During a retrosigmoid (or combined retrolabyrinthine-retrosigmoid) approach to the posterior fossa for vestibular neurectomy or removal of small acoustic neuromas, a white dural fold is a consistent landmark to cranial nerves VII through XII. This fold of dura appears as a white linear structure extending from the foramen magnum across the sigmoid sinus, attaching to the posterior aspect of the temporal bone, anterior to the vestibular aqueduct. The name "jugular dural fold" is suggested for this landmark. The jugular dural fold overlies the junction of the sigmoid sinus and the jugular forame

This report describes in a stepwise fashion the surgical anatomy of an approach to the midline and lateral compartments of the skull base (clivus, infralabyrinthine/infratemporal regions). The salient features of this procedure are represented by a mandibulotomy and by detachment of the pharynx from the skull base through a combined oral and cervical approach. There is full neurovascular control of the internal carotid artery and lower cranial nerves with the possibility of complete exposure of the intrapetrous and intracavernous segments of the internal carotid artery on the side of the expos…

We studied 11 patients with prostate cancer metastatic to the base of skull that caused cranial nerve deficits. Patients with occipital condyle, jugular foramen, middle fossa, parasellar, and orbital syndromes are described. Other patients had combinations of these syndromes or other cranial nerve involvements. Two patients had 6th nerve palsies secondary to prepontine cistern and clivus lesions. The median survival time from the diagnosis of cranial nerve involvement was 4 months. Two patients had cranial nerve involvement and, on subsequent investigation, were found to have carcinoma of the

Children with Down's syndrome have a higher incidence of middle ear effusion than normal children. Twenty-eight patients with Down's syndrome and 33 age and sex-matched normal controls were studied. They were subdivided into 2 further matched groups on the basis of having normal or impaired hearing. Each underwent pneumatic otoscopy, audiometry and lateral head and neck radiography. This study examines radiological parameters to determine whether skeletal or soft tissue factors within the skull base or nasopharynx could be implicated as a cause or diagnostic feature of Eustachian tube dysfunct…

Severe crushing head injuries can produce massive posterior epistaxis as a result of injury to the internal carotid artery at the skull base. We describe two such cases with traumatic pseudoaneurysm of the internal carotid therapy. They were managed with detachable balloon embolization. The anatomy of the internal carotid artery is reviewed. The diagnosis of pseudoaneurysms and their incidence, clinical presentation, and management are discussed.

The transcochlear approach is described for resection of lesions arising anterior or medial to the internal auditory canal as well as for those arising directly from the clivus. Through an extended complete mastoidectomy the facial nerve is totally decompressed and re-routed posteriorly from the stylomastoid foramen to the internal auditory canal. The fallopian canal, promontorium, and cochlea are removed anteriorly and medially as far as the internal carotid artery, obtaining exposure to a triangular area limited by the superior petrosal sinus, inferior petrosal sinus, carotid, and internal a

Necrotising otitis externa (NOE) is a life-threatening infection of the external auditory canal with skull base involvement. We report a case of Citrobacter koseri skull base osteomyelitis with NOE-like features, presenting three months following skull base surgery. A 69-year-old man underwent a left lateral temporal bone resection and pinnectomy with thoracodorsal artery perforator flap reconstruction for extensive basal cell carcinoma. Interval computed tomography and magnetic resonance imaging demonstrated enhancement deep to the flap with skull base and nasopharyngeal involvement, raising …

Delayed cerebral radiation necrosis (CRN) is a condition characterized by fibrinoid necrosis, blood-brain barrier disruption, and demyelination due to endothelial cell and oligodendrocyte injury as a result of radiation therapy. The presentation is variable as neurologic manifestations depend on the site of the lesion. This case report describes a rare presentation of delayed CRN years after diagnosis and treatment of osteoradionecrosis of the skull base. It highlights the importance of keeping this diagnosis on the differential for patients with a history of radiation therapy to the head or n…

Phosphaturic mesenchymal tumours (PMTs), which are common causes of tumour-induced osteomalacia (TIO), are exceptionally uncommon in the sinonasal tract and skull base. PMTs are characterized by their nonspecific location, small size, and indolent growth. When they occur in the sinonasal region and skull base, they often lack typical sinonasal symptoms, which poses significant diagnostic challenges and contributes to high rates of missed and misdiagnosis. A retrospective analysis was conducted on 5 patients with pathologically confirmed sinonasal/skull base PMTs treated at our institution from…

BACKGROUND: The pedicled nasoseptal flap (NSF) for skull base reconstruction has transformed the management of anterior skull base pathology. Despite its widespread adoption in adults and older pediatric patients, its application in infancy and younger children remains limited. Furthermore, its use in this population in low-resource settings is rarely reported.
METHODS: We present a surgical technique using simple instrumentation for the successful NSF reconstruction of a congenital anterior cranial fossa meningoencephalocele and cerebrospinal fluid leak in an infant in Accra, Ghana, which ha…

Necrotizing external otitis (NEO) is a rare but serious disease of the skull base (osteomyelitis). Immunosuppressed, and elderly patients are most commonly affected. The most common pathogen is Pseudomonas aeruginosa. Early diagnosis is crucial for the prognosis. To establish the diagnosis, both positive imaging findings, and clinical criteria are required to be met. Imaging is used during follow-up to monitor a potential recurrence due to its high risk, even after successful treatment. The use of radiological or nuclear medicine methods (scintigraphy) in the management of NEO is not currently…

This report describes the case of a 65-year-old male who developed Escherichia coli (E. coli) meningitis after endoscopic resection of a sinonasal neuroblastoma, complicated by a cerebrospinal fluid (CSF) leak, pneumocephalus, and subsequent concurrent infection with Actinomyces odontolyticus. The patient underwent prolonged, tailored antibiotic therapy and surgical CSF leak repair. The report highlights the importance of suspecting E. coli meningitis in post-neurosurgical patients, emphasizes the need for prompt management of CSF leaks, and underscores the need for vigilance regardless of imm

PURPOSE: In the literature, the number of studies on cases of injury due to isolated head trauma is limited. In addition, there are very few studies and publications on skull base and vault fractures in the forensic literature. This study aims to investigate the frequency and pattern of skull base and vault fractures in individuals exposed to isolated fatal and non-fatal head trauma.
METHODS: This retrospective cross-sectional and descriptive study includes 1260 cases who were injured due to isolated head trauma during the 10-year period between Jan 1, 2010 and Nov 30, 2019. In cases of fatal…

BACKGROUND: Skull base osteomyelitis (SBO) is a severe disease not only because of its rapid progression and its high mortality: diagnosis and treatment are often protracted and in more than 30% of cases no causative pathogen can be detected. SBO is usually preceded by immunodeficiency, which is why opportunistic infections caused by atypical pathogens must also be taken into consideration. In consideration of the different possible entities, an interdisciplinary approach with surgical debridement, pathological sampling, microbiological testing and antimicrobiological therapy is indispensable.…

Genetic testing based on family history and tumor type may miss numerous germline mutations. This case report describes an adolescent girl with a rare skull-base meningioma. Initial imaging suggested schwannoma but germline genetic testing with schwannomatosis panel (NF2, LZTR1, and SMARCB1) was negative. A stepwise surgical approach prioritized facial nerve preservation, followed by tumor removal. Histopathology identified the tumor as a meningothelial meningioma, WHO grade 1, rather than the initially suspected schwannoma. Comprehensive tumor-normal sequencing revealed a germline splice site
