Patterns of spontaneous saliva swallowings during awake and sleep states in Parkinson's disease.

OBJECTIVE: This study aimed to evaluate spontaneous swallowing frequency (SS-FR) and type of swallowing during awake and sleep states in patients with Parkinson's Disease (PD), using polygraphic recordings to identify possible associations with disease severity. METHODS: A total of 27 PD patients (19 male, 8 female) and 22 age-matched healthy controls were included. All participants underwent whole-night one-hour polygraphic monitoring, and SS-FR and type of swallowing were analyzed during both wakefulness and sleep stages. Clinical characteristics, disease severity (Hoehn and Yahr staging),

The metabolic topography of essential blepharospasm: a focal dystonia with general implications.

OBJECTIVE: To determine the metabolic topography of essential blepharospasm (EB). BACKGROUND: EB is a cranial dystonia of unknown etiology and anatomic localization. The authors have used 18F-fluorodeoxyglucose (FDG) and PET with network analysis to identify distinctive patterns of regional metabolic abnormality associated with idiopathic torsion dystonia (ITD), as well as sleep induction during PET imaging to suppress involuntary movements, thereby reducing this potential confound in the analysis. METHODS: Six patients with EB and six normal volunteers were scanned with FDG-PET. Scans were

Short-Term Therapeutic Effect of Repetitive Transcranial Magnetic Stimulations of Sleep Disorders in Parkinson's Disease: A Randomized Clinical Trial (Pilot Study).

This study aimed to evaluate the efficacy of rTMS in treating sleep disorders in PD. It included 24 patients with PD who had sleep disorders. Group allocations (active or sham with a ratio of 2:1) were placed in serially numbered closed envelopes. Each patient was evaluated with the following: MDS-UPDRS, Parkinson's Disease Sleep Scale (PDSS), Beck Depression Inventory (BDI), and polysomnography (PSG) before and 10 days after the treatment sessions. Each session consisted of 10 trains, 20 Hz, 10 sec for each, over the parietal cortex (bilaterally). Scores of UPDRS, BDI, and PDSS improved signi

Sleep disorders and late-onset epilepsy of unknown origin: Understanding new trajectories to brain amyloidopathy.

The intertwining between epilepsy, sleep disorders and beta amyloid pathology has been progressively highlighted, as early identification and stratification of patients at high risk of cognitive decline is the need of the hour. Modification of the sleep-wake activity, such as sleep impairment or excessive daytime sleepiness, can critically affect cerebral beta amyloid levels. Both mice models and human studies have demonstrated a substantial increase in the burden of beta amyloid pathology after sleep-deprivation, with potential negative effects partially restored by sleep recovery. The accumu

[Drowsiness and traffic accidents. Importance of diagnosis].

"Tiredness", often cited in civil and penal responsibilities secondary to car accidents, hides neurophysiological phenomena which must now be taken into account. The problems of watchfulness in car-driving, is sleepiness, are indeed linked to a genuine disorder in the sleep-wake rythm and attention should be drawn to them in medico-legal cases. Investigations and clinical examinations concerning 110 experts' reports confirm the role played by sleep' disorders in the occurrence of car accidents. Prevention in actually possible. Adolescent sleep defects and dopaminergic hyperactivity in mice with a schizophrenia-linked Shank3 mutation.

Shank3 is a shared risk gene for autism spectrum disorders and schizophrenia. Sleep defects have been characterized for autism models with Shank3 mutations; however, evidence has been lacking for the potential sleep defects caused by Shank3 mutation associated with schizophrenia and how early in development these defects may occur. Here we characterized the sleep architecture of adolescent mice carrying a schizophrenia-linked, R1117X mutation in Shank3. We further employed GRABDA dopamine sensor and fiber photometry to record dopamine release in the nucleus accumbens during sleep/wake states.

Mechanism of sleep-induced periodic breathing in convalescing stroke patients and healthy elderly subjects.

Cerebral vascular ischemic strokes are known to precipitate Cheyne-Stokes periodic breathing. Interestingly, Cheyne-Stokes-like breathing during sleep may be associated with obstructive sleep apnea (OSA) in some individuals. Therefore, it was reasoned that stroke patients with periodic breathing in sleep would be susceptible to OSA. Because oscillations in upper airway resistance can occur as a component of sleep-induced periodic breathing, we hypothesized that stroke patients with sleep-induced periodic breathing would have oscillations in upper airway resistance. These oscillations in resist

Daily variation in sleep characteristics in individuals with and without post traumatic stress disorder.

BACKGROUND: Sleep disturbances are common in individuals with post traumatic stress disorder (PTSD). However, little is known about how daily variation in sleep characteristics is related to PTSD. This study examined the night-to-night and weekday versus weekend variation in sleep duration, sleep quality, trouble falling asleep, and difficulty staying asleep in individuals with and without PTSD. METHODS: Participants (N = 157; 80 with PTSD, 77 without PTSD) completed daily self-reports of their nighttime sleep characteristics for 15 consecutive days. Linear mixed models were used to examine t

Abnormal sleep and sleepiness in Parkinson's disease.

PURPOSE OF REVIEW: Sleep problems are frequent and disabling in patients with Parkinson's disease. Recent data provide major advances in the mechanisms and consequences of rapid eye movement sleep behavior disorders, insomnia and narcolepsy-like daytime sleepiness. RECENT FINDINGS: A large series confirms that rapid eye movement sleep behavior disorders may precede parkinsonism or dementia (particuarly, but not exclusively, Lewy bodies dementia) for several years. In Parkinson's disease, rapid eye movement sleep behavior disorders expose patients to higher risks of dementia and hallucinations

Brainstem control of sleep, wakefulness, alertness, and consciousness.

The brainstem is crucial for maintaining wakefulness, sleep regulation, and consciousness. It orchestrates alertness through the ascending reticular activating system, integrating sensory information and projecting to the cortex. Consciousness emerges from intricate interactions involving the brainstem, though the brainstem alone does not generate consciousness. Clinical syndromes resulting from brainstem lesions highlight distinctions between wakefulness, alertness, and consciousness. Disorders such as sleep paralysis, parasomnias, vegetative state, minimally conscious state, coma, and locked

Connectomics of orexin-producing neurons: interface of systems of emotion, energy homeostasis and arousal.

Avoiding danger and finding food, which are life-sustaining activities that are regulated by emotion, reward and energy balance, require proper wakefulness. The orexin system controls sleep and wakefulness through interactions with systems that regulate emotion, reward and energy homeostasis. Recent findings have brought about the possibility of novel therapies targeting the orexin system for sleep disorders, including insomnia and narcolepsy-cataplexy, as well as other pathological conditions such as obesity and drug addiction [1]. In this review, we will discuss the current understanding of

Orexins: from neuropeptides to energy homeostasis and sleep/wake regulation.

The neuropeptides orexin A and orexin B (also called hypocretin 1 and 2) were recently discovered by a "reverse pharmacology" approach as ligands for two previously orphan G protein coupled receptors: orexin receptors 1 and 2. Neurons producing orexins are located exclusively in the lateral hypothalamic area but project broadly to various parts of the brain, and they have been implicated in the control of energy homeostasis and arousal maintenance. The orexin receptors are also broadly expressed in the central nervous system. Murine and canine models suggest that defective signaling in the ore

The Impact of Antidepressant Medications on Sleep in Children: A Narrative Review.

Antidepressants are increasingly prescribed for pediatric patients to treat a vast array of mental health conditions, including anxiety disorders, depressive disorders, and obsessive-compulsive disorder (OCD). While there is extensive research supporting the efficacy of antidepressants in managing these conditions in the pediatric population, their effects on sleep, an essential component of overall well-being, remain a critical area of interest. This review explores the clinical effects on sleep disorders and the impact of antidepressants on sleep architecture and overall sleep quality in chi

Echoes in the night: How sleep quality influences auditory health.

The intricate relationship between sleep disorders and hearing loss emerges as a burgeoning field of scholarly inquiry. Numerous studies have illuminated a potential correlation between the two, affecting the quality of life and overall health of individuals. Hearing loss, or auditory impairment, serves as a critical indicator of physiological dysfunction, casting a pall over the daily existence and professional endeavors of those affected, potentially leading to irreversible deafness if left untreated. Sleep disorders may cause physical and psychological changes that further affect hearing, w

[Sleep disturbances--aspects of obstructive and sleep-related breathing disorders].

Obstructive sleep apnea (OSA) is a wide spread affliction in industrialised countries. OSA has to be distinguished from simple snoring which is socially annoying but physically harmless, since it does not harm the physical health. Patients with OSA complain about disruptive snoring, daytime sleepiness, and loss of intellectual power. OSA is associated with increased mortality. It is possible to distinguish between complete respiratory pauses or apnoea and periods of too shallow breathing or hypopnoea. History taking, clinical examination, recording of daytime sleepiness, and cardio-respiratory

Sleep and stroke.

More than 50% of stroke patients have sleep-disordered breathing (SDB), mostly in the form of obstructive sleep apnea (OSA). SDB represents both a risk factor and a consequence of stroke. The presence of SDB has been linked with poorer long-term outcome and increased long-term stroke mortality. Continuous positive airway presure is the treatment of choice for OSA. Oxygen and other forms of ventilation may be helpful in other (e.g., central) forms of SDB. SDB can improve spontaneously after stroke. About 20 to 40% of stroke patients have sleep-wake disorders (SWD), mostly in form of insomnia, e

Sleep disturbances in Parkinsonism.

The present article is meant to suggest an approach to the guidelines for the therapy of sleep disturbances in Parkinson's Disease (PD) patients. The factors affecting the quality of life in PD patients are depression, sleep disturbances and dependence. A large review of the literature on sleep disturbances in PD patients, provided the basis for the following classification of the sleep-arousal disturbances in PD patients. We suggest a model based on 3 steps in the treatment of sleep disturbances in PD patients. This model allowing the patient, the spouse or the caregiver a quiet sleep at night
