RRI), and skin conductance were recorded for each session. The results from the first session indicated that the development of motion sickness was accompanied by increased EGG 4-9 cpm activity (gastric tachyarrhythmia), decreased mean successive differences of RRI, increased ski

Vestibular recovery and central compensation after acute unilateral vestibulopathy - changes in saccadic response patterns and gains over time.

INTRODUCTION: Acute unilateral vestibulopathy (AUVP), or vestibular neuritis, disrupts the vestibulo-ocular reflex (VOR), leading to impaired gaze stability during head movements. Vestibular function typically recovers to some extent. However, if recovery is incomplete, corrective eye saccades must compensate for the deficient VOR function. The aim was to analyze the progression of VOR gain recovery and development of different corrective eye saccade patterns after AUVP. METHODS: A prospective longitudinal study was performed, including 43 patients with AUVP, that were examined using the vide

Evaluation of the Clinical Utility of Monothermal Caloric Testing in Comparison With the Video Head Impulse Test in Patients With Vestibular Neuritis.

BACKGROUND: The monothermal caloric test (MCT) is a simplified alternative to the traditional bithermal caloric test for evaluating vestibular function. However, its diagnostic correlation with video head impulse test (vHIT) parameters, particularly in acute vestibular neuritis (VN), has not been thoroughly established. METHODS: This retrospective study included 32 patients with acute VN who underwent bithermal caloric testing and vHIT within 5 days of symptom onset. Caloric responses were analyzed to determine unilateral weakness (UW) and monothermal caloric asymmetry (MCA) for warm and cold
