A two-year-old, male, crossbreed dog was presented three days after being involved in a motor vehicle accident. Survey radiographs showed multiple pelvic fractures and poor intraperitoneal and retroperitoneal contrast. Ultrasound indicated the left kidney to be hypoperfused, and a thrombus was visible at the origin of the left renal artery. Ultrasound-guided fine needle aspiration confirmed that free fluid visible in the retroperitoneal and peritoneal spaces was blood. No improvement was seen in the renal perfusion over a 43 hour period, and the intestinal hypomotility worsened over this time.

Accessory spleen is a rare anomaly occurring in approximately 10% of people. Most often, accessory spleen can be located in spleen gate. However, localization in parenchymal abdominal organs, retroperitoneal space, pelvis and chest is also possible. Congenital accessory spleen is extremely rare, and only several cases are described in the literature. Diagnosis causes some difficulties. CT-based differentiation with malignant tumor is difficult. We present a 72-year-old patient with congenital intrathoracic accessory spleen.

A 30-year-old man with peripheral neuroepithelioma which was present in the retroperitoneum underwent surgical treatment under the diagnosis of germ cell tumor. Subsequently he died of local recurrence and multiple metastases to the liver seven months after surgery. To our knowledge, this is the second case of peripheral neuroepithelioma which was observed in the retroperitoneal space in the literature of the world. We also reviewed 20 cases of peripheral neuroepithelioma reported in Japan.

During a 13-month period, 1,344 patients underwent extracorporeal shock wave lithotripsy. Plain x-rays done routinely 24 hours after lithotripsy to assess stone fragmentation revealed evidence of retroperitoneal air in 6 patients. We report a case of pneumoperitoneum at the end stage of lung cancer. The patient was an 81-year-old man who experienced only mild abdominal pain and fullness. The pneumoperitoneum spontaneously disappeared. Ruptured viscera and peritonitis were not observed at autopsy. The pancreas is almost impossible to palpate because it is located in the retroperitoneal space. Life-threatening lesions, therefore, often are not diagnosed until they are very advanced and encroaching on adjacent structures. The pancreas has a large endocrine and exocrine reserve, which also contributes to difficulty in diagnosing pancreatic disease. A system for the assessment of patients with multiple injuries is presented in which the torso is divided into three regions: (a) the great vessels, heart and esophagus, (b) the intrathoracic abdomen and (c) the pelvis and retroperitoneal space. The three areas of abdominal injury were subdivided into individual organs likely to be the site of serious injury. This system has been of great value in arousing suspicion of injury to specific organs that might otherwise have remained hidden.
